A case study on the application of International Classification of Functioning, Disability and Health (ICF)-based tools for vocational rehabilitation in spinal cord injury.

OBJECTIVES: Vocational rehabilitation (VR) plays a key role in bringing persons with acquired disabilities back to work, while encouraging employment participation. The purpose of this case study is to illustrate the systematic application of International Classification of Functioning, Disability, and Health (ICF)-based documentation tools by using ICF Core Sets in VR shown with a case example of a client with traumatic spinal cord injury (SCI). PARTICIPANT: The client was a 26-year-old male with paraplegia (7th thoracic level), working in the past as a mover. METHODS: This case study descr

Structured reporting of B-mode, color Doppler, and CEUS in testicular tumor assessment: a reader study with urologist ratings.

PURPOSE: Structured reporting (SR) offers standardized radiological documentation, enhancing clarity and reproducibility. However, its role in contrast-enhanced ultrasound (CEUS) for testicular tumors remains underexplored. This study evaluates urologist-perceived clarity, completeness, and clinical usefulness of SR compared to free-text reporting (FTR). METHODS AND MATERIALS: In this retrospective, single-center study, 65 male patients with suspected testicular tumors underwent CEUS at LMU University Hospital. Reports were initially documented as FTRs by an experienced radiologist and later c

Knowledge translation in surgery: a scoping review of implementation strategies, effectiveness and contextual barriers and enablers.

BACKGROUND: Knowledge translation (KT) interventions are essential for implementing evidence-based practices in healthcare. However, despite their proven effectiveness in addressing global health challenges, KT strategies in surgery remain challenging to apply. This scoping review examines KT strategies in surgery, their effectiveness, and key barriers and enablers to their implementation. METHODS: This scoping review followed the Arksey and O'Malley and Levac et al. frameworks, integrating the RE-AIM (Reach, Effectiveness, Adoption, Implementation, Maintenance) and the PRISM (Practical Robus

[ePROs in the routine care of a major oncology center: Results and experiences].

BACKGROUND: To what extent and under what conditions electronically captured patient-reported outcomes (ePROs) can be used in routine medical care and contribute to improved patient care is a widely discussed question. In the field of oncology, few studies in Germany have focused this topic that go beyond the scope of time-limited studies. METHOD: First, we present the centrally coordinated collection of ePROs in the routine care of a comprehensive cancer center of the German Cancer Aid in its development, and then describe its qualitative dimension. Subsequently, the intervention is discusse

Experiences of patients with advanced chronic diseases and their associates with a structured palliative care nurse visit followed by an interprofessional case conference in primary care - a deductive-inductive content analysis based on qualitative interviews (KOPAL-Study).

BACKGROUND: Chronic, non-malignant diseases (CNMD) like chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF) and dementia in advanced stages are very burdensome for patients. Timely palliative care with strong collaboration between general practitioners (GPs) and specialist palliative home care (SPHC) teams can reduce symptom burden, hospitalization rates, hospitalization costs and overall healthcare costs. The KOPAL-study on strengthening interprofessional collaboration for patients with palliative care needs tested the effect of an intervention comprising of a SPHC nu

How to customize common data models for rare diseases: an OMOP-based implementation and lessons learned.

BACKGROUND: Given the geographical sparsity of Rare Diseases (RDs), assembling a cohort is often a challenging task. Common data models (CDM) can harmonize disparate sources of data that can be the basis of decision support systems and artificial intelligence-based studies, leading to new insights in the field. This work is sought to support the design of large-scale multi-center studies for rare diseases. METHODS: In an interdisciplinary group, we derived a list of elements of RDs in three medical domains (endocrinology, gastroenterology, and pneumonology) according to specialist knowledge a

[Postextubation dysphagia in intensive care unit : Epidemiology, clinical course, and management].

Postextubation dysphagia (PED) is common in intensive care units (ICU), affecting about 20% of patients of mixed medical surgical ICU populations. PED is an independent risk factor for increased 28-day and 90-day mortality in both neurological and nonneurological ICU patients (28-day mortality: plus 9%). The increased mortacity risk can be demonstrated for up to approximately one year after the ICU stay. Due to the consequences of PED, all ICU patients should undergo systematic dysphagia screening after extubation/decannulation (e.g., water swallow test) and fiberoptic endoscopic evaluation of

[Invasive ENT infections in immunocompetent schoolchildren in the winter half year 2022/23 : A case series].

We report a case series of severe complications following rhinosinusitis in the winter season 2022/2023 at the Franz-Lust Hospital for children and adolescents. Due to the severity of these complications in children with suspected complicated rhinosinusitis/orbital phlegmona, an immediate interdisciplinary approach is mandatory to prevent long-term sequelae. In addition, during this time period, we observed the unusual occurrrence of a number of invasive streptococcal infections in this age cohort.

EXPRESS: Progress and perspectives on developmental language and reading disorders.

Why do some children learn to read or talk more easily than others? We review recent advances in genetics, neuroscience, cognition, and intervention research. About half of the variation in language and reading outcomes is explained by genetic differences. Yet genes and environments are intertwined - for example, children's early language exposure is partly shaped by their parents' genetically influenced traits. Neuroimaging studies show that language and reading disorders involve not only domain-specific brain regions but also subcortical systems supporting learning in general. In cognition,

[Interdisciplinarity, Evidence and Relevance of Classification for Care in Thoracic Outlet Syndrome].

Thoracic Outlet Syndrome (TOS) refers to the compression of the neurovascular structures at the thoracic outlet. Its incidence is estimated at 2-3 cases per 100000 individuals per year. Patients are frequently affected by pain and/or functional impairment. The diagnostic and therapeutic process typically involves multiple medical specialties. Diagnosis is primarily clinical and requires the exclusion of differential diagnoses. Additional diagnostic modalities, such as ultrasound, MRI, angiography, or electroneurography, may provide supportive information. Depending on the treating discipline,

[Update of the 2Sk guidelines on the diagnostics, treatment and metaphylaxis of urolithiasis (AWMF register number 043-025) : What is new?].

The increase of medical knowledge and technical innovations together with the demographic change represent a challenge for the new conception of guidelines and clinical studies. The present S2k guidelines, which are exclusively concerned with kidney and ureteral stones, should support the treatment of urolithiasis in hospitals and private practices and provide information on urolithiasis for patients. Increasing interdisciplinary collaboration in stone treatment is also demonstrated in the number of professional and working groups participating in the update of the new guidelines. The present

New Advances in Stem Cell Therapy for Osteonecrosis of the Femoral Head.

Osteonecrosis of the femoral head (ONFH) is a common refractory orthopedic disease with multiple etiologies that more frequently occurs in middle-aged and young people. ONFH is the main cause of hip replacement in young patients. Since Professor Hernigou first reported the use of stem cells in the treatment of early stage ONFH, a large number of studies have demonstrated the potential of stem cells in the treatment of adult patients with ONFH. With the rise of interdisciplinary stem cell therapy combined with platelet-rich plasma therapy, gene therapy or other methods have gradually attracted

High-risk pregnancy and neonatal care.

The delivery of high-risk maternal and neonatal homecare has been shown to be a cost-effective model of care delivery. The homecare management of preterm labor, placenta previa. PIH, and hyperemesis gravidarum has proven to be a safe interventional strategy with successful clinical outcomes. Studies of home uterine activity monitoring and phototherapy have demonstrated that the use of advanced technologic devices is spearheading the drive to reduce hospital lengths of stay, enabling the safe return of the patient to the home setting, under the watchful supervision of the high-tech homecare nur

Collaborative neurocardiology board meetings for decision-making in stroke care: a real-world experience.

BACKGROUND: Interdisciplinary collaboration between neurologists and cardiologists is needed to provide state-of-the art stroke care and individualized decisions about secondary prevention strategies. This highlights the need to establish working structures for joint decision-making between cardiologists and stroke neurologists. At present, studies describing the real-world decision-making as part of such brain-heart teams are scarce. METHODS: We established a structured heart–brain team approach with monthly virtual neurocardiology board (NCB) meetings within a neurovascular network. We condu

[Comparing Outcomes of Primary vs. Secondary Admission to a Level 1 Trauma Centre in the Management of Gustilo-Anderson≥Type IIIB Fractures: A Systematic Review].

The management of open fractures with extensive soft tissue defects presents a significant challenge to healthcare systems. A key factor for a successful and efficient treatment is the early implementation of an interdisciplinary approach involving both trauma and plastic surgery - commonly referred to as the "orthoplastic approach". This systematic review aims to compare treatment outcomes between patients who were either directly transferred (primary transfer) from the scene of injury to a level 1 trauma centre and those who initially received care at a lower-level hospital and were subseque

[Pathophysiology, Diagnostics and Therapy of Pulmonary Contusion - Recommendations of the Interdisciplinary Group on Thoracic Trauma of the Section NIS of the German Society for Trauma Surgery (DGU) and the German Society for Thoracic Surgery (DGT)].

Pulmonary contusion usually occurs in combination with other injuries and is indicative of a high level of force. Especially in multiply injured patients, pulmonary contusions are frequently detected. The injury is characterised by dynamic development, which might result in difficulties in recognising the actual extent of the injury at an early stage. Subsequently, correct classification of the extent of injury and appropriate initiation of therapeutic steps are essential to achieve the best possible outcome. The main goal of all therapeutic measures is to preserve lung function as best as pos

Scientific knowledge about dementia: From the Global South to the world.

Most people with dementia live in low- and middle-income countries (LMICs), but scientific knowledge about dementia is dominated by Global North perspectives. This interdisciplinary article combines expertise from Latin America, India, and Europe, using a mixed-methodology approach that incorporates expert discussions, a narrative review, and five illustrative case studies. The review on dementia in the Global South yielded 82 results, highlighting the themes of challenges in diagnosis and assessment, socioeconomic and systemic barriers to cognitive health and care pathways, and the need for l
