During the last ten years, 14 patients with proven aortoenteric fistulas were admitted to the Greenville Hospital System, a community system of 1200 beds in four units serving a county of 250,000 residents. The experience obtained is tabulated, and illustrative case reports are used to emphasize the diagnostic and treatment problems inherent in this group of patients. The clinical and radiological picture observed in a female patient with cholelithiasis urgently hospitalised for abdominal occlusion is described. Radiological examination of the digestive tract disclosed a bilio apico-bulbar fistula and ectasis and hypotonia of the jejuno-ileal loops. In patients with recurrent colorectal cancer, the development of fistulae presents a difficult therapeutic problem. The traditional surgical approach of resection and intestinal diversion may not always be appropriate in those with disseminated or terminal disease. We present the successful use of a covered esophageal stent to occlude an enterorectal rectal fistula in a patient with recurrent inoperable colonic carcinoma.

Personal experience about 11 cases of spontaneous biliary enteric fistula caused by lithiasic disease operated in the General Surgical Clinic of the University of Siena from 1969 to 1988, is reported. The epidemiology, pathogenesis, symptomatology, indications for operation and various operating techniques performed are discussed. In the gallstone intestinal obstruction, the authors prefer to resolve immediately the obstruction, deferring to a later moment the specific operation for the treatment of the biliary enteric fistula and the lithiasic disease.

BACKGROUND: Intestinal fistulae represent a severe complication of Crohn's disease (CD). The authors have demonstrated that epithelial-to-mesenchymal transition plays a pivotal role in their pathogenesis. High levels of interleukin-13 and tumor necrosis factor (TNF) are detected in myofibroblast-like transitional cells covering the fistula tracts.
