Pastoral Authority and Spiritual Harm: Evangelii Gaudium and an Ethics of Pastoral Responsibility in Mental Health.

Pastoral care can provide meaning, consolation, and spiritual support to persons experiencing psychological distress, yet religious authority may also become a source of harm when it exceeds pastoral competence or constrains the agency of those seeking care. This conceptual article develops pastorally mediated spiritual harm as a distinct ethical category for injury arising through pastoral authority even when deliberate abuse or coercive intent is absent. Using normative conceptual reconstruction, it reads Francis's Evangelii Gaudium, especially §§169-173, as a structured account of reverence

A Randomized Controlled Trial of an Extension for Community Healthcare Outcomes (ECHO) Tele-mentoring Program to Increase Clozapine Utilization.

BACKGROUND AND HYPOTHESIS: To determine whether a tele-mentoring program increases clozapine utilization by improving prescriber knowledge and perceived competence in clozapine management. STUDY DESIGN: In a cluster-randomized controlled design, we tested the effectiveness of an Extension for Community Healthcare Outcomes (ECHO) model-based intervention, consisting of 26 biweekly didactic and case-based tele-mentoring sessions, vs enhanced treatment as usual (eTAU), in which both conditions received access to a consultation phone line and a point of care device for on-site hematologic monitor

Disabled Students in Health and Social Services Fieldwork: Perceptions of Canadian Fieldwork Educators and Academic Coordinators.

Ensuring equitable access to professional education programs for learners who need accommodations is distinctly challenging when education moves beyond the classroom into clinical or fieldwork sites. Fieldwork educators and university academic coordinators who arrange fieldwork placements work with university accessibility services and students to arrange required accommodations, while preserving confidentiality, maintaining high learning standards, and ensuring attainment of professional competencies. This work is complicated by time pressures and heavy caseloads in fieldwork settings. Here w

Mobility behaviour and driving status of patients with mental disorders - an exploratory study.

BACKGROUND: Driving is an important activity of daily life and an integral part of mobility. However, impact of mental illness on road mobility is widely unexplored. METHOD: Driving status in 1497 psychiatric inpatients (PPs) and a clinical control group of 313 neurological inpatients (NPs) was investigated using a brief questionnaire. RESULTS: 67% of PPs (89% NPs) reported to have a valid driver's licence and 77% of them (92% NPs) reported to regularly use their cars. Within driver's license holders, patients with organic mental disorder (32%), substance dependence (37%) and psychotic disor

The Implementation of Mental Health Policies and Practices in Schools: An Examination of School and State Factors.

BACKGROUND: Poor mental health outcomes persist among adolescent youth. Secondary schools play a critical role in fostering positive mental health by implementing policies and practices grounded in evidence. The factors associated with implementation, however, are unclear. This study examines which school- and state-level factors are associated with improved implementation of mental health policies and practices at secondary schools across the United States. METHODS: US Centers for Disease Control and Prevention data stemming from 3 datasets (School Health Policies and Practices, School Healt

Restraining good practice: Reviewing evidence of the effects of restraint from the perspective of service users and mental health professionals in the United Kingdom (UK).

Safeguarding, balancing the concept of risk with the need for public protection and its implication for the lives of individuals, is an important facet of contemporary mental health care. Integral to safeguarding is the protection of human rights; the right to live free from torture, inhuman, or degrading treatment, and having the right to liberty, security, respect, and privacy. Professionals are required to recognise all of these rights when delivering care to vulnerable people. In the United Kingdom (UK) there has been growing public concern regarding abusive practices in institutions, with

[Physical activity and mental health in childhood and adolescence: a summary of the evidence from three national studies].

Young people are increasingly burdened by global crises. Physical activity (PA) is considered a key factor for mental health; however, for Germany, an integrated synthesis of population-based evidence is still lacking. The aim of this narrative overview is therefore to consolidate current findings on the association between PA and mental health. The analysis draws on three population-based studies conducted as module or follow-up studies in cooperation with the KiGGS study: the MoMo study (motor performance/physical activity), the BELLA and COPSY studies (mental health), and the COMO study (int

Mental health promotion in the upper level of comprehensive school from the viewpoint of school personnel and mental health workers.

Prevalence of mental health problems among adolescents varies from 10 to 30%. Therefore, mental health promotion in school has risen as a very important developing area in public health services. The need is international. Despite the large number of projects and recommendations on the promotion of schoolchildren's mental health, the literature does not offer a comprehensive theoretical description of what mental health work with people of this particular age really is as a whole. The theory can be constructed by combining and comparing the viewpoints of the different parties--the employees, t

Enabling mental health task-sharing: a collective case study of undergraduate clinical associate training programmes in South Africa.

BACKGROUND: There is a shortage of the human resources needed to deliver mental health services which is likely to be exacerbated by COVID-19. Due to mental health workforce shortages, task-shifting and task-sharing approaches have been implemented in a number of countries. Clinical associates, a mid-level cadre working under the supervision of medical practitioners, could play a role in delivering mental health services but it is not clear if they are adequately prepared. This study explored the mental health curriculum content of the undergraduate clinical associate training programmes in So

Using Conversation Analysis to explore assessments of decision-making capacity in a hospital setting.

BACKGROUND: Healthcare professionals (HCPs) have a responsibility to conduct assessments of decision-making capacity that comply with the Mental Capacity Act 2005 (MCA). Current best-practice guidance, such as the Mental Capacity Code of Practice and National Institute for Health and Care Excellence decision-making and mental capacity guidance, does not stipulate how to accomplish this in practice, for example, what questions should be asked, how options and information should be provided. In addition, HCPs struggle to assess the capacity of individuals with communication difficulties. AIMS:

The U-ARE Protocol: A Pragmatic Approach to Decisional Capacity Assessment for Clinical Research.

With increased longevity and growth in the number of older adults comes rising rates of individuals with cognitive impairment and dementia. The expansion of this population has important implications for research on aging and dementia syndromes, namely increased enrollment of older individuals in clinical research. Ethical prerogatives, as well as historical underrepresentation of persons with dementia in research studies due to the perceived burden of traditional decisional capacity evaluations, necessitates the development of pragmatic approaches to ascertain decisional abilities in research

Impersonal trust and professional authority: exploring the dynamics.

AIM: The aim of the study was to explore the relationship between impersonal trust, governmentality and professional activity through an analysis of professional discourse. BACKGROUND: This study emerged from an earlier paper that described a four-dimensional model based on Luhmann's definition of trust as "reducing complexity and managing expectations". Linking trust with the Foucauldian notion of governmentality develops this further. Governmentality raises the question of how professional authority is constituted through the practice of nurses and other health and welfare professionals. M

Medical ethics and the executing process in the United States of America.

The article focuses on the ethical and moral issues raised by the participation of physicians in the execution process in the United States of America. Discussion centres on two main areas. Firstly, participation in the actual execution, particularly where the method is lethal injection; and secondly psychiatric assessment and treatment of inmates who are deemed not competent in law for execution. It is argued that as an execution is a harm, participation runs counter to the ethics of the medical participation and cannot be justified even on the basis of relieving pain. Treatment of incompeten

Development and evaluation of the psychometric properties of the Adolescent Motor Competence Questionnaire (AMCQ) for Adolescents.

BACKGROUND: There are no valid and reliable self-report measures designed to identify level of motor competence and suspected motor difficulties among 12-18year old adolescents. AIM: This paper reports the development and evaluation of a self-report questionnaire (Adolescent Motor Competence Questionnaire; AMCQ) to address this need. METHOD: The project proceeded in 3 phases; (A) item development, (B) content evaluation, and (C) examination of reliability and validity of the final questionnaire. Each phase was informed by criteria A and B in the Diagnostic and Statistical Manual of Mental Di

The Relationship Between Indicators of Depression and Anxiety and Sexual Orientation in Canadian Women.

Previous studies examining the associations between women's sexual orientation and mental health have obtained inconsistent results. Whereas some studies have suggested that status as a lesbian or bisexual woman may be associated with greater mental health risk, others have suggested that bisexual women may be specifically vulnerable to mental health problems. The current study examined two competing models in a non-clinical sample of Canadian women (N = 278). The first model predicted that women who reported bisexual attraction would endorse more indicators of depression and anxiety compared

A European paramedic curriculum for geriatric emergency medicine developed via a modified Delphi technique.

BACKGROUND: Older emergency patients currently account for most European emergency medical service dispatches. Due to demographic changes and increasing comorbidities in advanced age, this number is expected to rise substantially in the coming years. Prehospital professionals require specialised training to provide high-quality care for complex, multimorbid patients. The aim of this study is to define minimum competencies for paramedic education in Europe on the management of emergencies in older adults. METHODS: A modified electronic Delphi study was performed from January 2023 to November 2

Family cohesion and conflict in the context of adult children and parents living with early-stage cognitive impairment.

OBJECTIVES: Adult child caregivers of older adults living with Alzheimer's disease and related dementias are at risk of burnout due to competing caregiving demands from multiple generations. Yet, drawing from structural family theory, this study aims to examine the extent to which family dynamics, especially greater family cohesion and less family conflict, are associated with children's mental and physical health. METHOD: The present study was secondary data analysis of an observational study, in which 150 pairs of adult-child caregivers and parents with early-stage cognitive impairment self
