RATIONALE: Spinal cord infarction is one of the complications of epidural steroid injections (ESIs), but has only been reported in cervical vertebra by transforaminal injection and lumbar vertebra by transforaminal injection; and up to now, there is no reporting about spinal cord infarction caused by caudal injection. Here, we report a case.
PATIENT CONCERNS: A 52-year-old man was admitted to our hospital. He was diagnosed as lumbar disc herniation in other hospital, and the patient suffered bilateral lower limb motor and sensory disorders after administration of caudal ESI.
DIAGNOSIS: Spina

We report a patient with a previously undiagnosed spinal dural arteriovenous fistula (SDAVF) who became acutely paraplegic following a lumbar epidural steroid injection for lumbar spinal stenosis. Magnetic resonance imaging showed multiple flow voids and serpentine vessels on the cord surface with cord edema extending from T3 through the conus. Spinal angiography confirmed an SDAVF fed by the left lateral sacral artery, which was subsequently endovascularly embolized, and the patient had a partial return of function. Presence of an undiagnosed SDAVF should be considered in patients presenting

Cervical radiculopathy is typically characterized by neurologic symptoms that are traced to disturbances of discrete spinal nerve root(s) due to inflammatory or mechanical etiologies. Here we present three patients diagnosed with cervical radiculopathy, whose directional preference only surfaced after either a cervical transforaminal or intralaminar nerve root epidural steroid injection. This retrospective observational case series describes three men who presented with cervical radiculopathy with 7-9/10 neck pain, neck disability index (NDI) ranging between 44% and 90%, and an irreducible der

OBJECTIVE: This study aimed to determine whether the distribution of radicular pain has predictive value in identifying patients who would benefit from transforaminal cervical epidural steroid injections.
DESIGN: Retrospective cohort study of subjects with cervical radiculopathy who underwent cervical epidural steroid injections from February 2005 to January 2006. Data from subjects were divided into groups based on pain distribution, imaging-based diagnosis of cervical disk herniation or stenosis, and their response to treatment. χ tests were used to assess the relationships between distribu

This report describes a case of variant angina induced by epidural infusion of bupivacaine hydrochloride for the treatment of intractable low back pain in a 66-year-old male patient with lumbar discopathy. Severe reversible coronary artery spasm of right coronary artery was demonstrated by coronary angiography. Withdrawal of epidural anesthesia and treatment with nitrates and calcium channel antagonists resulted in cessation of variant angina.

BACKGROUND: Little attention has been afforded to the potential adverse sequelae of withholding anticoagulation therapy in a patient after neuraxial interventions.
OBJECTIVE: Presenting a case of thromboembolic stroke in a patient after holding warfarin for a lumbar epidural steroid injection, previously unreported in published literature. The discussion that follows reviews the guidelines available to reduce the risk of thromboembolic events in anticoagulated patients in the periprocedural period.

PURPOSE: To assess the efficacy of translaminar cervical epidural steroid injection (ESI) in the management of localized or radicular neck pain and assess categoric factors that can help predict clinical outcome.
MATERIALS AND METHODS: In all patients studied, treatment of neck pain with oral pain medications or physical therapy had failed. A total of 280 translaminar cervical ESIs were performed in 161 patients with an average age of 58 years (range, 26-82 y). The average duration of symptoms until the time the procedure was performed was 18.2 months (range, 0.25-240 months). All patients we

The effects of epidural injections of triamcinolone acetonide and bupivacaine in the treatment of sciatica were analyzed in a retrospective series of 526 consecutive cases with measurements. A new test (the whistle test) is described. There is a paucity of measureable parameters in reports on the subject in the literature, and many are not specific or symptom-oriented to sciatica. The procedure was performed by the same operator and reviewed one week post-operatively with measurements. 491 patients (93.35%) achieved excellent to good pain relief, backed by appropriate increases of straight-leg

STUDY DESIGN: Retrospective Cohort Study.
OBJECTIVE: This study aimed to identify radiographic and patient-dependent factors that predict failure of epidural steroid injection (ESI) management, leading to subsequent surgical management in patients with lumbar stenosis.
SUMMARY OF BACKGROUND DATA: Lumbar stenosis is a common cause of radiculopathy and neurogenic claudication, and ESIs are a mainstay of conservative care. However, the benefits of ESIs are debated, and limited evidence exists on how stenosis characteristics correlate with the need for eventual surgery post-ESI.
METHODS: Adult

Lumbar epidural steroid injections (ESI) are frequently used to treat radicular pain; however, various early and late complications, such as steroid-induced osteoporosis and fracture risk, have been reported. This case report describes a 77-year-old male patient who developed a femoral neck fracture two hours after transforaminal ESI. The patient presented with acute pain and difficulty walking shortly after discharge. Radiographic evaluation confirmed the femoral neck fracture, and total hip arthroplasty was performed. This rare case highlights the importance of assessing osteoporosis and fra

A recent lumbar puncture (LP) is a relative contraindication for intravenous thrombolysis (IVT) due to the potential for the development of spinal hematoma. While there have been case reports of IVT administered without complications following LP or epidural anesthesia, there is limited data overall regarding the safety of IVT for acute ischemic stroke (AIS) in patients who have undergone a neuraxial procedure. We present a case report of an 83-year-old man who presented with a sudden onset of symptoms consistent with left middle cerebral artery (MCA) stroke after receiving a cervical epidural

A male patient presented to the clinic with persistent hiccups and the complaint of subacute low back pain. Subsequently, he was administered a transforaminal epidural betamethasone injection (TFESI) plus lidocaine and developed persistent hiccups thereafter, which lasted for 6 days and subsided spontaneously. He presented with similar complaints again 5 months later and was administered TFESI, this time along with betamethasone and saline. Yet, he once again developed persistent hiccups, which this time lasted for 7 days and subsided spontaneously. This case report, featuring a patient who wa
