Why Is Caries Prevention in Children So Difficult? A Narrative Opinion.

Caries is among the most common non-communicable diseases worldwide, yet it is commonly described as preventable. Caries prevention is, however, difficult and complex, since the disease has strong social, parental, behavioral, political, medical/genetic, and psychological elements, and the payment models are targeted at traditional conservative care. The aim of this paper is to discuss some key issues that make caries prevention in children be perceived as "difficult": i) the communication gap between researchers and clinicians, creating unrealistic expectations of intervention efficacy; ii) t

Midwife-Led Mobile Antenatal Clinic: An Innovative Approach to Improve Utilization of Services in Pwani, Tanzania.

Participating in antenatal clinics is a major determinant in reducing poor maternal and neonatal birth outcomes. We aimed to evaluate the utilization of antenatal clinic (ANC) services provided by a mobile clinic led by skilled midwives and determine the acceptability in the Pwani region, Tanzania. For a year, the mobile clinic, nicknamed "Mkunga Kitaani" and equipped with necessary tools and staff, served seven villages in the Kisarawe district that lacked health facilities. The research was conducted using a descriptive study design, incorporating both qualitative and quantitative methods. Q

Scale up of anaesthesia services in underserved rural Tanzania.

BACKGROUND: Because of critical shortage of physician anaesthesiologists, the government of Tanzania adopted a task shifting strategy for provision of anaesthesia services. This paper describes the results of an operational study designed to increase the number of anaesthesia providers for emergency obstetric surgeries in order to reduce maternal and perinatal mortality in underserved rural Tanzania. METHODS: In 2016 a before-after cohort study was conducted in seven health centres in rural Tanzania. Five health centres received an intervention and two were selected to track secular trends (c

Accessibility to medical institutions in Aichi Prefecture: A geographic information systems-based analysis.

We investigated the geographic accessibility of medical institutions in Aichi Prefecture, Japan, using Geographic Information Systems (GIS). Three facility types-pharmacies, hospitals, and clinics-were analyzed across 11 secondary healthcare regions. Accessibility was evaluated based on spatial proximity by calculating population and elderly population coverage rates, defined as the proportion of residents located within the vicinity of medical institutions. Catchment areas were defined by travel mode: 400-m buffers represented a 5-min walk, and 2-km buffers approximated a 5-min drive. We test

Principles of trauma-informed care evident in person centered care for persons living with dementia in low resource settings.

OBJECTIVES: This study identified how long-term care stakeholders describe trauma-informed care (TIC) practices in low resource settings. METHOD: This was a secondary data analysis about person-centered care (PCC) for persons living with dementia (PLWD). The parent study purposively recruited administrators, direct care staff, PLWD, and care partners in two nursing homes and two assisted living communities in urban and rural federally designated medically underserved areas. In depth semi-structured interviews were conducted about PCC; the parent study did not include TIC-specific probes. Resp…

Maintaining paediatric cardiac services during the COVID-19 pandemic in a developing country in sub-Saharan Africa: guidelines for a "scale up" in the face of a global "scale down".

The COVID-19 pandemic is currently ravaging the globe and the African continent is not left out. While the direct effects of the pandemic in regard to morbidity and mortality appear to be more significant in the developed world, the indirect harmful effects on already insufficient healthcare infrastructure on the African continent would in the long term be more detrimental to the populace. Women and children form a significant vulnerable population in underserved areas such as the sub-Saharan region, and expectedly will experience the disadvantages of limited healthcare coverage which is a maj

Integrating LGBTQIA + Community Member Perspectives into Medical Education.

ProblemLGBTQIA+ (lesbian, gay, bisexual, transgender, queer, intersex, asexual, and all sexual and gender minorities) people have unique health care needs related to their sexual orientation, gender identity and expression, and sex development. However, medical education has historically excluded LGBTQIA + health-related content in formal curricula. It is common for medical students to interact with diverse patient populations through clinical rotations; however, access to and knowledge about LGBTQIA + patients is inconsistently prioritized in medical schools. This is especially true for LGBTQ

Indicators of substance abuse treatment demand in Cape Town, South Africa (1997-2001).

Few studies have investigated the demand for substance abuse treatment in South Africa. This article uses data collected from specialist substance abuse treatment centres to describe substance abuse treatment demand and patterns of service utilisation in Cape Town for the period January 1997 to December 2001. Findings suggest that although treatment demand for alcohol-related problems remains high, treatment demand for substances other than alcohol has increased over time. Patterns of treatment service utilisation suggest that women and black South Africans remain underserved. The need for com

A Vision-Language-Guided Multimodal Fusion Network for Glottic Carcinoma Early Diagnosis: Model Development and Validation Study.

BACKGROUND: Early diagnosis and intervention in glottic carcinoma (GC) can significantly improve long-term prognosis. However, the accurate diagnosis of early GC is challenging due to its morphological similarity to vocal cord dysplasia, with the difficulty further exacerbated in medically underserved areas. OBJECTIVE: This study aims to address the limitations of existing technologies by designing a vision-language multimodal model, providing a more efficient and accurate early diagnostic method for GC. METHODS: The data used in this study were sourced from the information system of the Fir…

Interventions on informal healthcare providers to improve the delivery of healthcare services in low-and middle-income countries: a systematic review.

OBJECTIVE: Informal healthcare providers (IHCPs) play a big role in health systems in low-and middle-income countries (LMICs) and are often the first point of contact for healthcare in rural and underserved areas where formal healthcare infrastructure is insufficient or absent. This study was performed to systematically review the literature on interventions targeting IHCPs in improving the delivery of healthcare services in LMICs. METHODS: PubMed, Embase, and Cochrane CENTRAL databases were searched for studies that assessed any type of intervention among IHCPs to improve the delivery of hea

Understanding Patient Experience With Hyperhidrosis: A National Survey of 1,985 Patients.

Hyperhidrosis affects 4.8% of the US population, and despite the well documented negative impact of hyperhidrosis on patients' lives, data are generally lacking on the patient experience with this condition. The International Hyperhidrosis Society (IHHS) conducted a study in 2014, and initial results confirmed the multifaceted impact of hyperhidrosis on quality of life and underscored the need for disease awareness and effective management. To provide further insight into the hyperhidrosis patient experience, additional results focusing on family history, physician interaction, impact on daily

Role of the Internet in Solving the Last Mile Problem in Medicine.

Internet-augmented medicine has a strong role to play in ensuring that all populations benefit equally from discoveries in the medical sciences. Yet, data from the Centers for Disease Control and Prevention collected from 1999 to 2014 suggested that during the first phase of internet diffusion, progress against mortality has stalled, and in some cases, receded in rural areas that are traditionally underserved by medical and broadband resources. This problem of failing to extend the benefits of extant medical knowledge equitably to all populations regardless of geography can be framed as the "l

Intelligent Diagnosis and Classification of Keratitis.

A corneal ulcer is an open sore that forms on the cornea; it is usually caused by an infection or injury and can result in ocular morbidity. Early detection and discrimination between different ulcer diseases reduces the chances of visual disability. Traditional clinical methods that use slit-lamp images can be tiresome, expensive, and time-consuming. Instead, this paper proposes a deep learning approach to diagnose corneal ulcers, enabling better, improved treatment. This paper suggests two modes to classify corneal images using manual and automatic deep learning feature extraction. Different

Mapping 340B Funds Flow Through Contract Pharmacies.

The 340B Drug Pricing Program, established in 1992, was designed to help safety-net healthcare providers stretch scarce resources by allowing them to purchase outpatient medications at significant discounts. However, the program's dramatic expansion, particularly following the Affordable Care Act, and the rise of contract pharmacy arrangements have led to increasing scrutiny regarding its current function and impact. This study examines the flow of funds within the modern 340B ecosystem, with a particular focus on contract pharmacies, pharmacy benefit managers (PBMs), third-party administrator

Inspiring the next generation: a qualitative study of rural healthcare professionals' experiences as "books" in a living library.

BACKGROUND: Rural medical education requires learning approaches that connect learners with the lived realities of rural practice. One structured approach is the Living Library model, using narratives of lived experiences of rural healthcare professionals as knowledge resource. The benefits on learner outcomes are well-established, such as increased empathy and improved understanding of social and professional contexts of health, but little is known on the impact of telling their stories on the storytellers. This study explored what it means for rural healthcare professionals to serve as "huma
