BACKGROUND: Distal femoral replacement (DFR) with a rotating hinged prosthesis (RH) is sometimes used as a salvage procedure following complications related to total knee arthroplasty (TKA).
METHODS: This was a retrospective case series performed at a tertiary academic medical center from 2008 to 2023. Patients that are age 18-70, male or female, who underwent DFR-RH who experienced a jump post type dislocation event postoperatively were included in this study. Since January 1988 21 operations using a modified Madigan and Goldthwait method have been carried out on 18 patients suffering from recurrent dislocation of the patella. The underlying complex pathoanatomy of a laterally positioned and shortened extensor mechanism poses challenges in its management. The knee was assessed by normal plain radiographs in 20 achondroplasia patients (40 lower extremities), whose age ranged from 10.5 to 23 years. Five patients (seven knees) had permanent patellar dislocation. There was a significant difference between the achondroplasia group and control group (50 knees without skeletal dysplasia) for femoral sulcus angle but not for femorotibial angle, Insall-Salvati index and Q angle. Patellar dislocation for achondroplasia may be caused not only by the abnormalities of soft tissue but also a small groove of patellofemoral joint as a bony malformation.
