Intraoperative Calcar Fractures in Uncemented Total Hip Replacement: A Systematic Review.

BACKGROUND: Intraoperative calcar fractures (IOCFs) are a recognized complication of uncemented total hip arthroplasty (THA), often occurring during femoral preparation or stem insertion due to the press-fit fixation required for cementless implants. The aim of this systematic review was to synthesize current evidence regarding the epidemiology, risk factors, management strategies, and outcomes associated with IOCFs in uncemented THA. METHODS: A systematic search of electronic databases, including PubMed, MEDLINE, and Embase, was performed for studies reporting IOCFs during primary uncemented

Low pneumoperitoneum pressure facilitates postoperative pain relief and gastrointestinal function recovery in laparoscopic gastrointestinal surgery: a systematic review and meta-analysis.

OBJECTIVE: The potential benefits of low pneumoperitoneum pressure (LPP) in laparoscopic gastrointestinal surgery, particularly gastric procedures, remain insufficiently investigated. This meta-analysis aims to systematically evaluate the advantages of LPP in laparoscopic gastrointestinal surgery compared to standard pneumoperitoneum pressure (SPP). METHODS: A comprehensive literature search was conducted in Embase, Web of Science, PubMed, and Cochrane Library databases from inception to April 10, 2025. Studies comparing LPP with SPP in laparoscopic gastrointestinal surgery, including both ra

Surgical management of intraoperatively diagnosed portal annular pancreas: Two case reports.

RATIONALE: Portal annular pancreas (PAP) is a rare pancreatic anomaly characterized by portal vein encasement in the pancreatic parenchyma. Due to its rarity, PAP may often be missed on preoperative computed tomography (CT) review, and surgeons may face challenges in dealing with an unexpected intraoperative encounter with PAP. We documented 2 such intraoperatively diagnosed cases and illustrated their surgical management. PATIENTS CONCERNS: In case 1, a 70-year-old man was found to have a 15-mm mass in the pancreatic body and dilatation of the peripheral main pancreatic duct on enhanced CT.

Incidental ureteral triplication discovered during surgery for severe hydronephrosis: a case report.

INTRODUCTION: Ureteral triplication is an exceptionally rare congenital anomaly of the upper urinary tract, with fewer than 100 cases reported worldwide. Preoperative diagnosis remains challenging, particularly in low-resource settings. PRESENTATION OF CASE: A 3.5-year old boy presenting with dysuria and intermittent severe left flank pain. Initially diagnosed as ureteral duplication with hydronephrosis. Due to limited access to advanced imaging modalities, no further diagnostic evaluation was performed. Intraoperative findings revealed type 3 ureteral triplication with marked hydroureteronep

[Expert consensus on perioperative management of adult patients combined with obstructive sleep apnea (2025 edition)].

Obstructive sleep apnea (OSA) is a common chronic disease closely associated with various complications during the perioperative period, such as respiratory failure, hypertension, coronary artery ischemia, arrhythmia, stroke, etc. In recent years, with the increasing prevalence of OSA, the incidence of OSA in patients undergoing surgery has also been increasing. Screening, diagnosis, and treatment of OSA before surgery can help to reduce surgical related complications. At the same time, the management of patients with OSA during the perioperative period also has its own characteristics. In ord

Unusual Presentation of Parieto-Visceral Peritoneal Band Obstruction in Adults: The Role of Congenital Peritoneal Bands.

Peritoneal bands are rare congenital remnants of mesenteric folds that may persist due to incomplete embryological regression. While they are a recognized cause of small bowel obstruction in children, their occurrence in adults is uncommon and often incidental. During routine abdominal dissection of a 42-year-old female cadaver, an atypical parieto-visceral peritoneal band was identified. The band originated from the anterior abdominal wall, approximately two inches above the umbilicus near the apex of the falciform ligament, and coursed posteriorly. It exerted a constrictive effect on the dis

Multimodal Ultrasound Imaging in the Diagnosis of Primary Giant Cell Tumor of the Breast: A Case Report and Literature Review.

Giant cell tumor of soft tissue (GCT-ST) is an extremely rare phenomenon in the breast. Herein, a case involving a 75-year-old female with a painless lump and bloody discharge from the nipple of her left breast is reported. A diagnosis of malignant tumor was arrived at by observing the location of the tumor, interior echo, margins, vascular distribution, hardness, and microvascular density on preoperative multimodal ultrasonography. However, no evidence of malignancy was found on ultrasonography-guided nuclear needle biopsy or rapid intraoperative pathological examination. Subsequently, an exc

Perioperative Management in Neuromuscular Diseases: A Narrative Review.

Patients with neuromuscular diseases are particularly vulnerable in the perioperative period to the development of pulmonary and cardiac complications, or medication side effects. These risks could include hypoventilation, aspiration pneumonia, exacerbation of underlying cardiomyopathy, arrhythmias, adrenal insufficiency, prolonged neuromuscular blockade, issues related to thermoregulation, rhabdomyolysis, malignant hyperthermia, or prolonged mechanical ventilation. Interventions at each of the perioperative stages can be implemented to mitigate these risks. A careful pre-operative evaluation

Learning-based keypoint registration for fetoscopic mosaicking.

PURPOSE: In twin-to-twin transfusion syndrome (TTTS), abnormal vascular anastomoses in the monochorionic placenta can produce uneven blood flow between the two fetuses. In the current practice, TTTS is treated surgically by closing abnormal anastomoses using laser ablation. This surgery is minimally invasive and relies on fetoscopy. Limited field of view makes anastomosis identification a challenging task for the surgeon. METHODS: To tackle this challenge, we propose a learning-based framework for in vivo fetoscopy frame registration for field-of-view expansion. The novelties of this framewor

[Processed EEG for personalized dosing of anesthetics during general anesthesia].

Electroencephalogram (EEG)-guided anesthesia is indispensable in modern operating rooms and has become established as the standard form of monitoring. Many anesthesiologists rely on processed EEG indices in the hope of averting anesthesia-related complications, such as intraoperative awareness, postoperative delirium and other cognitive complications in their patients. This educational review aims to provide information on the five most prevalent monitors used to guide depth of sedation during general anesthesia. This article elucidates the principles underpinning the application of these moni…

Editorial Commentary: An Optimal Classification System to Guide Prognosis and Treatment in Greater Trochanteric Pain Syndrome: Now We're Speaking the Same Language.

The optimal classification system in arthroscopic and related surgery research and clinical practice should be clinically relevant, descriptive, reproducible, simple, inexpensive, safe, and widely applicable. For the hip, classification systems that characterize intra-articular disorders like femoroacetabular impingement (FAI) syndrome, dysplasia, labral tears, and articular cartilage disease predominate the literature. Recently, awareness of peritrochanteric and other extra-articular disorders has increasingly led to greater recognition, diagnosis, and treatment of what has been historically

Complications associated with anterior shoulder instability repair.

Anterior shoulder instability is a common orthopaedic problem, and the surgical treatment, both open and arthroscopic, has been shown to effectively restore stability and prevent recurrence. However, despite success with these surgical techniques, there are several clinically relevant complications associated with both open and arthroscopic techniques for anterior shoulder stabilization. These complications can be subdivided into preoperative, intraoperative, and postoperative and include entities such as nerve injury, chondrolysis, incomplete treatment of associated lesions, and subscapularis
