Automatic cerebral microbleeds detection from MR images via multi-channel and multi-scale CNNs.

BACKGROUND: Computer-aided detection (CAD) systems have been widely used to assist medical professionals in interpreting medical images, aiding in the detection of potential diseases. Despite their usefulness, CAD systems cannot yet fully replace doctors in diagnosing many conditions due to limitations in current algorithms. Cerebral microbleeds (CMBs) are a critical area of concern for neurological health, and accurate detection of CMBs is essential for understanding their impact on brain function. This study aims to improve CMB detection by enhancing existing machine learning algorithms. ME

The ALPL gene variant project: results of the first 100 reclassified variants.

Hypophosphatasia (HPP) is an inherited disorder that affects bone and tooth mineralization, among other body systems. HPP is caused by pathogenic variants in the alkaline phosphatase-liver (ALPL) gene, which encodes tissue nonspecific alkaline phosphatase. One major challenge in diagnosing HPP is interpreting variant of uncertain significance (VUS), which can create uncertainty for patients and healthcare professionals, leading to delays in diagnosis and treatment or incorrect diagnoses. Since February 2021, the ALPLgene variant consortium has reclassified 100 VUS using adjusted American Colle

Measuring reader fatigue in the interpretation of screening digital breast tomosynthesis (DBT).

OBJECTIVES: The interpretation of digital breast tomosynthesis (DBT) screening examinations is a complex task for an already overstretched workforce which has the potential to increase pressure on readers leading to fatigue and patient safety issues. Studies in non-medical and medical settings have suggested that changes in blink characteristics can reflect fatigue. The purpose of this study is to investigate the use of blink characteristics as an objective marker of fatigue in readers interpreting DBT breast screening examinations. METHODS: Twenty-six DBT readers involved in the UK PROSPECTS

Questionnaire assessment of usual practice in mood and cognitive assessment in Scottish stroke units.

PURPOSE: National and International guidelines recommend cognition and mood assessment for all stroke survivors. However, there is no consensus on preferred screening tool or method of assessment. We aimed to describe clinical practice in cognitive and mood assessment across Scottish stroke services. METHOD: We used a questionnaire based survey. After local piloting, we distributed the questionnaire using mixed methodologies (online and paper) across all Stroke Managed Clinical Networks in Scotland. We also distributed the questionnaire to specialist societies representing stroke physicians,

Potential risk of using General Estimates System: bicycle safety.

Beneficial effects of bicycle helmet use have been reported mostly based on medical or survey data collected from hospitals. This study was to examine the validity of the United States General Estimates System (GES) database familiar to many transportation professionals for a beneficial effect of helmet use in reducing the severity of injury to bicyclists and found potential risk of erroneous conclusions that can be drawn by a narrowly focused study when the GES database is used. Although the focus of the study was on bicycle helmet use, its findings regarding potential risk might be true for

Essentiality of the lateral cervical spine radiograph.

Proper interpretation of the lateral cervical spine (C-spine) film is a prime emergency medicine responsibility. Patients with potential neck injury must be identified rapidly and handled carefully; their C-spine films must be interpreted quickly and accurately. We stress the urgency in obtaining a good, readable lateral C-spine film and present a systemic "ABCs" approach to reading this film. This approach includes specific, sequential attention to Alignment, Bony integrity, Cartilaginous structures, and the surrounding Soft-tissue spaces. In the cases of 50 consecutive patients with C-spine

Antibiotics and return visits for respiratory illness: a comparison of pooled versus hierarchical statistical methods.

BACKGROUND: Antibiotic prescribing for respiratory illness has been associated with small reductions in return visits in an analysis of a large practice-based network. In this study, we apply hierarchical analytical methods that account for the clustering of patients by practices to identify whether antibiotic prescribing by primary care physicians reduces subsequent visits for 6 acute respiratory illnesses-upper respiratory infection, pharyngitis, bronchitis, otitis media, sinusitis, and cough. METHODS: The study data came from 318 family physicians and internists in 45 practices in the Prac

Qualitative evaluation of survey questions to assess treatment preference for daily oral or long‑acting injectable antiretroviral therapy among people living with HIV.

Treatment of HIV has historically required taking daily oral antiretroviral therapy (ART). A recent alternative to daily oral ART is long-acting injectable ART with cabotegravir plus rilpivirine, administered monthly or every 2 months. The purpose of this qualitative study was to evaluate the concept relevance and interpretability of five previously developed questions: one treatment preference question and four questions designed to assess how the emotional burden associated with HIV treatment impacts treatment preferences. Thirty adults in the United States currently receiving HIV treatment

Variation in paediatric 24-h ambulatory blood pressure monitoring interpretation by Canadian and UK physicians.

Twenty-four-hour ambulatory blood pressure monitoring (ABPM) is widely accepted as a more accurate method for measurement of blood pressure (BP) compared to a single office-based measurement of BP. However, it is unclear how physicians interpret ABPM and make management decisions. This study's goal is to investigate variation in ABPM interpretation among paediatric nephrologists (Canada and UK) and paediatric cardiologists (Canada only) via an online survey. The survey content included baseline demographics, questions on the use and indications for ABPM, interpretation of results, and subseque

Endoscopy for Altered Anatomy in Inflammatory Bowel Disease.

A significant proportion of patients with Crohn's disease or ulcerative colitis will ultimately necessitate surgical interventions, despite advances in diagnosis, medical therapy, and endoscopic interventions. These surgeries lead to substantial alterations in bowel anatomy. The present review aims to emphasize the key role of conducting a comprehensive endoscopic evaluation of both anatomic landmarks and endoscopic features for accurate clinical interpretation, thereby impacting decisions regarding medical or endoscopic interventions.

Interpretable CNN-Multilevel Attention Transformer for Rapid Recognition of Pneumonia From Chest X-Ray Images.

Chest imaging plays an essential role in diagnosing and predicting patients with COVID-19 with evidence of worsening respiratory status. Many deep learning-based approaches for pneumonia recognition have been developed to enable computer-aided diagnosis. However, the long training and inference time makes them inflexible, and the lack of interpretability reduces their credibility in clinical medical practice. This paper aims to develop a pneumonia recognition framework with interpretability, which can understand the complex relationship between lung features and related diseases in chest X-ray

Perspectives on Hypertension in the New England Cape Verdean Community.

This study characterizes patient understanding of hypertension and its management in a Cape Verdean immigrant community. We conducted 20 semi-structured interviews with Cape Verdean Creole-speaking adults about their beliefs and behaviors regarding hypertension. Fourteen women and six men ages 35-87 were interviewed. The majority of the participants believed hypertension is symptomatic, with headache being the most common symptom. Many reported taking anti-hypertensive medication, though some took medication only when they felt symptoms. The majority were unfamiliar with a normal blood pressur
