Lead foreign body arthropathy in a horse.

A diagnosis of degenerative joint disease secondary to an intra-articular metallic foreign body in the right metacarpophalangeal joint was made in a Quarter Horse gelding. Arthroscopy, performed to evaluate the joint and remove the foreign body, revealed yellow discoloration of the articular cartilage and synovium, and blunting and proliferation of the synovium. The foreign body was identified as a lead sphere. Microscopic examination of synovium revealed chronic synovitis, with accumulation of hemosiderin and multifocal, mild mineralization. Another pigment was evident extracellularly in the

[Locking of the metacarpophalangeal joint of articular origin. Review of the literature and case report].

We report a case and review concerning literature of intrinsic locking of the metacarpophalangeal joint. These lockings are usually due commonly to degenerative arthritis of the joint, rarely to an abnormal shape of the head of metacarpal or intraarticular loose bodies. They should be differentiated from the extension lag of proximal interphalangeal joint usually encountered in "trigger finger" due to stenosing tenosynovitis. Conservative treatment must be careful and may fail or be complicated. Surgical treatment is effective, best done via an anatomical approach depending on the pathologic f

[Management of malacia of the os lunatum by implantation of a dorsal metacarpal vascular bundle].

In the treatment of malacia of the lunate bone, beside the prosthesis and procedures aiming the decrease of load, the revascularization of the bone is the third basic therapeutic principle. One possibility of the later, not yet current in this country, is the implantation of the metacarpal vascular bundle in the lunate bone. As far as we know we were the first to perform this operation in Hungary. In this paper our first cases are described together with a detailed description of the operative technique and we would call at the same time to further common examinations.

Hamate-metacarpal fracture dislocation.

A patient is described who punched a brick wall with his clenched fist and sustained a fracture of the body of the hamate with dislocation of the ring and little finger metacarpals. He was treated successfully with surgery. This is a rare injury that is frequently missed on the initial presentation and should be considered in the differential diagnosis of posttraumatic ulnar-sided hand pain.

Metacarpal and phalangeal osteotomy with miniplate fixation.

We reviewed the results of corrective oseotomies performed with power tools for symptomatic malunions of metacarpal and phalangeal fractures in 10 patients. At an average follow-up of 30 months, all patients had a healed osteotomy with correct alignment and had an average increase of 30 degrees in the flexion arc of the involved digit. We recommend a surgical technique of miniplate fixation over K-wire fixation where possible; it allows precise, intraoperative correction and rigid stabilization. A shortened immobilization time and early motion are significant advantages of plate fixation. Howe

Thumb reconstruction by metacarpal lengthening after traumatic loss at the level of the interphalangeal joint.

This paper describes a unique procedure utilized on seven occasions to restore thumb function in amputation at or near the level of the interphalangeal joint. Consisting of two phases, the procedure adds approximately 2.5 cm of functional length to the thumb. The metacarpal bone is lengthened approximately 1.5 cm by a bone-lengthening device, and an additional 1 cm is obtained by deepening the thumb web space.

Stabilization of fractures in the hand and wrist with traumatic soft tissue and bone loss.

Open type III fractures of the hand or wrist with severe bone and soft tissue loss justify aggressive treatment to restore anatomy, assure healing, and maximize functional recovery. The techniques of modern wound excision used at initial surgery predictably result in a decompressed and surgically clean wound within a few days from injury in the vast majority of cases. This allows a safe application of delayed primary internal fixation and bone grafting for fracture restoration or joint arthrodesis as well as early wound closure or coverage. The immediate or early application of stable external

A patient of short stature with idiopathic hypoparathyroidism round face and metacarpal signs.

A 16-year old girl of short stature, with round face, mental retardation, and Albright's dimple sign was admitted for evaluation of hypocalcemia. Her serum calcium levels were 6.3-8.0 mg/dl, and phosphorus 6.9-7.8 mg/dl. Although a diagnosis of pseudohypoparathyroidism was initially suggested, her serum iPTH concentration was low (0.1 ng/ml). Furthermore, an injection of synthetic human parathyroid hormone (100 U, hPTH (1-34] was followed by a marked increase in urinary excretion of cyclic AMP and phosphorus. This case suggests that a shortened metacarpal is not a reliable guide in distinguish

Manipulation and external fixation of metacarpal fractures.

We have developed a method of manipulation and external fixation of metacarpal fractures using Kirschner wires bonded with acrylic resin. This simple procedure requires no special instruments and it facilitates accurate reduction and stable fixation of the fracture while allowing free movement of the hand. Access to the wound is unhampered in open fractures. Thirty-six fractures were treated by this technique. All healed within five weeks, during which time free movement of the hand was encouraged. There were no complications.

Management of hand injuries in a professional football team. Review of 15 years of experience with one team.

Thirty-eight players from one professional football team suffered 46 major hand and wrist injuries during a 15-year period. Twenty-one of the injuries occurred in offensive players and 25 occurred in defensive players. The injuries included fractures, dislocations, fracture dislocations, and soft tissue injuries of the phalanges, metacarpals, carpals (particularly the navicular), and distal radius/ulna, including intra-articular injuries. Twelve surgical procedures were performed. Open reduction, internal fixation, and Lightcast immobilization devices (3M Company, Atlanta, Georgia) allowed the

The carpus in congenital anomalies of the hand.

Congenital anomalies of the carpus associated with congenital hand and upper limb anomalies were investigated from the point of view of the development of the hand. Defects of the carpus were categorized as follows: 1) Radial ray defect: loss of the scaphoid, trapezium and trapezoid. 2) Central ray defect: loss of the capitate and loss of a part of the trapezium and hamate. 3) Ulnar ray defect: loss of the triguetrum, pisiformis and hamate. Deficiencies of the carpus and more distal parts of the hand proved to be secondary to defects of the forearm bones in both the radial and ulnar ray. On th

Dorsal metacarpal artery flaps with extensor indices tendons for reconstruction of digital defects.

INTRODUCTION: Distally based dorsal metacarpal artery (DMCA) flaps are an established technique for the reconstruction of extensive finger defects. In many cases, such defects also include an injury of the extension tendon over the proximal and distal finger joint, which can lead to a reduced range of motion or finger deformation such as boutonniére deformity. To prevent this, operative techniques are necessary that allow complete defect coverage while simultaneously stabilizing the extension apparatus. METHODS: In two cases, DMCA flaps were combined with vascularized extensor indices tendons

Magnetic resonance imaging features of proximal metacarpal and metatarsal injuries in the horse.

Magnetic resonance (MR) imaging abnormalities in horses with lameness localized to the proximal metacarpal or metatarsal region have not been described. To accomplish that, the medical records of 45 horses evaluated with MR imaging that had lameness localized to either the proximal metacarpal or metatarsal region were reviewed. Abnormalities observed in the proximal suspensory ligament or the accessory ligament of the deep digital flexor tendon included abnormal high signal, enlargement, or alteration in shape. Twenty-three horses had proximal suspensory ligament desmitis (13 hindlimb, 10 fore

Acute-onset, painful acral granuloma annulare: a report of 4 cases and a discussion of the clinical and histologic spectrum of the disease.

BACKGROUND: Granuloma annulare is a benign cutaneous inflammatory disease of unknown etiology most commonly presenting on the hands and feet and consisting of asymptomatic to mildly pruritic, flesh-colored to erythematous annular plaques. To our knowledge, an acute-onset, painful acral eruption has not been previously recognized. OBSERVATIONS: We report 4 patients who presented with acute-onset, painful acral granuloma annulare. CONCLUSIONS: It is possible for granuloma annulare to present as a painful eruption with an acute onset and an acral distribution. Biopsy results are a crucial facto

Avulsion of the extensor carpi radialis brevis insertion: a case report and review of the literature.

Injuries to the bases of the index finger and long finger metacarpals are unusual because of the stability of the carpometacarpal joints. Such stability is provided by the strong capsuloligamentous attachments and the unique bony architecture. Given the rare nature of these injuries, there is no consensus regarding the optimal management of avulsion fractures of the bases of the index finger and long finger metacarpals. Open reduction and internal fixation of the fracture, with anatomic repair of the extensor carpi radialis brevis or extensor carpi radialis longus, offers several advantages ov

A vascularized technique for bone-tissue-bone repair in scapholunate dissociation.

Several surgical options have been used for the repair of scapholunate instability over the last 50 years. The many options have included neglect, reduction with percutaneous pinning, primary repair, partial fusions, tendon weaves, and others. Recent advancements in scapholunate repair and anatomy have been aimed at a more physiological repair. Composite replacement of the entire scapholunate interval similar to other tendon repairs seen in orthopedic surgery has become popular. Currently, more common hand-based grafts are bone-retinaculum-bone, third or second metacarpal-carpal bone or hamate
