Autoimmune inner ear disease is frequently characterized by progressive bilateral hearing loss, which is not necessarily symmetrical. Vertigo, aural fullness, and tinnitus may also accompany it. There are 2 ways that the inner ear might be impacted: either as a primary disorder when the immune response directly attacks inner ear cells or as a secondary symptom of a systemic autoimmune disease. We describe the case of an 11-year-old boy with morphea, who was diagnosed with autoimmune inner ear disease after he developed hearing loss. This relationship in children has not yet been documented in

INTRODUCTION: Autoimmune Inner Ear Disease (AIED) can be of a primary or secondary type. To date, a clear pathogenesis of the disease is still not available. Focusing on the secondary forms of AIED, the aim of this review is to (i) assess and describe the hearing involvement in patients affected by autoimmune diseases, (ii) describe the possible association between clinical features (among serological/laboratory data and disease activity/duration) and hearing impairment, (iii) show evidence connecting the AIED types with various etiopathogenetic mechanisms.
AREAS COVERED: A PRISMA-compliant s

Disorders of the inner-ear balance organs can be grouped by their manner of presentation into acute, episodic, or chronic vestibular syndromes. A sudden unilateral vestibular injury produces severe vertigo, nausea, and imbalance lasting days, known as the acute vestibular syndrome (AVS). A bedside head impulse and oculomotor examination helps separate vestibular neuritis, the more common and innocuous cause of AVS, from stroke. Benign positional vertigo, a common cause of episodic positional vertigo, occurs when otoconia overlying the otolith membrane falls into the semicircular canals, produc

Optokinetic nystagmus was recorded and measured in 101 subjects comprising six diagnostic categories: 1) normal, screened for otologic disease, 2) chronic unilateral labyrinthectomy, 3) unilateral Menière's disease, 4) neurologically confirmed focal brainstem lesion, 5) brainstem-cerebellar syndrome, and 6) focal unilateral supratentorial lesion. For the OKN test, each subject looked at a translucent screen onto which a field of parallel black and white bars was back-projected. The array of bars could be projected vertically or horizontally to allow for study of nystagmus beating right and lef

OBJECTIVE: To provide a detailed description of the coexistence of benign paroxysmal positional vertigo (BPPV) and Meniere's disease in individuals and to offer a possible mechanism that explains the findings in these patients.
STUDY DESIGN: Retrospective.
METHODS: Chart review.
RESULTS: Of 162 patients diagnosed with Meniere's disease between January 1998 and January 1999, 9 were found to have both "definite" Meniere's disease and "certain" BPPV. Meniere's symptoms preceded the onset of BPPV in all of our patients. Seven of the 9 patients were female. Except for one patient who experienced

OBJECTIVE: To evaluate the impact of dizziness on quality of life (QOL) in Meniere's disease (MD) patients.
METHODS OF STUDY: Fifty definite MD patients were submitted to the Dizziness Handicap Inventory (DHI), in and out of crisis. The DHI scores were correlated to age, sex, race, disease duration, unilateral/bilateral labyrinth involvement, duration/frequency of vertigo attacks, vestibular and auditory function, aural fullness, tinnitus, imbalance and functional disabilities. Statistical analysis was performed by ANOVA.
RESULTS: During crisis, all DHI aspects scores were statistically high

OBJECTIVE: To systematically evaluate the evidence for treatment of autoimmune inner ear disease (AIED).
DATA SOURCES: Medline/EMBASE, Cochrane Review.
STUDY SELECTION: Studies reviewed were English language original articles reporting outcome data for subjects with AIED undergoing any treatment other than oral steroids alone with particular emphasis on prospectively collected data.
DATA EXTRACTION: The full text of articles meeting selection criteria was reviewed for subject inclusion criteria, number of subjects, treatment type and duration, audiologic and vestibular evaluations, blinding

OBJECTIVES: The objective of our study was to review our current knowledge of the aetiopathogenesis of Cogan's syndrome, including viral infection and autoimmunity, and to discuss disease pathogenesis with relevance to pharmacotherapy.
SYSTEMATIC REVIEW METHODOLOGY: Relevant publications on the aetiopathogenesis and pharmacotherapy of Cogan's syndrome from 1945 to 2012 were analysed.
RESULTS AND CONCLUSIONS: Cogan's syndrome is a rare autoimmune vasculitis, and its pathogenesis is unknown. Infection, but primarily autoimmunity, may play contributing roles in the pathogenesis of this disease.

A labyrinthine fistula is a frequent complication of long-standing unsafe chronic suppurative otitis media. It is characterized by a slowly progressive erosion of the bony labyrinth. In this paper we present our observations regarding the diagnosis and management in 50 patients with unsafe chronic suppurative otitis media with labyrinthine fistula.

43 cases of large labyrinthic fistulas caused by a cholesteatoma of the middle ear were studied (33 External Semi-circular Canal fistulas, 4 External Semi-circular Canal amputations with opening of the vestibule, 3 of the promontory, 2 of the oval window, 1 of the Semi-circular Canal). The audiometric, preoperative, radiological data was analyzed. The radiological exam is often disappointing for the average-sized fistulas. Preoperative labyrinthization is moderate for External Semi-circular Canal fistulas and even for certain vestibular amputations; it is often incomplete in the other location
