Pediatric gastrointestinal motility disorders are a heterogeneous group of conditions that can be particularly difficult to recognize and manage. Symptoms are typically chronic and have a significant impact on quality of life. Medical therapies are limited and definitive treatment often requires surgical intervention that can alleviate certain symptoms, but predispose the patients to other comorbidities. The goal of this review is to summarize the current understanding of the primary gastrointestinal motility disorders in childhood and their management.

The authors report on an 18 month-old boy affected by Kawasaki's disease (KD). The diagnosis of KD was made after exclusion of conditions with similar presentation. Discussed is a patient with both a mechanical and paralytic ileus as a result of a gangrenous appendicitis with perforation and a circumscribed peritoneum and incarceration of an inguinal hernia on the opposite side. The patient's heart too was suffering a disease. The postoperative course was difficult. It was complicated by pleural hemorrhaging and by lung infiltrations because of embolisms. With an emergency operation involving the entire team, the patient was saved. Although the pathology is rare, the patient was saved.
