Minimally Invasive Modified Trans-zygomatic Approach for Enucleation of Asymptomatic Infra-temporal Fossa Schwannoma: A Report of Three Cases and a Review of Literature.

ObjectiveSchwannoma located in the infratemporal fossa (ITF) is a rarely encountered clinical condition, with symptoms that may be minimal. However, ITF is very difficult to reach. This work summarizes our experience with the surgical enucleation of ITF schwannoma using a modified trans-zygomatic approach, with minimal postoperative complications.MethodsA retrospective review of ITF schwannoma cases treated at our tertiary medical institution between August 2019 and November 2024 was undertaken. Data were retrieved from case notes and analyzed.ResultsIn total, three cases were included: two ma

Transorbital management of selected spheno-orbital meningiomas: Insights from a multicentric retrospective Italian study of 52 cases.

Spheno-orbital meningiomas (SOMs) are a rare type of intracranial tumor. Their location and behavior often make radical resection unfeasible. Recently, less invasive techniques have gained traction. The endoscopic transorbital approach (TOA), sometimes combined with lateral orbital rim removal or additional routes, offers a flexible surgical corridor for selected SOMs. This study evaluates the outcomes of TOA in SOM patients across five Italian tertiary centers, focusing on feasibility, safety, symptom relief, and tumor control. A retrospective multicenter study was conducted from January 2012

Jugular Foramen Paragangliomas.

Paragangliomas are the most common tumors at jugular foramen and pose a great surgical challenge. Careful clinical history and physical examination must be performed to adequately evaluate neurological deficits and its chronologic evolution, also to delineate an overview of the patient performance status. Complete imaging evaluation including MRI and CT scans should be performed, and angiography is a must to depict tumor blood supply and sigmoid sinus/internal jugular vein patency. Screening for multifocal paragangliomas is advisable, with a whole-body imaging. Laboratory investigation of endo

Surgical management of tympanojugular paragangliomas using the flexible CO2 laser.

PURPOSE: Surgery is a standard therapy for tympanojugular paragangliomas (TJP). Maintaining the quality of life (QoL) requires functional preservation. The flexible CO2 laser allows contact-free tumor removal. This retrospective study compares the postoperative functional outcomes of TJP surgery with and without the flexible CO2 laser. METHODS: Between 2005 and 2019, 51 patients with TJP were surgically treated at a tertiary hospital. Until 2012, 17 patients received conventional surgery. Thereafter, the flexible laser was used in 34 patients. Tumor extend, pre- and postoperative cranial nerv

Resection of Condylar Skull Base Tumor via Combined Far Lateral and Infrajugular Approaches with Single-Stage Occipitocervical Fusion.

Epithelioid hemangioma is a rare vascular mesenchymal tumor with a paucity of reports of cranial involvement. In particular, guidance on treatment for lateral skull base lesions is lacking, despite this being a highly technically challenging location. Nuances in the management decisions for this tumor type are discussed. Two major challenges with this location are proximity to critical neurovascular structures and managing secondary craniocervical instability. We present a patient with a lateral skull base epithelioid hemangioma treated with transcondylar resection, single-stage occipitocervic

Giant Cholesteatoma: A Case Report and Review of the Literature.

OBJECTIVE: To present a case of giant cholesteatoma and review of the contemporary literature. PATIENTS: A 61-year-old male presented with foul-smelling discharge from his right ear in the setting of a previous canal wall down mastoidectomy. Otomicroscopy demonstrated a mastoid cavity filled with infected keratinous debris. Neuro-imaging revealed soft tissue inflammatory changes in the mastoid cavity, with involvement of the temporomandibular joint (TMJ) and parotid gland and tegmen erosion. Biopsies showed evidence of desquamated epithelium consistent with cholesteatoma, with no malignant ce

Functional Petrosectomy Via a Suboccipital Retrosigmoid Approach: Guidelines and Topography.

OBJECTIVE: Recent reports have validated the use of retrosigmoid approach extensions to deal with posterior fossa lesions extending laterally extracranially or superiorly into the petroclival areas. The purpose of our research is to describe the topographic retrosigmoid anatomy of the petrous pyramid and provide guidelines for neurovascular sparing drilling (hence for a functional petrosectomy), via this surgical route. METHODS: Suprameatal and inframeatal retrosigmoid approach extensions were performed bilaterally in 6 specimens in the semisitting position. Topographic relationships of perti

Management of advanced intracranial intradural juvenile nasopharyngeal angiofibroma: combined single-stage rhinosurgical and neurosurgical approach.

BACKGROUND: Although intracranial extension of juvenile nasopharyngeal angiofibroma (JNA) occurs commonly, intradural penetration is extremely rare. Management of such tumors is a challenging issue in skull-base surgery, necessitating their removal via combined approaches. In this work, we share our experience in management of extensive intradural JNA. METHODS: In a university hospital-based setting of 2 tertiary care academic centers, retrospective chart of 6 male patients (5 between 15 and 19 years old) was reviewed. Patients presented chiefly with nasal obstruction, epistaxis, and proptosi

Combination antifungal therapy for invasive aspergillosis: can it replace high-risk surgery at the skull base?

OBJECTIVE: The increasing volume of literature on Aspergillus sinus infection is confusing because different authors stress different aspects of the disease. It is generally accepted that standard therapy consists of surgical debridement and medication with systemic amphotericin B. Patients who fail the standard therapy or those who cannot tolerate amphotericin B pose a dilemma. This study attempted to address the issues concerning combination therapy in invasive aspergillosis. MATERIALS AND METHODS: We conducted a prospective randomized study on 6 apparently immunocompetent patients with inv

Malignant epithelioid cranial nerve sheath tumor: case report of a radiation response.

Malignant epithelioid nerve sheath tumors (MESs) especially those involving intracranial cranial nerves are rare and thought to be radioresistant. We report a case of a MES involving the Vth and VIIth cranial nerves responsive to radiotherapy. A 41-year-old man with progressive left facial weakness underwent an MRI that disclosed an enhancing lesion involving both V3 cranial nerve and the distal VIIth nerve. Biopsy confirmed a malignant epithelioid schwannoma. The tumor was resected but residual tumor was present at the cut end of the infraorbital nerve, within the oral cavity and at the brain

Topographic anatomy of the lingual nerve and variations in communication pattern of the mandibular nerve branches.

We made a thorough observation of the morphology and course of the lingual nerve (LN) and inferior alveolar nerve (IAN) to clarify their topographical relationships in the infratemporal fossa and in the paralingual area. Thirty-two Korean hemi-sectioned heads were dissected macroscopically and microscopically from a clinical viewpoint. On the 32 tracings on the radiograph, the average distance between the retromolar portion and the LN was 7.8 mm, and no case was found where the LN ran above the alveolar crest as passing along the mandibular lingual plate. The bifurcation of the LN and IAN was

MRI vs. high-resolution CT scanning: evaluation of the anterior skull base.

While magnetic resonance imaging (MRI) is no longer a tool of the future, its availability is limited. Most hospitals still use high-resolution computed tomography (CT) scanning as their major imaging modality. Although numerous articles point to the improved diagnostic uses of MRI--as compared to high-resolution CT--a change from high-resolution CT scanning to MRI scanning throughout the United States would cost billions of dollars. We should, therefore, delineate those clinical situations in which magnetic resonance imaging is distinctly superior to high-resolution CT scanning. It is already

Endoscopic surgical management of juvenile nasopharyngeal angiofibroma: Correlating tumour characteristics, risk of hemorrhage, and recurrence.

OBJECTIVES: Juvenile nasopharyngeal angiofibroma is an uncommon tumour primarily seen in young males. It is characterized by its vascular nature and aggressive growth pattern, presenting a significant surgical challenge. Endoscopic sinonasal surgery has become the treatment of choice, reducing both morbidity and blood loss, as well as recurrence rates. This study aims to present the clinical characteristics and surgical outcomes of patients with juvenile nasopharyngeal angiofibroma treated exclusively with endoscopic surgery at our centre. METHODS: A retrospective cohort study was conducted o
