Lichen planus responding to efalizumab.

Lichen planus (LP) is a common inflammatory skin disorder of unknown origin. When generalized and relapsing, treatment of LP can be a challenge. We present a patient with widespread and relapsing generalized LP who was treated with efalizumab for 3 months. Both pruritus and the skin lesions responded promptly to this biologic. In accordance with the previously reported efficacy in oral erosive LP, efalizumab may be a promising novel future treatment option for patients with cutaneous LP.

Annular atrophic lichen planus.

A 30-year-old woman presented with a 1-year history of a pruritic eruption on the extremities, characterized by several annular plaques. The patient had been treated unsuccessfully with medium-potency topical steroids. The lesions had an erythematous papular border with an atrophic center (width, 1-4 cm) (Fig. 1). No oral, genital, or nail lesions were observed. A skin biopsy from one of the plaques was performed. Histopathologic examination of the raised border showed hyperkeratosis of the stratum corneum, focal thickening of the granular layer, basal liquefaction degeneration of the epidermi

[Lichen planus with esophageal involvement].

Lichen planus is an inflammatory disease of unknown etiology which may affect the esophagus. Patients with esophageal lichen planus are usually elderly women. The most frequent symptoms are dysphagia and odynophagia. There is usually concomitant oral involvement in cases of esophageal lichen planus. Endoscopic findings include erosions, exudates and stenosis. Systemic steroids are the most effective therapy for these patients. We report a 68-year-old woman with oral, genital and submammary lesions of lichen planus of nine months duration, that referred dysphagia and odynophagia. The endoscopic

Thymoma accompanied by lichen planus.

Thymomas are associated at a high frequency with paraneoplastic autoimmune diseases. We treated a 64-year-old male with a thymoma, who also had lichen planus. The tumor was resected and diagnosed as thymoma, however, the symptoms associated with lichen planus did not subside and persisted. A preoperative examination showed an elevated serum level of squamous cell carcinoma antigen, which gradually decreased to normal after surgery. The findings of this case are interesting for understanding the correlation between a thymoma and autoimmune abnormalities.

[Lichen sclerosus in children with phimosis].

AIM: Lichen sclerosus, also known as lichen atrophicus, balanitis xerotica obliterans and kraurosis vulvae, is a chronic limphocytes-depending dermatitis predominantly found in the anogenital area of any age group, sex or race. It is most commonly found in Caucasian women: the female to male ratio ranges from 6:1 to 10:1. The real incidence of the disease observed in the literature depends remarkably on the age and sex group studied. The incidence in the pediatric population ranges from 10-15% to nearly 95% if we consider only boys with truly sclerosed and stenotic foreskins. METHODS: Our pur

Treatment of lichen sclerosus with antibiotics.

Current therapy for lichen sclerosus centers on topical steroids, particularly clobetasol propionate. As some evidence suggests an infectious etiology owing to Borrelia, we studied the effect of penicillin and cephalosporin therapy on patients with lichen sclerosus who had responded poorly to treatment with potent topical corticosteroids. Fifteen patients with lichen sclerosus were treated for 3-21 months with either penicillin or cephalosporins in an observational study. Thirteen patients (nine women, four men) received penicillin, including intramuscular penicillin G benzathine suspension an

[Vulvar lichen planus in children].

BACKGROUND: Lichen planus is less common in children about in adults and mostly affects the skin. CASE REPORT: A 9-year-old girl was referred for a purplish blue and violaceous, unilateral and asymptomatic vulvar lesion noted 4 months earlier and stable. Histopathologic examination showed typical features of lichen planus. She responded well and rapidly with topical steroids. DISCUSSION: We report the first case of vulvar lichen planus in a little girl. Mucosal involvement is uncommon in children with lichen planus and genital localisation is extremely rare. Vulvar lichen sclerosis is the pr

A case of linear lichen planus.

Linear lichen planus (LLP) is a very rare form of lichen planus (LP) characterized by a linear distribution of the lichenoid lesions. It usually follows the lines of Blaschko with unilateral involvement. We report such a case of LLP. A 50-year-old man presented with slightly pruritic, linear, violaceous, papular lesions on the right side of his jaw. A biopsy specimen demonstrated the typical histology of LP. Based on the clinical and pathological findings, the diagnosis was LLP.

Peno-gingival lichen planus.

BACKGROUND: Lichen planus with simultaneous oral and genital involvement in males is a quite rare condition and often difficult to diagnose. The prevalence, treatment options, and management for this condition are far from being established, and research in this area primarily relies on anecdotes. We present the clinical features of eight cases of peno-gingival lichen planus and propose a management algorithm for this condition based on the best available published evidence. METHODS: Personal medical history was collected for all cases. Following careful examination of the oral and genital mu

Immune dysregulation in lichen sclerosus.

Lichen sclerosus (LS) is a chronic localized lymphocyte-mediated dermatosis of genital skin with a presumed autoimmune origin. LS is characterized by localized dense lymphocytic tissue infiltrates, vasculitic processes and extensive tissue destruction. The lymphocytic infiltrate of LS biopsies contains between 1.4% and 21% of T-cells with monoclonally rearranged T-cell receptor gamma-chain gene, and the immunophenotype is dominated by B-cells, CD4-positive T-cells and antigen-presenting dendritic cells. Antigen-driven selection of T-cells and restricted T-cell receptor usage reflects prolonged

Lichen striatus in a pregnant woman.

We report a case of lichen striatus in a 37-year-old woman in the third trimester of pregnancy. She presented at 35 weeks of pregnancy with a 6-week history of a pruritic rash on the left side of her abdomen. Examination revealed an erythematous papular eruption arranged in a linear fashion from the umbilicus to the left mid back. Histological examination demonstrated lichenoid perivascular and periadnexal inflammatory infiltrate and the presence of colloid bodies. The eruption resolved prior to delivery with the application of mometasone furoate 0.1% ointment.
