Mucoepidermoid carcinoma of the middle ear: a case report.

Primary mucoepidermoid carcinoma of the middle ear is a rare malignancy. We report a case of 58-year-old woman who presented with purulent discharge from the left ear for seven months, followed by bloody otorrhea for one month. She was diagnosed with mucoepidermoid carcinoma of the middle ear after mastoidectomy. Then, she received x-ray radiation therapy (total dose: 3,000 cGy). However, nine months later, she underwent a comprehensive surgical intervention, including subtotal temporal bone resection, partial parotidectomy, and neck dissection, due to local tumor recurrence. The patient succu…

Surgical Option for External Auditory Canal Cholesteatoma: A Case Report.

External Auditory Canal Cholesteatomas (EACC), is an exceptionally rare condition with a prevalence of only 0.1-0.5% among new patients1. EACC are known to possess bone eroding properties, causing a variety of complications, similar to the better-known attic cholesteatomas. We describe here the novel surgical management of a case of EACC. She is 38-year-old female who presented with otorrhea for 6 months. Clinical examination and radiological investigations suggested the diagnosis of an external auditory canal cholesteatoma. The patient underwent modified radical mastoidectomy with type 1 tymp…

Robotic Ear Surgery.

Robots under research and development for otology can be classified as collaborative (intervention is constrained by the robot but surgeon directly actuates the end-effector), teleoperated (surgeon remotely controls a tool with modification [eg, tremor reduction] by the robot), or autonomous (surgeon monitors the robot performing a task). Current clinical trials focus on more accurate stapes surgery, minimally invasive access to the cochlea, and less traumatic insertion of cochlear implant electrode arrays. Autonomous approaches to major aspects of surgical interventions (eg, mastoidectomy) wi…

Sonographic diagnosis of acute mastoiditis and subsequent retroauricular abscess in a pediatric cochlear implant recipient: A case report.

When acute mastoiditis occurs in cochlear implant recipients, it can progress to subsequent retroauricular abscess due to the absence of the external mastoid cortex resulting from mastoidectomy performed for cochlear implantation. The management goal is to control infection while preserving the implanted device. A 2-year-old boy with cochlear implants developed acute mastoiditis and a subsequent retroauricular abscess. The patient underwent a surgical intervention based on the diagnosis made utilizing gray-scale and power Doppler sonography. This case illustrates the diagnostic usefulness of s…

Endoscopic Facial Nerve Surgery.

Tympanic facial nerve segment surgery has been traditionally performed using microscopic approaches, but currently, exclusive endoscopic approaches have been performed for traumatic, neoplastic, or inflammatory diseases, specially located at the geniculate ganglion, greater petrosal nerve, and second tract of the facial nerve, until the second genu. The tympanic segment of the facial nerve can be reached and visualized using an exclusive transcanal endoscopic approach, even in poorly accessible regions such as the second genu and geniculate ganglion, avoiding mastoidectomy, bony demolition, an…

Arterial brain infarction in complicated acute otitis media.

We report a rare case of acute otitis media (AOM) complicated by arterial brain infarction, meningitis and orbital cellulitis. Computed tomography (CT) initially showed suspected epidural abscess, coalescent mastoiditis, and orbital cellulitis. Further clinical deterioration occurred following mastoidectomy and evacuation of the abscess. Magnetic resonance imaging (MRI) revealed subacute brain infarction and an extra-axial brainstem abscess. Revision of the initial CT revealed subtle signs suggestive of arterial brain infarction. Although CT with contrast is the standard of care for suspected …

Huge cholesterol granuloma of the middle ear extending to middle cranial fossa.

Cholesterol granuloma (CG) may erode into the middle ear, the mastoid bone and the petrous apex. However, aggressive erosion into the crania … d the middle cranial fossa without invasion into the brain. The CG was removed by extended cortical mastoidectomy. The postoperative course was uneventful and there were no neurological complications.

[Jugular bulb diverticulum mimicking Menière's disease. Surgical treatment].

The position of the jugular bulb is extremely variable. A high jugular fossa with a diverticulum of the jugular bulb can alter the inner ear function with neurosensory hearing loss, vertigo and tinnitus. Six cases of jugular bulb diverticulum with vertigo mimicking Meniere's disease were operated on and followed up from 6 months to 4 years. The jugular bulb was approached through a mastoidectomy and the diverticulum was decompressed downward using bone wax. The vertigo disappeared after surgery in all the cases. These observations suggest that an abnormality of the jugular bulb should be consi…

Clarifying the Diagnosis and Management of Acute Uncomplicated Pediatric Mastoiditis.

INTRODUCTION: Acute pediatric mastoiditis is a bacterial infection of the mastoid bone most commonly associated with acute otitis media. Com … ts, categorized with the proposed definition, were treated more consistently: no patients underwent mastoidectomy and they were less likely to receive a long term course of antibiotics. On multivariate regression analysis, only categorization with the proposed definition of uncomplicated mastoiditis was independently associated with less long-term antibiotic th …

Mastoid cavity dimensions and shape: method of measurement and virtual fitting of implantable devices.

Temporal bone implants can be used to electrically stimulate the auditory nerve, to amplify sound, to deliver drugs to the inner ear and potentially for other future applications. The implants require storage space and access to the middle or inner ears. The most acceptable space is the cavity created by a canal wall up mastoidectomy. Detailed knowledge of the available space for implantation and pathways to access the middle and inner ears is necessary for the design of implants and successful implantation. Based on temporal bone CT scans a method for three-dimensional reconstruction of a vir…

["Fuldaer ventilation surgery"--a surgical concept in severe ventilation disorders of the middle ear].

BACKGROUND: This report presents the long-term results of a special surgical technique in cases of persistent severe dysfunction of the Eust … ventilation of the middle ear and protect the eardrum against partial vacuum. The concept combines mastoidectomy, posterior tympanotomy, and removal of the incus and the head of the malleus with an interposition of the incus. The tympanic membrane is then stabilized with a cartilage-perichondrium graft, and a tube is placed leading from the tympanic cavity to …

New window for cochlear implant insertion.

Nine patients with profound bilateral hearing loss received a cochlear implant via the middle fossa. Subjects presented with a bilateral radical mastoidectomy cavity, fibroadhesive otitis media, otosclerosis, autoimmune inner ear disease, previous cranial trauma and genetic prelingual deafness. A classic middle fossa approach was adopted. A small cochleostomy was performed on the most superficial part of the basal turn. A Nucleus CI24M cochlear implant system (Cochlear Corporation) was inserted in four patients, a Lauraflex implant (Philips Hearing Implants) in three patients and a Combi 40 + …

Ossicular fusion and cholesteatoma in auriculo-condylar syndrome: in vivo evidence of arrest of embryogenesis.

Auriculo-condylar syndrome (ACS) is a rare condition affecting first branchial arch structures. The types of hearing loss and temporal bone findings in ACS have not been reported. We describe a 14-year-old male with constricted pinnae, mandibular dysostosis, glossoptosis, a high-arched palate, hearing loss, and cholesteatoma. Computed tomography imaging demonstrated malleoincudal joint ankylosis. The fused malleoincudal complex was removed during mastoidectomy for cholesteatoma. Electron microscopy and histopathology of the joint suggested the fusion was congenital. This is the first report of…

The transcochlear approach is described for resection of lesions arising anterior or medial to the internal auditory canal as well as for those arising directly from the clivus.
