Cervical Tuberculous Lymphadenitis in a Healthy Individual Following Latent Tuberculosis Treatment: A Case Report.

A 33-year-old healthy female, employed as a hospital-based physical therapist, was exposed to a colleague diagnosed with pulmonary tuberculosis. Screening revealed a positive interferon-gamma release assay (IGRA) result. She was diagnosed with latent tuberculosis infection (LTBI) and completed a 6-month isoniazid (INH) regimen. Nine months post-treatment, she presented to the otolaryngology department with right cervical swelling and ulceration. Polymerase chain reaction (PCR) of the pus confirmed Mycobacterium tuberculosis, establishing a diagnosis of cervical tuberculous lymphadenitis. She w…

Penile ulcers unmasking disseminated tubercular lymphadenitis in a patient with idiopathic interstitial lung disease.

We report a rare case of penile papulonecrotic tuberculid (PNT) in a middle-aged male with idiopathic interstitial lung disease (ILD) on chronic steroids, presenting with recurrent, painless genital ulcers. Systemic evaluation revealed disseminated tubercular lymphadenitis as the primary focus. The patient was diagnosed with PNT based on clinical and histopathological findings, and tuberculosis was confirmed through imaging and cytology. This case highlights the diagnostic challenge of penile PNT, particularly in immunosuppressed individuals, and underscores the importance of comprehensive tub…

Purulent lymphadenitis caused by ST59 community-associated Methicillin-resistant Staphylococcus aureus in an infant.

INTRODUCTION: Methicillin-resistant Staphylococcus aureus (CA-MRSA), which has the potential to produce serious infections, was a common cause of skin and soft tissue infections, acute purulent lymphadenitis was rare. CASE REPORT: The patient was a female infant with lumps, tenderness, and fever on the right side of the neck and groin. Laboratory tests suggested a bacterial infection. The diagnosis of acute purulent lymphadenitis was made based on the clinical signs and the results of a supporting exam. After three days, MRSA developed in the secretions of suppurative lymph nodes. Her mother'…

Cat Scratch Disease: An Unusual Case of Right Inguinal Lymphadenitis Due to Bartonella henselae.

Cat scratch disease (CSD) is caused by a bacterial infection due to Bartonella henselae and is associated with young cats and kittens. CSD commonly occurs as regional lymphadenitis in the setting of subacute regional lymphadenopathy predominantly in children and young adults. The prognosis for immunocompetent patients is favorable with complete recovery, however, immunocompromised adults can progress to life-threatening complications such as neuroretinitis, osteomyelitis, and bacillary angiomatosis. B. henselae is transmitted from cats to humans through scratching or biting when located on the…

Toxoplasma cyst detection in Piringer-Kuchinka lymphadenitis. Report of two cases and literature review.

The diagnosis of acute toxoplasmic lymphadenitis is traditionally based on the combination of lymph node excisional biopsy with specific tests. The classic triad (marked follicular hyperplasia, small irregular clusters of epithelioid histiocytes in germinal centers, and sinusoidal distension by monocytoid B lymphocytes) is considered diagnostic of the so-called Piringer-Kuchinka lymphadenitis. Toxoplasma gondii organisms have been exceptionally disclosed in such histopathological setting, establishing the diagnosis of toxoplasmic lymphadenitis. Two cases of Piringer-Kuchinka lymphadenitis with…

Empirical treatment with non-anti-tuberculosis antibiotics decreased microbiological detection in cervical tuberculous lymphadenitis.

Diagnosis of cervical tuberculous lymphadenitis (CTL), the most commonly occurring form of extrapulmonary tuberculosis, remains as a challenge in clinic. Detection of the presence of Mycobacterium tuberculosis (Mtb) in fine needle aspiration cytology (FNAC) samples is one golden criterion to confirm the CTL diagnosis. Due to the non-specific clinical presentation, CTL might be confused with other lymph node enlargement diseases; therefore empirical treatment with non-anti-TB antibiotics is often initially administered. However, it is still unclear whether this diagnostic antibiotic treatment a…

Lymphadenitis associated with cat-scratch disease simulating a neoplasm: Imaging findings with histopathological associations.

The lymphadenitis associated with cat-scratch disease (CSD) is often confused with neoplasms by a number of radiologists and clinicians, and consequently, unnecessary invasive procedures or surgeries are performed. In the present study, the contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI) findings of 10 patients (6 men and 4 women) with clinically and pathologically confirmed lymphadenitis associated with CSD were retrospectively analyzed (CT in 3 patients, MRI in 6 patients, and CT and MRI in 1 patient) at The Second Affiliated Hospital of Zhejiang University Sc…

Broadening the Morphologic Spectrum of Bartonella henselae Lymphadenitis: Analysis of 100 Molecularly Characterized Cases.

Bartonella henselae lymphadenitis, or cat-scratch lymphadenitis (CSL), is classically associated with stellate microabscesses, occasional giant cells, and extension of the inflammatory infiltrate into perinodal soft tissue. Availability of B. henselae molecular testing on tissue specimens has broadened our understanding of the morphologic variation in this disease. Here we sought to describe the histopathologic features of the largest series to date of molecularly proven CSL. B. henselae polymerase chain reaction-positive tissue specimens from 2010 to 2012 were identified, and hematoxylin and …

Increase of DC-LAMP+ mature dendritic cell subsets in dermatopathic lymphadenitis of mycosis fungoides.

BACKGROUND: Little is known about the immunological milieu of the skin-draining lymph nodes (LNs) in mycosis fungoides (MF). OBJECTIVES: We studied dendritic cell (DC) subsets in the dermatopathic lymphadenitis of MF patients. METHODS: We immunohistochemically examined DC subsets and their distribution in 16 LN samples from 14 patients with MF (N1 LN, eight patients; N2, four; and N3, four), and we compared them with non-metastatic sentinel LNs from eight patients with melanoma. RESULTS: The number of S-100 protein+ DCs was markedly increased in the LNs from the MF patients and the major co…

Kikuchi-Fujimoto disease (histiocytic necrotizing lymphadenitis): report of a case with other autoimmune manifestations.

Kikuchi-Fujimoto disease (KFD), or histiocytic necrotizing lymphadenitis, is a benign and self-limited disease that mainly affects young women. Patients present with localized lymphadenopathy, fever, and leukopenia in up to half of the cases. KFD can occur in association with systemic lupus erythematosus. We present the case of a patient with KFD and systemic lupus erythematosus, as well as relapsing polychondritis. This patient had persistently low C4 complement levels, so she was evaluated for a genetic defect in complement production and was found to have two "null" C4 alleles. We believe t…

T cell mediated immune responses in patients with tuberculous lymphadenitis from Butajira, southern Ethiopia.

The control of tuberculosis (TB) requires improved vaccines in addition to chemotherapy. It is essential to understand the immune response in tuberculosis to successfully evaluate potential vaccines. Current investigations have focused on immune responses in pulmonary forms. We studied the T-cell response of peripheral blood mononuclear cells (PBMC) from HIV-infected (n=8) and non-infected patients (n=19) with lymph node tuberculosis to PPD and short-term culture filtrates (ST-CF) of M. tuberculosis. PBMC from HIV-negative TB lymphadenitis patients proliferated in response to both antigens (p<…

Atypical Pneumocystis carinii infection in AIDS: massive cervical lymphadenitis and fever of unknown origin.

Pulmonary Pneumocystis carinii infections are relatively common in patients with the acquired immunodeficiency syndrome (AIDS). Extrapulmonary pneumocystis is a less common manifestation, particularly when it occurs without concurrent Pneumocystis carinii pneumonia. Disseminated pneumocystis is most commonly found in lymph nodes, the liver, and the spleen and may result in nonspecific debilitating illness, which is often overlooked in the absence of pulmonary symptoms. We present the case of an AIDS patient who had massive cervical pneumocystis lymphadenitis and minimal pulmonary infiltrates o…

Mycobacterium bovis lymphadenitis complicating BCG immunization in an infant with symptomatic HIV-1 infection.

A 3-month-old infant with HIV-1 infection who recently immigrated from Ethiopia developed regional lymphadenitis and systemic symptoms subsequent to BCG immunization. She was suffering from axillary lymphadenitis ipsilateral to the BCG vaccination site, failure to thrive, unresolving fever and hepatosplenomegaly. Acid-fast bacilli were seen on staining and Mycobacterium bovis was isolated from the regional lymph node. The infant responded promptly to triple antituberculous therapy but died 2 months later from overwhelming pneumonia and respiratory failure. This case emphasizes the iatrogenic h…

Bilateral Parotid Tuberculous Lymphadenitis Mimicking Warthin Tumor in an Elderly Male: A Case Report.

Tuberculosis involving the parotid lymph nodes is rare and can clinically and radiologically mimic benign neoplasms such as Warthin tumor, particularly when bilateral. We present the case of a 72-year-old male with bilateral parotid swellings, initially suspected to be Warthin tumors based on magnetic resonance imaging (MRI) findings showing well-defined, cystic lesions with focal solid components and central necrosis. Histopathological examination following ultrasound-guided biopsy revealed caseating granulomas consistent with tuberculous lymphadenitis. This case highlights the importance of …
