INION compared with titanium osteosynthesis: a prospective investigation of the treatment of mandibular fractures.

We prospectively studied two groups of 30 patients to assess the outcome of treatment of mandibular fractures with the biodegradable INION system compared with osteosynthesis with titanium miniplates. The degree of occlusion, wound healing, and swelling, were noted preoperatively and at 1 week, 6 weeks, and 6 months postoperatively. All fractures healed uneventfully, both clinically and radiologically, and independently of the osteosynthesis used. We found no long-term disturbance of occlusion, but there were twice as many malocclusions in the INION group at one week. We now use a 3-5 day peri…

Intraosseous mandibular schwannoma mimicking an odontogenic keratocyst, with a postsurgical pathological fracture.

OBJECTIVE: Schwannomas are slowly growing tumours derived from Schwann cells. We present a clinical case of schwannoma in the mandibular angle. METHOD: Case report and a review of the world literature concerning intraosseous schwannoma of the maxillofacial region. RESULTS: Schwannomas or neurilemmomas are slow-growing, benign neoplasms derived from Schwann cells. Intraoral lesions are unusual and intraosseous schwannomas are even rarer, representing less than 1 per cent of benign primary tumours of the bones. We present a clinical case of schwannoma in the mandibular angle mimicking a kerato

Comparison of mandibular premolars and canines with respect to their resistance to vertical root fracture.

OBJECTIVE: To compare the force required to vertically fracture uninstrumented and instrumented mandibular premolars and canines. METHODS: Two groups of single round-canal mandibular premolars and two groups of single oval-canal mandibular premolars were selected after radiographs indicated an internal long:short diameter of 1 or >2 at a level 5 mm from the apex. In addition, two groups of maxillary and mandibular canines were selected. The root canals of three of these groups (one from each category) were conventionally enlarged using same-sized instruments, while the other three groups were

Computed tomographic findings of the fractured mandibular condyle after open reduction.

The purpose of this study was to evaluate the long-term radiological results obtained with open reduction and fixation of unilateral mandibular condyle fractures in 10 patients. CT images taken at the end of the follow-up period (average of 22 months, range 7 to 33 months), were traced and digitized, and the position and morphology of the fractured condylar process was statistically compared with those of the contralateral non-fractured condylar process in the coronal, transverse and sagittal planes. Little difference was observed in the position or morphology of the condylar process in the op

Assessment of patients treated for intracapsular fractures of the mandibular condyle by closed techniques.

PURPOSE: The goals to study different lines of intracapsular fractures of the mandibular condyle and to evaluate their influence on the prognosis after closed treatment. PATIENTS AND METHODS: Clinical, radiologic, and axiographic follow-up of 40 patients with 50 intracapsular fractures of the mandibular condyle was carried out after closed treatment. The examinations were performed an average of 22 weeks after treatment. Three types of intracapsular fractures were distinguished: type A, or fractures through the medial condylar pole; type B, or fractures through the lateral condylar pole with

Etiology and management of mandibular fractures associated with endosteal implants in the atrophic mandible.

Mandibular fractures can occur with the insertion of endosseous implants. Four patients whose mandibles were fractured with the removal or insertion of mandibular endosseous implants are described. Three of the patients required an autogenous bone graft to repair the fracture, and 1 patient was managed with a reconstruction plate. Strategies for prevention and treatment of this uncommon complication are discussed.

Bifid mandibular condyle resulting from a sagittal fracture of the condylar head.

Sagittal or vertical fractures of the mandibular condyle and chip fractures of the medial part of the condylar head are very rare entities and their identification is difficult with conventional radiographs. We report a case of a unilateral bifid mandibular condyle which resulted following a sagittal condylar fracture. This was associated with a chip fracture of the medial part of the condylar head on the contralateral side. These types of condylar fracture do not require any surgical treatment, but early mobilization is indicated.

A comparative study of the treatment of unilateral fractured and dislocated mandibular condyles in the rabbit.

This study evaluated the results of three methods of managing simulated unilateral dislocated fractured mandibular condyles in the rabbit. The first method consisted of surgical reduction of the fractured condyle to its anatomic position and its subsequent fixation with a titanium plate. The second method differed from the first by completely detaching the condyle, then replacing it as free graft. The third method left the fractured condyle in its dislocated position outside the glenoid fossa. Eighteen rabbits were used, 6 animals for each method. Half the animals in each group were killed at

Changes in the main masticatory muscles in CT after mandibular condyle fracture.

The aim was to study changes in the main masticatory muscles after mandibular condyle fracture as seen by CT, comparing the densities and sizes of the masseter, medial pterygoid and lateral pterygoid muscles on the fractured side with those on the non-fractured side. More or less permanent muscular changes are detectable several years after the accident, especially in the lateral pterygoid muscle, which is attached to the condylar head. This muscle was statistically significantly smaller (p less than 0.001) on the fractured side. Examination of the masticatory muscles immediately after the fra

Aseptic osteomyelitis and necrosis of the mandibular condylar head after intracapsular fracture.

Aseptic osteomyelitis and necrosis of the mandibular condylar head is a rare complication which may occur several months after an untreated intracapsular fracture. Clinical symptoms are pain in the temporomandibular joint on the affected side and limitation of opening of the mouth. Radiographically, the condylar head is characteristically eroded and irregular. Treatment consists of surgical removal of the necrotic condylar head and débridement of the area, which are performed via a preauricular approach.

Effectiveness of Single Miniplate Fixation for Mandibular Symphysis and Parasymphysis Fractures: A Clinical Study.

Introduction Symphysis and parasymphysis fractures are common in maxillofacial trauma, requiring precise management to restore form, function, and aesthetics. While Champy's technique recommends two miniplates - superior and inferior - for optimal stability, placing a superior plate in the canine-premolar region risks damaging dental roots and neurovascular structures. Additionally, the cost of dual plating may be a burden in low-resource settings. This study evaluates the efficacy of a single miniplate at the inferior border, aided by an arch bar as a tension band, for managing these fracture

Advances in Open Reduction and Internal Fixation of Multiple Mandibular Fractures with Condylar Involvement.

Multiple mandibular fractures with condylar involvement are challenging to treat. This article presents a series of steps to simplify the treatment and improve the outcomes in open reduction and internal fixation of multiple mandibular fractures with condylar involvement. Four new elements are identified: three-dimensional printing, model surgery, Kirschner wires to manipulate the segments in condylar fractures, and a top-to-bottom sequence. This algorithm has contributed to streamlining the procedure and improving the outcomes of these challenging fractures. For the same operator, reduction a…

Evaluation and Prediction of Healing Morphology After Closed Reduction for Unilateral Mandibular Condyle Fractures.

Although closed reduction is common for condylar fractures, bone fragments may heal improperly. This study aimed to investigate the healing morphology of unilateral condylar fractures. We retrospectively investigated 70 patients with unilateral condylar fractures. Clinico-statistical analyses were performed on the whole-condylar fracture, closed reduction, and observation/functional therapy groups. Among these patients, 52 patients aged older than 16 years underwent closed reduction. The extent of maximum mouth opening, the incidence of malocclusion, and the relationship between healing morpho
