Large language models for neurology: a mini review.

Large language models have the potential to transform neurology by augmenting diagnostic reasoning, streamlining documentation, and improving workflow efficiency. This Mini Review surveys emerging applications of large language models in Alzheimer's disease, Parkinson's disease, multiple sclerosis, and epilepsy, with emphasis on ambient documentation, multimodal data integration, and clinical decision support. Key barriers to adoption include bias, privacy, reliability, and regulatory alignment. Looking ahead, neurology-focused language models may develop greater fluency in biomedical ontologi…

Progressive Supranuclear Palsy with Predominant Speech/Language Disorder.

We herein report a rare case of progressive supranuclear palsy (PSP) that initially presented as progressive fluent aphasia and echolalia. A 75-year-old woman exhibited significant language impairments for 2 years before developing motor symptoms, including vertical gaze palsy and frequent falls. Neuroimaging revealed asymmetric frontal and midbrain atrophy, with reduced perfusion in the left frontal and parietal lobes. This case highlights the importance of recognizing atypical PSP presentations, particularly language-dominant variants that may mimic other neurodegenerative disorders. Early i…

Language barriers in mental healthcare: A critical analysis.

In multicultural healthcare settings, language barriers pose significant challenges, particularly in mental health where nuanced communication is vital. This manuscript examines the impact of limited English proficiency (LEP) on mental healthcare in New Zealand, where a substantial portion of the population and healthcare workforce are foreign-born. Despite official recommendations, professional interpreter use remains low, compromising care quality for LEP patients. The paper explores interpreters' roles beyond interpretation, including cultural mediation, while noting limitations in interpre…

Developmental language trajectories in children with critical CHD.

INTRODUCTION: Medical and surgical advancements have enabled a 95% survival rate for children with CHD. However, these survivors are disproportionately affected by neurodevelopmental disabilities. In particular, they have behavioural problems in toddlerhood. Because there is a known relationship between behavioural problems and early language delay, we hypothesise that children with critical CHD have early detectable language deficits. To test our hypothesis, we performed a retrospective study on a cohort of children with critical CHD to visualise their early language developmental trajectorie…

Brain structure correlates of foreign language learning experiences.

Previous research demonstrates that bilingual experiences produce dynamic and variable structural adaptations in the brain. However, most studies focus on individuals who naturally acquire multiple languages, leaving the neural effects of long-term foreign language learners with limited exposure less understood. In this study, we examined 32 young adult long-term foreign language learners in South Korea to determine how age of acquisition, proficiency, and daily use shape gray matter volume and white matter integrity in predominantly monolingual environments. We found structural changes in bra…

Remapping and Reconnecting the Language Network after Stroke.

Here, we review the literature on neurotypical individuals and individuals with post-stroke aphasia showing that right-hemisphere regions homologous to language network and other regions, like the right cerebellum, are activated in language tasks and support language even in healthy people. We propose that language recovery in post-stroke aphasia occurs largely by potentiating the right hemisphere network homologous to the language network and other networks that previously supported language to a lesser degree and by modulating connection strength between nodes of the right-hemisphere languag…

Comparing the dental knowledge of large language models.

Introduction With the advancement of artificial intelligence, large language models (LLMs) have emerged as technology that can generate human-like text across various domains. They hold vast potential in the dental field, able to be integrated into clinical dentistry, administrative dentistry, and for student and patient education. However, the successful integration of LLMs into dentistry is reliant on the dental knowledge of the models used, as inaccuracies can lead to significant risks in patient care and education.Aims We are the first to compare different LLMs on their dental knowledge th…

Assessing language barriers in health facilities in Malawi.

BACKGROUND: Language barriers in healthcare lead to miscommunication between professionals and patients, thereby reducing the quality of and equitable access to healthcare. In African countries, the recognition and formal study of these barriers is severely limited despite Africa having more languages than any other continent. Our study investigates language barriers in healthcare facilities in Zomba district in Malawi, where three major local languages are spoken. METHODS: We employed a mixed methods approach and conducted a questionnaire-led study. Data were gathered at 22 health facilities…

PURPOSE: Language, as an integral aspect of human interaction, plays a pivotal role in the reformation process within the correctional setting. Nigerian correctional centers consist of a diverse population of incarcerated individuals originating from various cultural backgrounds, each possessing distinct linguistic abilities and comprehension. However, a conspicuous gap in the literature remains concerning the language practices of instructors in Nigerian correctional education programs and the active participation of incarcerated individuals in shaping their own educational experiences. The p…

Foreign Language Learners' Uncertainty Experiences and Uncertainty Management.

Uncertainty is unavoidable in life; it is also an indispensable characteristic of educational settings. The process of learning a foreign language naturally involves interaction and coconstruction of knowledge. During this process, students may encounter uncertainty from a variety of sources, including the course, context, friends, or the teacher. Besides, to communicate meaningfully, the mutual effort of both the listener and the speaker is needed to deal with uncertainty. Considering this, the present study makes an effort to comprehend how learners appreciate and manage uncertainty in langu…

Reading and language profiles among children with epilepsy.

This study evaluated the profile of language and reading skills among children with epilepsy. We utilized a sample of children from an epilepsy database who were administered a measure of reading comprehension, excluding those whose intellectual skills were in the impaired range (N=147; age range 4-20 years, 52 % female). Additional measures that were considered within the sample included broad language skills, pre-reading skills (phonological processing, rapid naming, decoding), and basic reading skills (sight word reading, reading fluency). We further considered associations between these sk…

Foreign Language Acquisition in Adolescent Cochlear Implant Users.

OBJECTIVES: This study explores to what degree adolescent cochlear implant (CI) users can learn a foreign language in a school setting similar to their normal-hearing (NH) peers despite the degraded auditory input. DESIGN: A group of native Dutch adolescent CI users (age range 13 to 17 years) learning English as a foreign language at secondary school and a group of NH controls (age range 12 to 15 years) were assessed on their Dutch and English language skills using various language tasks that either relied on the processing of auditory information (i.e., listening task) or on the processing o…

Identifying and describing developmental language disorder in children.

BACKGROUND: In 2016-17 an important consensus was established regarding the use of the diagnostic label 'developmental language disorder' (DLD) to describe children with a persistent language problem having a functional impact on communication or learning and in the absence of any other biomedical condition. Despite this consensus, past research has revealed ongoing uncertainty regarding when to use the DLD label among speech-language pathologists (SLPs). AIMS: In response to this uncertainty, a survey of SLPs was conducted aimed at investigating which types of clinical language profiles, and…

Frontoparietal Anatomical Connectivity Predicts Second Language Learning Success.

There is considerable individual variability in second language (L2) learning abilities in adulthood. The inferior parietal lobule, important in L2 learning success, is anatomically connected to language areas in the frontal lobe via the superior longitudinal fasciculus (SLF). The second and third branches of the SLF (SLF II and III) have not been examined separately in the context of language, yet they are known to have dissociable frontoparietal connections. Studying these pathways and their functional contributions to L2 learning is thus of great interest. Using diffusion MRI tractography, …

Language Impairments in Individuals With Coffin-Siris Syndrome.

Coffin-Siris syndrome (CSS, MIM 135900) is a now well-described, multiple congenital anomaly/intellectual disability syndrome classically characterized by fifth digit/nail hypoplasia, coarse facial features, and a range of organ-system related anomalies. Since its initial description in 1970, and the discovery of associated genes in 2011, CSS now encompasses a wide range of phenotypes and abilities caused by pathogenic variants in the BAF complex (often referred to as "BAFopathy"). It appears that the BAF complex leads to speech and language impairments in this population, and subsequently we …
