177Lu can be imaged after administration using SPECT/CT. Most work to date has focused on using posttreatment imaging to measure normal organ and tumor dose. We aimed to assess the impact of posttreatment SPECT/CT on the management of patients undergoing 177Lu-prostate-specific membrane antigen (PSMA) radiopharmaceutical therapy (RPT). Methods: In this retrospective study, 122 patients underwent PSMA RPT with subsequent SPECT/CT 24 h after treatment. We determined a qualitative response at each cycle and reviewed patient charts to assess the impact that posttreatment SPECT/CT had on patient ma

We present a case involving a 9-year-old boy diagnosed with metastatic carotid body paraganglioma. The metastases were detected in cervical lymph nodes and lungs using 68 Ga-DOTANOC PET/CT imaging. The patient received peptide receptor radionuclide therapy with 177 Lu-DOTATATE. Following 3 treatment cycles, a significant improvement was observed in the metastatic lesions. After 4 cycles, the patient achieved a complete response, with a cumulative administered activity of 16.65 GBq during the therapy. This case underscores the effectiveness of using 177 Lu-DOTATATE in managing metastatic caroti

The VISION and TheraP trials introduced different PET-based criteria for patient selection for treatment with 177Lu-PSMA-617 (LuPSMA). TheraP used a higher prostate-specific membrane antigen (PSMA) uptake threshold than VISION and required 18F-FDG PET to exclude patients with discordant findings. Although the screen-failed patients had shorter overall survival (OS) than those treated with LuPSMA, it remains unclear whether their outcomes might have been modified if they had been exposed to LuPSMA. In this study, we evaluated associations between the TheraP eligibility criteria and subgroups an

PURPOSE: AG10 is a molecule that can bind to the thyroxine (T4) binding pocket of transthyretin (TTR). The reversible association between the hydrophilic peptides and TTR using AG10 could increase the circulation half-life (t1/2). We hypothesized that modifying PSMA-617 with AG10 could extend the t1/2 of PSMA-617 and mitigate the limitations caused by insufficient uptake. The aim of this study was to develop an optimal AG10-modified radiotherapeutic agent that could maximize both tumor uptake and absorbed dose, thereby enhancing therapeutic efficacy for the treatment of tumors, even with moder

Radiopharmaceutical therapy with [177Lu]Lu-PSMA-617 has improved outcomes for patients with advanced prostate cancer (PCa), yet modest survival benefits underscore the need for more effective agents. Here, we describe the development of [177Lu]Lu-ART-101, a next-generation prostate-specific membrane antigen (PSMA)-targeting radioligand, and evaluate its biodistribution, dosimetry, therapeutic efficacy, and toxicity in advanced PSMA-expressing PCa models. Methods: Lipophilic ART-101 was synthesized and radiolabeled with 177Lu. [177Lu]Lu-ART-101 longitudinal biodistribution and dosimetry were as

BACKGROUND: Personalized pretreatment dosimetry planning is crucial for optimizing [177Lu]Lu-prostate-specific membrane antigen-617 (Lu-PSMA) radioligand therapy (RLT) in patients with metastatic castration-resistant prostate cancer (mCRPC).
PURPOSE: This study addresses two goals. First, we develop a machine learning (ML)-based pretreatment planning model to predict post-therapy absorbed doses (ADs) in metastatic lesions by integrating clinical biomarkers (CBs) with radiomic features (RFs) and dosiomic features (DFs) extracted from [⁶⁸Ga]Ga-PSMA-11 (Ga-PSMA) positron emission tomography/comp

OBJECTIVE: Fibroblast activation protein alpha (FAPα) plays a key role in cartilage degradation, inflammation, and bone erosion, particularly in rheumatoid arthritis (RA) where fibroblast-like synoviocytes in synovial tissue show elevated FAPα expression. This study explored radiolabeled FAP inhibitors for arthritis diagnosis and therapy.
DESIGN: We used the radiotracer 68Ga-FAPI-04 for PET/CT imaging to predict collagen-induced arthritis (CIA) onset. Weekly scans quantified tracer uptake via SUV values, correlating results with disease scores and incidence. For therapeutic evaluation, 177Lu-

A 76-year-old woman affected by pancreatic neuroendocrine tumor previously subjected to surgery with progressive liver disease and a concomitant mild symptomatic meningioma of the left pontocerebellar angle underwent 4 cycles of peptide receptor radionuclide therapy with 177 Lu-DOTATATE. A prophylactic therapy with corticosteroids was carried out before each treatment cycle, and the neurosurgery unit was alerted in case of cerebral edema and related neurologic symptoms. A 68 Ga-DOTATOC PET/CT scan performed after the completion of the 4 cycles' treatment documented a hepatic partial response a

68 Ga-DOTATATE PET/CT is indicated for selecting patients for peptide receptor radionuclide therapy (PRRT). Although highly sensitive, the detectability of smaller lesions, particularly in the liver, is lower. We present the case of a 58-year-old man with metastatic well-differentiated pancreatic neuroendocrine tumor whose MRI revealed progression of hepatic metastases. 68 Ga-DOTATATE PET/CT performed to determine eligibility for PRRT did not demonstrate DOTATATE-avid disease within the liver. 18 F-FDG PET/CT was also negative at the liver and the patient proceeded to 177 Lu-DOTATATE PRRT, whe

Today's common practice of empiric activity prescription in radiopharmaceutical therapy is inconsistent with the modern philosophy of personalized medicine. We investigate the radiobiologic relationship between tumor mean absorbed dose (D mean) and progression-free survival (PFS) in 177Lu-PSMA I&T therapy guided by personalized predictive dosimetry in metastatic castration-resistant prostate cancer. Methods: We conducted a single-center retrospective study of the first 20 patients treated with 177Lu-PSMA I&T at our institution. PFS was calculated from the day a patient was assessed by a nuclea

Prostate-specific membrane antigen (PSMA) is an ideal target of prostate cancer (PCa) for theranostics, combining diagnosis and therapy in the field of nuclear medicine. [177Lu]Lu-PSMA-617 is a gold standard in PSMA-targeting radioligands, whereas its rapid clearance from the tumor and high uptake in the kidney may compromise the efficacy of theranostics. In this study, we developed novel PSMA-targeting radioligands, [111In]In/[225Ac]Ac-PDI2 and [111In]In/[225Ac]Ac-PDI4, by introducing a positively charged diethylenetriamine (PEI2) or tetraethylenepentamine (PEI4) structure, respectively, to P

BACKGROUND AND OBJECTIVE: Angiogenesis-targeting tyrosine kinase inhibitors (TKIs) and immune checkpoint inhibitors (ICIs) are standard treatments for metastatic clear-cell renal cell carcinoma (ccRCC). However, therapeutic options remain limited after progression on these agents. Prostate-specific membrane antigen (PSMA) is highly expressed on the surface of endothelial cells in ccRCC, making radioligand therapy with [177Lu]Lu-PSMA-617 a promising approach.

BACKGROUND: Enzalutamide and lutetium-177 [177Lu]Lu-prostate-specific membrane antigen (PSMA)-617 both improve overall survival in patients with metastatic castration-resistant prostate cancer. Androgen and PSMA receptors have a close intracellular relationship, with data suggesting complementary benefit if targeted concurrently. In this study, we assessed the activity and safety of enzalutamide plus adaptive-dosed [177Lu]Lu-PSMA-617 versus enzalutamide alone as first-line treatment for metastatic castration-resistant prostate cancer.
