Cocaine and opiates use in pregnancy: detection of drugs in neonatal meconium and urine.

In this study, the case of a newborn with symptoms of hyperexcitability was analyzed. After it was confirmed in the hospital that the mother had consumed drugs during pregnancy using an enzyme multiplied immunoassay technique, samples of the newborn's urine and meconium were sent to our laboratory to observe the evolution in the distribution of cocaine and opiates during the days following birth. For urine analysis, screening was done with an immunoassay technique, and the confirmation was done by gas chromatography-mass spectrometry (GC-MS) according to a published method. A GC-MS method for …

Effects of dexamethasone on cardiovascular functions in acute phase in meconium-injured rabbits.

BACKGROUND: Because cardiovascular functions and their control mechanisms may be influenced by meconium aspiration and by treatment, cardiovascular parameters including heart rate variability (HRV) were evaluated after meconium instillation and treatment with dexamethasone. METHODS: Adult rabbits were anesthetized, paralyzed, and artificially ventilated. Meconium suspension (25 mg/mL, 4 mL/kg) was instilled intratracheally to induce respiratory failure. Thirty minutes later, animals received i.v. dexamethasone (0.5 mg/kg; dexa group, n = 6) or were left without treatment (control group, n = 5…

What (not) to do at and after delivery? Prevention and management of meconium aspiration syndrome.

Meconium aspiration syndrome (MAS) is a life-threatening disorder in newborn infants. Universal intrapartum suction of infants with meconium stained amniotic fluid (MSAF) and postnatal suction of vigorous infants have been used in an attempt to decrease the incidence and severity of the disease by clearing the airway. Both procedures have been proven fruitless when challenged through randomised control trials (RCTs). Endotracheal intubation and suctioning are currently recommended only for non-vigorous infants. Respiratory failure in infants with MAS is frequently treated initially with conven…

The pros and cons of suctioning at the perineum (intrapartum) and post-delivery with and without meconium.

Routine oronasopharyngeal suctioning (ONPS) of the infant at delivery is a common practice in the delivery room. ONPS is performed to remove lung fluid, meconium, or other secretions from the airway, thereby improving oxygenation and/or preventing aspiration. However, there are controversies regarding this practice, as it seems to be associated with complications. In the presence of clear amniotic fluid, routine ONPS in infants born vaginally and by cesarean section is associated with bradycardia, apnea, and delays in achieving normal oxygen saturations, with no benefit. Intrapartum ONPS and p…

[IgE level of cord blood in neonates with meconium-stained amniotic fluid].

OBJECTIVE: To study the effects of meconium-stained amniotic fluid on the cord blood IgE level in neonates. METHODS: A total of 404 neonates with meconium-stained amniotic fluid who were born by cesarean delivery between August 2003 and August 2005 (meconium-stained group) and 256 neonates with normal amniotic fluid delivered by cesarean (control group) were enrolled in this study. The meconium-stained group consisted of 80 cases of mild, 62 cases of moderate and 262 cases of severe meconium-stained amniotic fluid. The cord blood IgE level was measured using ELISA. RESULTS: The cord blood Ig…

The epidemiology of meconium aspiration syndrome: incidence, risk factors, therapies, and outcome.

OBJECTIVE: We sought to examine, in a large cohort of infants within a definable population of live births, the incidence, risk factors, treatments, complications, and outcomes of meconium aspiration syndrome (MAS). DESIGN: Data were gathered on all of the infants in Australia and New Zealand who were intubated and mechanically ventilated with a primary diagnosis of MAS (MASINT) between 1995 and 2002, inclusive. Information on all of the live births during the same time period was obtained from perinatal data registries. RESULTS: MASINT occurred in 1061 of 2,490,862 live births (0.43 of 1000…

Mechanisms of meconium passage: cholinergic stimulation of electromechanical coordination in the fetal colon.

OBJECTIVE: Fetal gastrointestinal function develops in utero, with evidence of enhanced motility near-term. Although colonic passage of meconium in utero may be associated with fetal maturation or stress, little is known of the mechanisms potentiating motility. We assessed the effect of bethanechol, a cholinergic prokinetic agent, on colonic muscle muscular contractile and electromyogram (EMG) activity in the near-term ovine fetus. METHODS: Near-term (130 days, n = 8) singleton ovine fetuses were chronically prepared with vascular catheters and three sets of miniature strain gauges and bipola…

Induction of early meconium evacuation: is it effective in reducing the level of neonatal hyperbilirubinemia?

This prospective randomized trial evaluated the effect of induction of early meconium evacuation on neonatal jaundice in healthy term neonates. The study group was given glycerin suppository immediately after birth and every 4 hours thereafter, until evacuation of first stool. Glycerin suppository caused faster meconium evacuation but had no effect on mean bilirubin levels at 48 hours. However, males had significantly lower mean bilirubin levels, especially if they also had type A blood group. Glycerin suppository cannot be routinely recommended as a means for reducing the severity of neonatal…

Airway lavage with exogenous surfactant in an animal model of meconium aspiration syndrome.

BACKGROUND: Meconium aspiration syndrome (MAS) is a major cause of respiratory morbidity and mortality in term infants, and occasionally causes serious respiratory disturbance. Viscous meconium debris in the trachea interferes with ventilation, and chest physiotherapy (CPT) is effective for removing secretions from the trachea. The effects of conventional exogenous diluted surfactant lavage combined with CPT were evaluated in a MAS animal model in a randomized controlled study. METHODS: Twenty-three MAS model adult Japanese rabbits were randomized into three groups and artificially ventilated…

Asphyxia aggravates systemic hypotension but not pulmonary hypertension in piglets with meconium aspiration.

Meconium aspiration and birth asphyxia are both separately connected to significant pulmonary and systemic hemodynamic changes in newborns, but, although these insults frequently coexist, their combined effects on the neonatal circulation are still controversial. To determine the pulmonary and systemic circulatory changes induced by pulmonary meconium contamination with concurrent asphyxia, 41 anesthetized and ventilated newborn piglets (10-12 d) were studied for 6 h. Eleven piglets were instilled with a bolus of human meconium intratracheally, and 10 piglets had meconium instillation with imm…

Effect of meconium on the rate of in vitro subtype conversion of swine pulmonary surfactant.

UNLABELLED: The interactions between pulmonary surfactant and meconium have been previously reported, however, no prior study has paid attention to the effect of meconium on the extracellular surfactant metabolism. We studied the effect of meconium on the rate of surfactant subtype conversion from surface-active large surfactant aggregates to inactive small surfactant aggregates using the in vitro surface area cycling method. Human meconium was added to a suspension of large surfactant aggregates isolated from swine lung lavage fluid and the mixture was cycled in a capped polystyrene tube at 3…

Meconium aspiration produces airway hyperresponsiveness and eosinophilic inflammation in a murine model.

Meconium aspiration syndrome is a cause of significant morbidity and mortality in the perinatal period and has been implicated in the pathogenesis of airway dysfunction. In this study, we developed a murine model to evaluate the effects of meconium aspiration on airway physiology and lung cellular responses. Under light anesthesia, BALB/c mice received a single intratracheal instillation of meconium or physiological saline. Respiratory mechanics were measured in unrestrained animals and expressed as percent increase in enhanced pause to increasing concentrations of methacholine (MCh). Furtherm…

Surfactant tracheobronchial lavage for the management of a rabbit model of meconium aspiration syndrome.

We evaluated the effect of tracheobronchial lavage with diluted surfactant solution (bovine lipid extract surfactant, bLES) in a rabbit model of meconium aspiration. All animals were anaesthetized, tracheotomized and given 3-4 ml/kg of 25% slurry of human meconium into the endotracheal tube and mechanically ventilated for 1 h. The animals were then randomly assigned to surfactant lavage (n = 12) with 15 ml/kg of diluted surfactant at a concentration of 5.4 mg phospholipid/ml administered in aliquots of 2 ml; or simple endotracheal suction (control n = 12) when the oxygenation index (OI) was >/…

Nonoperative treatment of simple meconium ileus: a survey of the Society for Pediatric Radiology.

To determine the practice pattern regarding nonoperative treatment of simple meconium ileus, a survey was sent to directors of 66 pediatric radiology departments in the USA and Canada. Thirty-nine responses were received reporting 1,236 patients. A wide variation of enema techniques and contrast media were used. While the success rate did not correlate with osmolality, mode of administration, catheter size, or perforation rate, there was a significantly higher overall success rate with the use of Gastrografin versus non-Gastrografin (p < 0.00076) and the use of additives such as Tween-80 and M…

Meconium aspiration syndrome: physiological and inflammatory changes in a newborn piglet model.

In order to evaluate further the physiological and inflammatory changes of meconium aspiration syndrome (MAS), 25 newborn piglets (1-2 days old, 1.5 +/- 0.4 kg) were studied. Piglets were briefly ventilated with 100% oxygen and then received an intratracheal bolus of 3 mL/kg of a 20% suspension of human meconium. They were then further ventilated, keeping PaCO2 at approximately 40 torr and PaCO2 at 70 torr during 4, 12, 24, and 48 h studies. Pulmonary function studies and tracheal aspirates were obtained at time zero and serially throughout the study. Bronchoalveolar lavage was performed at th…

Meconium peritonitis and pleuritis: a clue to perforation of an incarcerated Bochdalek hernia in a neonate.

The case of a neonate presenting with respiratory distress who had an incarcerated Bochdalek hernia with perforation and meconium peritonitis and pleuritis is reported. The patient had an associated jejunal atresia in an incarcerated segment of intestine, which undoubtedly contributed to the obstruction, strangulation, and perforation in the incarcerated hernia. Gastrointestinal complications attributable to the hernia are most unusual in the neonate, and aggressive preoperative resuscitation and stabilization with the earliest possible surgical intervention are imperative features in manageme…
