Iridodialysis is commonly encountered after blunt trauma to the eye. Most iris detachments are small, superior, and asymptomatic and require no surgical intervention. However, large areas of iridodialysis require early surgical repair to prevent the onset of iris necrosis, pigment dispersion, and secondary glaucoma. Suture fixation of iris to the sclera is the most commonly used method for iris repair; however, this technique becomes difficult in cases of near-total iris disinsertion, even in expert hands. We describe a case of posttraumatic near-total iris disinsertion with subluxated catarac

PURPOSE: The light adjustable lens (LAL) (RxSight, Aliso Viejo, CA) is a premium intraocular lens that allows for correction of residual refractive error and astigmatism following implantation. Herein, we describe the surgical approach and evaluate the visual outcomes of patients following scleral fixation of the LAL.
METHODS: Retrospective, single-surgeon surgical case series of 3 patients (3 eyes) with intraocular lens complications, who underwent combined pars plana vitrectomy and sutureless needle assisted intrascleral haptic fixation of the LAL between April 2022, to August 2023.

PURPOSE: To describe the vitreoretinal surgical technique and report the outcomes of our method of sutureless flanged intrascleral haptic fixation of dislocated 1-piece polymethyl methacrylate intraocular lenses with rigid haptics.
METHODS: Ciliary sulcus-based scleral tunnels were created by placing valved 27-gauge (g) trocar cannulas limbus parallel with conjunctival displacement. After complete vitrectomy, the rigid haptics were then externalized using 27g forceps. Cautery was then used to form flanges at the haptic tips. The haptics were then pushed back into the mouths of the scleral tun

BACKGROUND: To report a case of sutureless scleral-fixated hydrophilic intraocular lens (FIL SSF IOL, Soleko, Italy) opacification following pars plana vitrectomy surgery using sulfur hexafluoride (SF6) for traumatic lens luxation associated with retinal detachment.
CASE PRESENTATION: A 77-year-old woman was referred to our emergency department after blunt trauma in her right eye. At the ophthalmic evaluation, visual acuity was hand movement, biomicroscopy showed pseudoexfoliation syndrome and a traumatic lens luxation in the vitreous chamber. The patient underwent pars plana vitrectomy, subl

Eye injuries caused by sharp instruments range from superficial scratches of the corneal epithelium to serious lacerations of the globe of the eye. Scratches, conjunctival lacerations, and some eyelid lacerations can be dealth with by the primary physician in the office, but damage to the globe requires immediate referral to an ophthalmologist. Among possible results of blunt trauma to the area of the eye are ecchymosis, hyphema, blow-out fracture, subluxation or dislocation of the lens, or retinal detachment. Most of thes require thoroguh ophthalmologic examination and specialized treatment.

Dislocation of intraocular lens (IOL) is a serious complication of blunt ocular trauma in pseudophakic eyes. Here, a 72-year-old male patient with subconjunctival dislocation of an IOL (pseudophacocele) secondary to bull horn injury was reported. In this case report, a new sign named as "golden half ring sign" was described for easy identification and localization of subconjunctival dislocation of IOL in patient with open globe injury (surgical wound dehiscence) associated dense subconjunctival hemorrhage.

This article presents a case of bilateral posterior dislocations of the crystalline lens in a patient with epilepsy who presented with reduced vision and anisocoria 2 weeks after having sustained head injuries during a seizure. The possibility of lens dislocation was raised only at this time, and subsequently confirmed on computed tomography. Such patients may require prompt referral to the ophthalmologist to treat complications such as functional aphakia, uveitis and more seriously pupillary block glaucoma. This case highlights the importance of ocular examination of head injuries to rule out

The ocular findings in patients with homocystinuria due to cystathionine-beta-synthetase (CBS) deficiency are reviewed and a new approach to lens surgery is presented. Lens surgery in CBS deficiency was reported to be accompanied by a high risk of secondary complications like vitreal prolapse and retinal detachment. So far an operative procedure for these patients that allows a complete preservation of the anterior vitreous cortex, the posterior lens capsule and an implantation of an artificial lens is not established. A minimal invasive method of lens surgery was developed in vitro and applie

I performed a one-port pars plana vitrectomy during phacoemulsification to manage vitreous loss with and without posterior dislocation of lens fragments in nine consecutive cases. I used a 1 mm vitrectomy instrument with an infusion sleeve for the vitrectomy; the coaxial light of the microscope and an infusion contact lens held on the cornea provided intraocular illumination. Eight of the nine cases achieved a visual acuity equal to or better than 20/25 after a mean follow-up of 5.7 months. One patient's visual acuity was 20/70 because of severe background diabetic retinopathy.

Clinical data of 12 adult cases of dislocation of lens treated with couching-netting operating were analysed, including cases of traumatic cataracts with complications of glaucoma and cataract couching surgical complication of phacolysis. The result was rather satisfactory. Eye sight was corrected, most of the corrective vision was over 0.5 level, intraocular pressure was controlled and no serious complication was found. According to the authors experience, couching-netting was recommended as a simple and effective method for treating dislocation of adult lens.

Ectopia lentis is a common denominator of some connective tissue diseases such as Weill-Marchesani, Ehlers-Danlos, and Marfan's syndromes, osteogenesis imperfecta and homocystinuria. Early differential diagnosis (e.g., Marfan's syndrome vs. homocystinuria) is mandatory for prognostic (aortic aneurysm in Marfan's syndrome), therapeutic (dietetic and vitamin therapy in homocystinuria), and genetic reasons (genetic counseling).
