Left atrial appendage closure was originally described by Madden in 1949 who was the first to perform appendix amputation. This was a very invasive procedure with poor outcome, but preformed for stroke prevention. Much later, it was James Cox whom described the Cox-Maze procedure, which included multiple incisions to create a maze like pathway for the electrical impulse to go undisturbed from the sinus node down to the atrio-ventricular node. Since then cardiac surgeons stopped focusing on the left atrial appendage and went on about seemingly more important things. Only with the advent of cath…

Atrial fibrillation is associated with a significantly increased risk of stroke, and oral anticoagulation is the mainstay of preventative treatment. Scenarios arise where the risks of treatment with oral anticoagulation may outweigh the benefits, most commonly when there is an elevated risk of bleeding. Studies of percutaneous closure of the left atrial appendage have strongly implicated this structure in the etiology of stroke in atrial fibrillation, and provide some rationale for the discontinuation of oral anticoagulation following percutaneous closure device implantation. A common clinical

UNLABELLED: Cardioembolism associated with atrial fibrillation is a major cause of ischemic stroke. Left atrial appendage occlusion in atria … us should be evaluated after thoracoscopic clipping.
LEARNING OBJECTIVE: To illustrate, incomplete left atrial appendage closure may increase the risk of ischemic stroke even after thoracoscopic left atrial appendage clipping is performed to prevent embolism.

BACKGROUND: Atrial fibrillation (AF) is associated with profound morbidity and mortality, including a fivefold increase in stroke risk. Left atrial appendage occlusion (LAAO) devices have emerged as an alternative approach to anticoagulation in patients with elevated bleeding risk and/or have contraindications to anticoagulation. However, females have been underrepresented in PROTECT & PREVAIL landmark trials supporting LAAO use. We aim to compare gender differences in outcomes of LAAO occlusive devices.
METHODS: The national readmission database (2016-2020) was utilized to identify hospitali

A 74-year-old male with a history of cardioembolic stroke, chronic atrial fibrillation, and cerebral hemorrhage, who had undergone left atrial appendage closure using the WATCHMAN device 1 year prior, was diagnosed with a 25-mm intra-cardiac mass in the right atrium. The patient underwent the surgical removal of the right atrial mass and the explantation of the WATCHMAN device. The WATCHMAN device was explanted with an external incision at the base of the left atrial appendage, facilitating the removal of the device and the closure of the appendage through direct suturing. The pathological exa…

A case is reported of herniation of the left atrial appendage through a partial pericardial defect, probably congenital. The diagnosis was suggested by the history of chest pain and bulging of the middle segment of the left heart border on the plain chest film, without other signs. Angiography revealed a dilated left atrial appendage. An artificial left pneumothorax confirmed the presence of a pleuropericardial defect. The surgical procedure included excision of the appendage and closure of the defect.

Atrial fibrillation significantly increases the risk of thromboembolic events, necessitating anticoagulation for stroke prevention. However, patients with a history of intracranial hemorrhage pose unique management challenges, particularly regarding the use of anticoagulants and the need for dual antiplatelet therapy following procedures like percutaneous coronary intervention. In addition, the occurrence of bradyarrhythmias often necessitates pacing, underscoring the importance of innovative strategies such as left atrial appendage closure devices and leadless pacemakers to manage atrial fibr…

Transcatheter closure of the fossa ovalis atrial septal defect with adequate rims has become the procedure of choice in recent times. We report our experience with 439 patients who were selected for percutaneous device closure and 430 of whom underwent transcatheter closure of fossa ovalis atrial septal defects from May 1997 to May 2006. Device closure was not attempted in the remaining nine patients after initial evaluation due to high right atrial pressure following balloon occlusion (n=1 not parallel, high pulmonary vascular resistance (n=1), significant coronary artery disease (n=1), or th

BACKGROUND: Concomitant left atrial appendage exclusion is increasingly performed during cardiac surgery in patients with atrial fibrillation. However, real-world data on procedural variations, closure integrity, and early safety are limited. METHODS: We retrospectively reviewed 86 patients with atrial fibrillation who underwent concomitant left atrial appendage exclusion during cardiac surgery at a single center between 2012 and 2025. Nineteen patients underwent left atrial appendage occlusion using suture-based techniques or a closure device, and 67 underwent left atrial appendage resection

Atrial septal defect (ASD) closure is a common reason for referral to the cardiac catheterization laboratory. We report a case in which a relatively large Qp:Qs of 1.85:1 was demonstrated in the face of a small secundum ASD. This led to further investigation, ultimately leading to the discovery of an unusual atrial fistula. Rare cases of interatrial tunnels have been described in the literature, however, this is a unique case of a left atrial appendage to right atrial appendage fistula in the setting of a right juxtaposed left atrial appendage.

We present a congenital giant left atrial appendage in an asymptomatic 18-month-old girl featuring two morphological aspects not yet described, namely, a common wall between the giant left atrial appendage and the left ventricle and an abnormal course of the circumflex coronary artery across the surface of the grossly dilated left atrial appendage which prohibited its radical resection. Surgery was performed off-bypass, via anterolateral thoracotomy with plication of the aneurysm, leaving a residual pouch. A patch closure of the communicating os between the left atrium and the residual aneurys

Background and Objectives: Atrial fibrillation is a major cause of ischemic stroke, and in patients with non-valvular atrial fibrillation the left atrial appendage is the predominant site of thrombus formation. Although oral anticoagulation remains the standard strategy for stroke prevention, long-term therapy may be limited by bleeding risk, prior major bleeding, contraindications, intolerance, or difficulty maintaining stable anticoagulation. Endocardial left atrial appendage occlusion (LAAO) has therefore emerged as an alternative strategy in selected patients. Its role continues to evolve

BACKGROUND: The comparative safety and efficacy of Watchman FLX (Boston Scientific, Marlborough, MA, USA) and Amplatzer Amulet (Abbott, Abbott Park, IL, USA) devices for left atrial appendage occlusion (LAAO) remain unclear.
OBJECTIVE: To compare Watchman FLX and Amplatzer Amulet devices for LAAO.
METHODS: We systematically searched PubMed, Embase, and Cochrane Library for studies comparing Watchman FLX versus Amulet in patients with atrial fibrillation (AF) undergoing percutaneous LAAO. We applied a random-effects model to pool risk ratios (RR) with corresponding 95 % confidence intervals (

The advances in the left atrial appendage closure as a stroke prevention method in patients with atrial fibrillation and contraindications to oral anticoagulants made a huge revolution offering a great alternative for patients with a high risk of bleeding. The nitinol-based, self-expanding LAmbre closure system shows promising initial results in terms of efficacy and safety in worldwide different clinical trials offering an alternative to the known Amulet and Watchman closure systems. Lifetech received CE Mark approval for the LAmbre closure system on 15 June 2016. Different studies have shown…

A 70-year-old man underwent cardiac surgery including left atrial appendage closure. A pigtail catheter was inserted into the pericardial sac because of delayed tamponade. Removal of the catheter was planned for 2 days after drain insertion. However, the resistance was high and pulsatile. The patient was transferred to the catheterization laboratory and a guide wire was inserted through the catheter, revealing the catheter route around the left atrial appendage. The wire was exchanged for a stiff wire to uncurl the catheter as much as possible, then the catheter was removed. The left atrial ap…

Left atrial appendage closure is widely performed in cardiac surgery to prevent intracardiac thrombus in patients with atrial fibrillation. Herein, we report the surgical case of an 80-year-old man whose left atrial appendage became aneurysmic long after undergoing suture exclusion. At the age of 67, he underwent mitral valve annuloplasty and left atrial appendage suture exclusion for mitral regurgitation and chronic atrial fibrillation at our institution. At the age of 76, a mass near the left atrial appendage that was difficult to distinguish from a tumor was observed. During follow-up, the …

We report the case of an 83-year-old man with paroxysmal atrial fibrillation who underwent successful percutaneous left atrial appendage closure with the LAmbre device, being in sinus rhythm at implantation. Suddenly, the patient experienced cardiac tamponade and died within a few minutes. Autopsy revealed a slight protrusion of the LAmbre device into the atrial appendage wall, and pulmonary artery laceration. This is the first published report of pulmonary artery perforation by the LAmbre device. This case highlights the need for a detailed imaging study before this procedure is performed, to…

AIMS: Left atrial appendage (LAA) electrical isolation (LAAEI) in addition to pulmonary vein isolation is an emerging catheter-based therapy … rmation is common after LAAEI. Adjustment of OAC leads to LAA thrombus resolution in most patients. Left atrial appendage closure in the presence of LAA thrombi might be a feasible option in case of failed medical treatment.

Empirical left atrial appendage isolation (eLAAi) using a cryoballoon reduces atrial tachyarrhythmia recurrences in persistent atrial fibrillation. Nonetheless, the most significant concern associated with this procedure is the risk of thromboembolic events, particularly without consistent oral anticoagulant (OAC) use. This case highlights a late thrombus formation post-eLAAi despite proper OAC adherence, raising questions about OAC's effectiveness in such scenarios. The case suggests considering percutaneous left atrial appendage closure after eLAAi, even in patients with thrombus and ongoing…
