Lactose intolerance: diagnosis and management.

Lactose intolerance affects hundreds of millions of people worldwide. Although the presentation is frequently atypical, it should be part of the differential diagnosis when evaluating nonspecific gastrointestinal symptoms. We review the terminology, types of lactase deficiencies, diagnostic procedures, and management.

Accuracy of a Genetic Test for the Diagnosis of Hypolactasia in Chilean Children: Comparison With the Breath Test.

BACKGROUND: Lactase nonpersistence (LNP) in humans is a genetically determined trait. This age-dependent decrease of lactase expression is most frequently caused by single nucleotide polymorphisms in the regulatory region of the lactase (LCT) gene. The homozygous LCT-13,910C/C genotype (rs 4988235) predominates in Caucasian adults with LNP, and is useful for its diagnosis in this population. The accuracy of this genetic test (GT) has not been completely established in children or in a Latin-American population. OBJECTIVES: The aim of the study was to determine diagnostic accuracy of GT for LN…

Host and gut microbial tryptophan metabolism and type 2 diabetes: an integrative analysis of host genetics, diet, gut microbiome and circulating metabolites in cohort studies.

OBJECTIVE: Tryptophan can be catabolised to various metabolites through host kynurenine and microbial indole pathways. We aimed to examine r … ng Firmicutes. We identified a novel association between a host functional LCT variant (determining lactase persistence) and serum indolepropionate, which might be related to a host gene-diet interaction on gut Bifidobacterium, a probiotic bacterium significantly associated with indolepropionate independent of other fibre-related bacteria. Higher milk int …

Quantitative measurement of lactose absorption.

The quantity of lactose not absorbed by 4 normal and 6 lactase-deficient subjects was determined by three indirect methods which involved: (1) measurement of pulmonary hydrogen (H2) excretion, (2) pulmonary (14)CO2 excretion, and (3) stool (14)C excretion, after ingestion of 12.5 g of 1-(14)C-lactose and 4 g of polyethylene glycol (PEG). Results were compared with absorption determined directly from the (14)C:PEG ratio of multiple terminal ileal aspirates. The fraction of lactose not absorbed determined by ileal aspiration ranged from 0 to 8% in normals and 42 to 75% in mild-intolerant subject…

High prevalence of celiac disease in patients with lactose intolerance.

BACKGROUND/AIMS: Acquired lactase deficiency is a common cause of gastrointestinal symptoms but its etiology remains unclear. Celiac disease could lead to lactase deficiency and is much more common than previously suspected. Several studies have highlighted the prevalence of lactose intolerance in celiac disease, but studies assessing the prevalence of celiac disease in lactose intolerance are lacking. We evaluated the prevalence of celiac disease in patients with a positive H2-lactose breath test compared to a control group. METHODS: This retrospective study included 54 patients (15 males/39…

Inhibition of the glycaemic response by onion: a comparison between lactose-tolerant and lactose-intolerant adults.

This pilot study compared inhibition of the glycaemic response to glucose by a dietary source of quercetin glucosides (onion) in lactose-tolerant adults (n=12) and lactose-intolerant adults (n=12). We hypothesised that lactose-intolerant people (who do not express lactase) will retain intact quercetin glucosides that can inhibit glucose uptake via the glucose transporter SGLT1, whereas lactose-tolerant people (who do express lactase) will hydrolyse quercetin glucosides to free quercetin that does not inhibit glucose uptake. In a glucose tolerance test, reduction in peak glucose levels by an on…

High frequency of lactose intolerance in a prehistoric hunter-gatherer population in northern Europe.

BACKGROUND: Genes and culture are believed to interact, but it has been difficult to find direct evidence for the process. One candidate example that has been put forward is lactase persistence in adulthood, i.e. the ability to continue digesting the milk sugar lactose after childhood, facilitating the consumption of raw milk. This genetic trait is believed to have evolved within a short time period and to be related with the emergence of sedentary agriculture. RESULTS: Here we investigate the frequency of an allele (-13910*T) associated with lactase persistence in a Neolithic Scandinavian po…

The role of colonic microbiota in lactose intolerance.

In a previous study we observed a clear difference in lactose intolerance symptoms after a 25-g lactose load in two groups of persons with lactase nonpersistence and similar small intestinal lactase activity. From this observation we hypothesized a colon resistance factor. To identify this factor, the microbial composition of fecal samples of the two lactose intolerant groups (one with mild symptoms, n = 16, and one with diarrhea-predominant symptoms, n = 11) was compared using the fluorescent in situ hybridization technique. Large interindividual differences were found in the numbers of total…

[Colonic fermentation in humans: effect of the ingestion of meat associated to milk].

The amount of breath H2 produced for 6 h following the ingestion of 240 ml of milk was used as an index of the rate of colonic fermentation of undigested lactose in 8 lactase non-persistent Guatemalan adults. Treatments in separate days included milk alone, milk with 400 g cooked beef, lactose pre-hydrolyzed milk with the same amount of beef, beef alone and fasting for 6 h. Excess excretion of H2, calculated as the mean area under the curve were 178 +/- 31 ppm h with milk alone, 50 +/- 17 with milk plus beef, -1 +/- 26 with prehydrolyzed milk (p > 0.001). Peak increments of breath H2 followed …

Development of carbohydrate absorption in the fetus and neonate.

Maturation of mechanisms for carbohydrate absorption occurs in a defined sequence during human fetal development. The intestinal enzymes, lactase, sucrase, maltase, isomaltase, and glucoamylase, are at mature levels in the term fetus. Mature levels of pancreatic amylase activity and glucose transport occur postnatally, and levels are low in both the term and preterm neonate. In the preterm infant, sucrase, maltase, and isomaltase are usually fully active, but lactase activity, which increases markedly from 24 to 40 weeks, may be low depending upon fetal age. Despite these developmental pattern…

Disaccharidase-deficient animals have normal ultrastructure of intestinal brush border membranes.

The intestinal disaccharidases, lactase, sucrase-isomaltase complex, and glucoamylase are proteins intimately associated with the brush-border membrane of the epithelial cell. These three enzyme activities are found in the intestine of the adult rat; lactase and glucoamylase activities are primarily associated with the intestine of the infant rat. Only glucoamylase and isomaltase activities are detected in the intestine of the California sea lion, Zalophus californianus. The activities of these enzymes are detected only in villus cells, and not in crypt cells. We have carried out electron micr…

[Lactose intolerance: past and present. Part II].

The author summarises the interrelations between lactose intolerance, calcium and vitamin D metabolism and osteoporosis. Lactose intolerance enhances the risk of forearm and hip fractures in some patients. Lactase gene genotype and fracture risk are related in some populations. Calcium and vitamin D supplementation increase bone mineral content and they are justified in children, during pregnancy and lactation, and in postmenopausal women. The intake of milk and milk products could increase the risk of ovarian carcinoma. CC genotype of the lactase gene increased the risk of colorectal carcinom…

[Selective proximal vagotomy with and without pyloroplasty--results of a randomized clinical study of duodenal ulcer 5 and 8 years after surgery].

Selective proximal vagotomy (SPV) with pyloroplasty (n = 39) and without (n = 39) were compared five and eight years after surgery in a randomized clinical study. Pyloroplasty according to Finney had no effect on ulcer recurrence rates after both periods of time (7.9:7.9 or 10.8:8.3 per cent). Additional pyloroplasty, too, had no statistically significant effect on incidence and severity of post-vagotomy syndrome, dumping and diarrhoea, though some trend towards an increase was discernible in wake of pyloroplasty. Occurrence of the lactase deficit syndrome after SPV with pyloroplasty was incre…

Effect of dietary lactose on the absorption of protein, fat and calcium in the postweaning rat.

To assess the effect of dietary lactose on the intestinal absorption of protein, fat and calcium in lactose intolerance due to low levels of intestinal lactase, balance studies were carried out on postweaning rats (initial weight 164 g) for a period of 10 days. The experimental diets contained either 10% or 30% lactose, while in the control diets the lactose was replaced with equivalent amounts of sucrose. The excretions of fecal and urinary nitrogen, calcium and fat are expressed as precent of intake over the 10-day period. The fecal excretion of nitrogen and fat was significantly higher in t…
