PURPOSE: Actinic keratoses are commonly the result of intense sun damage to the skin and lips of susceptible patients. The purposes of this 2-section article are to familiarize the surgeon with options for care and to suggest methods of incorporation into practice as a true benefit to patients.
PATIENTS AND METHODS: The first section discusses the options chosen for patients who were referred to the Facial Lesion Clinic in a county hospital for precancerous or cancerous facial lesions; the second section reviews ramifications for precancerous sun-damaged lips with clinical leukoplakia.

Superficial mucocele is a relatively rare bullous disease that develops in the oral mucosa. Although the number of reported cases is limited, it seems that the superficial mucocele has been recognized as an independent disease belonging to a single entity. The present report describes a male with tuberous sclerosis and an extensive open lipped schizencephalic cleft affecting his right frontoparietal lobe. This case supports the hypothesis that the cerebral lesions in tuberous sclerosis are secondary to a defect in neuronal maturation and migration.

Traumatic arteriovenous fistulas can present days to years following penetrating trauma and are often challenging to diagnose and manage. Patients may present to the dermatologist with unilateral varicose veins or a pulsatile mass. Our case illustrates the value of palpation in a careful systematic approach to any new lesion, especially in the context of previous penetrating trauma. We also discuss the nomenclature of arteriovenous communications and review their current management.

Elephantiasis nostras (EN) is a clinical entity that usually presents as a persistent swelling of the lower extremities. It has been related to recurrent lymphangitis of bacterial origin that causes a fibrosis and thickening of both epidermal and connective tissue. Holoprosencephaly includes a spectrum of conditions that have in common the associated anomalies of false median cleft lip, nasal malformation, orbital hypotelorism, and brain malformations. We recently encountered a patient with holoprosencephaly presenting with a rare unilateral cleft of the lip. This patient was potentially viable, although mental development was retarded.

Extension of lip cancer to the mental nerve and perineural spread was found in four patients. The main complaint in all cases was sensory disturbance which preceded the diagnosis of recurrence by perineural spread as well as widening of the mandibular canal. In one patient, two cranial nerves, the trigeminal and the facial were involved and one patient had bilateral neural involvement. Melanomas in the anatomically complex area of the head and neck impose formidable obstacles to treatment. Mucosal melanomas, for reasons which remain uncertain, are associated with a poorer prognosis. Because no systemic treatment is known to be effective, increased awareness by patients and a high index of suspicion is warranted. The disease is potentially curable if diagnosed and treated at an early stage.

The advantages of palate repair by double opposing Z-plasty include excellent access for complete mobilization of the palatal muscles, construction of an overlapping muscle sling, and avoidance of the growth-limiting horseshoe-type scars of the hard palate that result from lateral relaxing incisions or pushback procedures.
