Ameliorative Effect of Beetroot Against Folic Acid Deficiency: A Review.

A lack of vegetables or other folate-rich foods in the diet leads to folate insufficiency. The most significant cause of this malformation may be a lack of folic acid. Folic acid deficiency, a critical maternal health component, can result in megaloblastic anemia, neural tube anomalies, cardiovascular complications, cancer, and Deoxyribonucleic acid (DNA) imbalance. Beetroot (Beta vulgaris) is one of the most widely consumed vegetables worldwide. Red beets have attracted more attention because of their rich source of bioactive substances, including betalain, indolent nitrates, polyphenols, and

Physiology of vitamin B12: a study on its molecular mechanisms using a Caenorhabditis elegans model.

Vitamin B12 (B12) is a water-soluble substance that is a member of the B-vitamin family. Its recommended daily dose in adult men and women is 2.4 µg, which is the lowest among the 13 vitamins. B12 deficiency causes megaloblastic anemia and neurological disorders. It is also associated with cognitive decline, growth retardation, infertility, and other symptoms. Nevertheless, the detailed mechanisms of which remain unclear. Caenorhabditis elegans is a small organism, with a length of approximately 1 mm and a lifespan of approximately 3 weeks. It has similar fundamental biological structures, suc

Progressive Decrease in Function and Ambulation Potential: A Case of Subacute Combined Degeneration.

Subacute combined degeneration (SCD) is a reversible cause of posterior and lateral spinal cord degeneration. Prolonged vitamin B12 deficiency is a common cause of SCD as it leads to inhibition of proper myelin synthesis and reduces myelin integrity. When left untreated, SCD causes progressive debility that can lead to irreversible damage. We describe the case of a 49-year-old male patient who presented with one year of worsening weakness, back pain, paresthesias, and gait abnormalities. Laboratory values revealed vitamin B12 deficiency, elevated homocysteine and methylmalonic acid, and megalo

Vitamin B12 deficiency in an infant with neurological and hematological findings: A case report.

KEY CLINICAL MESSAGE: It is important for pregnant and breastfeeding women who adhere to a strict vegetarian diet to take appropriate steps to avoid vitamin B12 deficiency in their infants. ABSTRACT: Vitamin B12 deficiency is rare during infancy. The initial symptoms of this deficiency are subtle and may include irritability, failure to thrive with a decline in growth rate, apathy, anorexia, refusal of solid foods, megaloblastic anemia, and developmental regression. The case presented here involves an 8-month-old male infant who showed neurological symptoms such as decreased activity, increas

Disturbed homocysteine metabolism is associated with cancer.

Hyperhomocysteinemia/Homocysteinuria is characterized by an increased level of toxic homocysteine in the plasma. The plasma concentration of homocysteine is 5-15 μmol/L in healthy individuals, while in hyperhomocysteinemic patients, it can be as high as 500 μmol/L. While increased homocysteine levels can cause symptoms such as osteoporosis and eye lens dislocation, high homocysteine levels are most closely associated with cardiovascular complications. Recent advances have shown that increased plasma Hcy is also a fundamental cause of neurodegenerative diseases (including Alzheimer's disease, P

Vitamin B12 Deficiency Presenting with Neurological Dysfunction in an Adolescent.

BACKGROUND: Vitamin B12 deficiency is classically encountered in the adult Caucasian population and manifests as a subacute combined degeneration in the presence or absence of macrocytic anemia. However, B12 deficiency is extremely rare in children in developed countries, and pernicious anemia is even rarer etiology of this deficiency. The clinical presentation of B12 deficiency in children is not as easily recognizable or well-characterized and may result in missed or delayed diagnosis. PATIENT DESCRIPTION: We describe a previously healthy adolescent girl who presented with emotional labilit

Gastric adenocarcinoma in the context of X-linked agammaglobulinemia: case report and review of the literature.

The hallmarks of X-linked Agammaglobulinemia (XLA) are panhypogammaglobulinemia, absent B-cells, and recurrent sinopulmonary and gastrointestinal infections starting at an early age, as well as other infections like cellulitis, meningitis, arthritis and sepsis. A number of non-infectious complications have been reported in these patients, including autoimmune diseases and malignancy, especially lymphomas. Here, we report the case of a 30-year old man who developed gastric adenocarcinoma in the context of XLA. Previous reports of, and hypotheses addressing the development of cancer in patients

An unusual etiology of erythropoietin resistance: hyperthyroidism.

Many possible causes of resistance to human recombinant erythropoietin (rh-EPO) have been reported in patients with renal failure. This case presents an unusual cause of erythropoietin-resistant anemia in a patient with chronic renal failure. A 61-year-old male patient who was on chronic hemodialysis program due to diabetic nephropathy for seven months developed erythropoietin resistant anemia. No iron deficiency was revealed by laboratory data, no megaloblastic anemia were found by biochemical investigation, and no inflammatory states including infection or neoplastic diseases were disclosed

[Management, prevention and control of pernicious anemia].

Pernicious anemia is the most frequent cause of megaloblastic anemia in our area, and it is the result of a vitamin B12 deficiency due, itself, to the decrease or absence of intrinsic factor (IF) because of gastric mucosa atrophy or autoimmune destruction of IF-producing parietal cells. With the existence of a severe gastric atrophy, there is a decrease in acid and IF production and a further change in vitamin B12 absorption. Fifty percent of the cases are associated to anti-IF antibodies, which presence in other autoimmune diseases is exceptional. In patients with pernicious anemia, measureme

Identification of a 4-base deletion in the gene in inherited intrinsic factor deficiency.

A 4-base deletion has been identified in the coding region of the gene for gastric intrinsic factor (IF) in an 11-year-old girl with severe anemia and cobalamin (Cbl) deficiency. The bone marrow showed frank megaloblastic morphology, and the Schilling test indicated a failure to absorb Cbl that was corrected by coadministration of IF. Pentagastrin administration induced acid secretion, but the gastric juice lacked IF as determined by CbI binding, by fractionation of protein-bound CbI, and by immunoprecipitation with anti-IF antiserum. Individual exons were amplified by the polymerase chain rea

[Dementia caused by bacterial overgrowth in a patient with Billroth II gastrectomy].

We report the case of a patient in the Psychiatric Department who complained of progressive impairment of cerebral functions consistent with dementia, diarrhea and fecal incontinence in the last few months. His medical history included a Billroth II gastrectomy for gastric ulcer. Biochemical tests detected cobalamin deficiency, without megaloblastic anemia, and an abnormal Schilling test that was not due to intrinsic factor deficiency. Once other causes of cobalamin deficiency were ruled out, we considered it as a deficiency disease due to blind loop syndrome. Treatment with parenteral vitamin

Neural tube defects in Jamaica following Hurricane Gilbert.

An increased incidence of obvious live-birth neural tube defects (i.e., spina bifida cystica and encephalocele) occurred in Jamaica 11 to 18 months after Hurricane Gilbert. The conceptions of the affected babies coincided with a rise in megaloblastic change in sickle cell patients, suggesting a wide-spread drop in dietary folate intake. A detailed history was taken from each of the 17 affected mothers (case subjects) and 51 unaffected mothers (matched control subjects). The case subjects reported a significantly lower mean intake of dietary folate in the periconceptional period (154 micrograms

Incidence and clinical significance of protein-bound vitamin B12 malabsorption.

Patient records from January 1975 to December 1984 were analysed to assess the possible incidence of protein-bound vitamin B12 malabsorption. This condition is characterised by a low serum vitamin B12 level and a normal Schilling test but impaired absorption of vitamin B12 bound to protein. We found that 48 (25%) patients with a low serum cobalamin level unexplained by other causes had a normal Schilling test. Megaloblastic haemopoiesis was found in 25 of these. From this group, all 10 patients who had a test of protein-bound vitamin B12 absorption showed impaired absorption. Protein-bound vit

Transcobalamin II deficiency associated with unusual bone marrow findings and chromosomal abnormalities.

A female infant presented at seven weeks of age with failure to thrive, progressively severe pancytopenia, hypogammaglobulinemia and, mucosal ulceration. Bone marrow morphology showed severed megaloblastic changes in the myeloid series with a shift to the left and an increased number of blasts with abnormal morphology. Erythroid precursors and megakaryocytes were markedly decreased. Cytogenetic studies showed marked aneuploidy and increased chromosomal breakage. Treatment with high doses of vitamin B12 resulted in a dramatic clinical response with hematological values becoming normal. The pati

Serum vitamin B12, folate, and iron levels in recurrent aphthous ulceration.

Hematologic deficiencies have previously been described in some cases of recurrent aphthous ulceration (RAU). Ninety patients with RAU and twenty-three healthy control subjects were evaluated by complete blood count, serum iron, and total iron-binding capacity, serum vitamin B12, and serum or red blood cell folate. Only three of ninety RAU patients proved to have abnormalities. There were no statistically significant differences between the patient and control populations, and no one in either group had abnormal serum vitamin B12 or serum or RBC folate assays. None of the patients or controls

Treatment of nutritional anemia in infants.

While iron deficiency is a common cause of anemia in infants, requiring specific treatment by administration of iron, there are other causes which require treatment equally specific. Anemia due to a nutritional deficiency can be improved only by providing the needed nutriment in sufficient quantity. Two case reports are presented. One is of megaloblastic anemia in an infant, which was treated with a variety of vitamin preparations before administration of folic acid produced improvement. The other report is of anemia due to iron deficiency treated successfully with ferrous sulfate in a dosage

Abruptio placentae.

Analysis of blood samples from pregnant women has shown that the mean histamine level starts to rise when the plasma ascorbic acid level falls below 1.0 mg/100 ml; it is doubled when the ascorbate level falls to 0.5 mg/100 ml and quadrupled when it falls below 0.2 mg/100 ml. The incidence of abruptio placentae was found to be seven out of 355 (or 2.0%) in women who had plasma ascorbic acid levels above 0.4 mg/100 ml and six out of 31 (19.4%) in women with plasma ascorbate levels below 0.4 mg/100 ml. This difference is highly significant. It is suggested that ascorbic acid deficiency and histam

Nuclear characteristics of buccal mucosa cells in sickle-cell anemia.

Exfoliative cytology smears from the buccal mucosa of patients with sickle-cell anemia, selected controls, and patients with other diagnosed anemias were studied for nuclear aberrations. A filar micrometer eyepiece was used to count the number of nuclei larger than 14 microns in mean diameter in a 500 cell count on each slide. Use of the Scheffe method of paired comparisons, disclosed that the number of cells larger than 14 microns in mean diameter, was significantly smaller in the control subjects, patients with microcytic anemia, and subjects with sickle-cell anemia than in the patients with
