Weil and Dorsal Closing Wedge Osteotomy for Freiberg's Disease.

BACKGROUND: Weil osteotomy is a type of distal osteotomy for the treatment of lesser metatarsalgia by shortening the metatarsal length. We applied Weil and dorsal closing wedge osteotomy for the treatment of Freiberg's disease. METHODS: Between September 1, 2006, and December 31, 2011, we performed Weil and dorsal closing wedge osteotomy of the second metatarsal in 15 feet of 15 patients (12 women, three men) diagnosed as having Freiberg's disease. The mean patient age was 29 years (range, 19-51 years), and mean follow-up was 47 months (range, 36-72 months). Postoperative shortening of the me…

Plantar approach for Morton's neuroma: An effective technique for primary excision.

BACKGROUND: Morton's neuroma is a common cause of inter-metatarsal foot pain. Surgical excision is generally indicated when non-operative measures have been unsuccessful; various surgical techniques have been described in the literature for excision, with no consensus on the overall ideal surgical approach. AIM: To assess patient outcomes and complications following plantar surgical approach to neurectomy in a consecutive series of patients. METHOD: An analysis of consecutive patients undergoing excision of Morton's neuroma using a plantar approach by a single surgeon over a 12 month period.

Interposition bone block arthrodesis for revision hallux metatarsophalangeal joint surgery: a case series.

BACKGROUND: The operative management of failed hallux metatarsophalangeal joint surgery can be difficult. There is often substantial shortening of the first ray. Arthrodesis of the first metatarsophalangeal joint is one treatment option, but results in further shortening of the first ray. METHODS: We present a large retrospective series of patients who had an interposition bone block arthrodesis procedure performed using a nonvascularized tricortical autologous iliac crest bone graft in an attempt to maintain first ray length. Twenty-four patients (25 feet) underwent this salvage procedure ov

Operative treatment of Freiberg disease using extra-articular dorsal closing-wedge osteotomy: technical tip and clinical outcomes in 13 patients.

BACKGROUND: Extra-articular dorsal closing wedge osteotomy has been used with successful results for Freiberg disease; however, there has been concern regarding the technical difficulty. The purpose of this study was to address technical tips and report on the clinical and radiographic outcome using Kirchner wire-assisted dorsal closing wedge osteotomy. METHODS: A retrospective review of 13 patients with a mean age of 29.1 years was conducted. Extra-articular dorsal wedge osteotomy was performed after debridement of the joint. Two Kirschner wires were inserted and used as a joystick to manipu

Clinical and Structural Associations of Disability and Gait Performance in Patients With Rheumatoid Arthritis in Remission and Metatarsal Pain.

BACKGROUND: Patients with rheumatoid arthritis (RA) may continue to experience foot-related disability despite clinical remission. Foot involvement is associated with self-reported disability and objective gait alterations. Foot involvement is heterogeneous; studying patients with localised forefoot pain may help clarify which structural and inflammatory factors are associated with functional impairment. OBJECTIVES: To identify clinical, structural and imaging factors associated with disability and gait performance in patients with RA in clinical remission with metatarsal-region forefoot pain

Morton's neuroma: A clinical versus radiological diagnosis.

BACKGROUND: The aim of our study was to compare the clinical versus radiological diagnosis of patients suffering from a Morton's neuroma. METHODS: Clinical assessments and pre-operative radiological imaging of patients who had excision of a Morton's neuromas were retrospectively compared. RESULTS: 43 neuromas were excised from 36 patients over 68 months. The commonest clinical symptoms were tenderness on direct palpation (100%), pain on weight bearing (91%) which was relieved by rest (81%). The most sensitive clinical sign was a Mulder's click. Clinical assessment had a sensitivity of 98% (4

Utilization of the Free Fibular Flap for First Metatarsal Reconstruction.

Beyond being aesthetically unpleasing, metatarsal defects have been known to lead to several patient concerns such as intermetatarsal malpositioning and metatarsalgia. There are several reconstructive techniques that have been utilized for reconstruction of bony defects in the foot, including the free dorsal toe flap and dorsal metatarsal perforator flap. Our institution has utilized the free fibular flap for surgical management pertaining to tarsal reconstruction. Our study looks to evaluate the work of a single plastic surgeon and identify patient postoperative outcomes. A retrospective char…

Revision of failed foot surgery: a critical analysis.

The purpose of this study was to consider the causes of patient dissatisfaction following foot surgery and review whether poor outcomes are more commonly associated with particular techniques. Two hundred forty-four patients who had previously undergone foot surgery were referred to a podiatric surgery service because of continued pain, disability, footwear-fitting problems, and cosmetic concerns. The majority of cases had previously undergone first ray surgery to correct hallux valgus. The most common reason for referral was transfer metatarsalgia followed by recurrence of hallux valgus and l…

Ultrasound Relationship of Plantar Fat and Predislocation Syndrome.

BACKGROUND: Plantar fat plays a crucial role in protecting and cushioning the metatarsals. Its degeneration is a risk factor for the development of metatarsalgia and, consequently, predislocation syndrome. OBJECTIVES: To evaluate the relationship between plantar fat thickness and predislocation syndrome in an adult population, and to determine a possible association between a decrease in forefoot plantar fat and the presence of symptoms. MATERIAL AND METHODS: A retrospective observational study was conducted, including records of patients who visited the podiatry clinic between December 2022…

Hallux rigidus with an increased or equal first metatarsal length after a cheilectomy and first metatarsal shortening osteotomy (FMSO) using a small shortening scarf: A retrospective 7-year clinical and gait analysis follow-up.

Malpractice claims in forefoot surgery.

Foot and hallux valgus surgery are amongst the most commonly performed surgeries, with a growing number of procedures leading to litigation. The aim of this conference is to provide an update on the causes of malpractice claims and the associated risk factors. What are the causes of litigation? Acute or chronic residual pain, stiffness, metatarsalgia, consolidation delays, secondary displacements, suboptimal results are the most common causes found in litigious proceedings. What are the risk factors? Surgeon-related and patient-related risk factors exist. Percutaneous surgery or the developmen…

Ultrasound findings of plantar plate tears of the lesser metatarsophalangeal joints.

Lesser metatarsophalangeal (MTP) joint plantar plate tears are a common and increasingly recognised cause of metatarsalgia, MTP joint instability and forefoot deformity. Increased interest in early accurate diagnosis of plantar plate tears follows recent surgical innovations allowing direct plantar plate repair. The purpose of this review is to describe the ultrasound features of normal lesser MTP joint plantar plates and to characterise and illustrate the direct and indirect ultrasound findings of plantar plate tears. Lesser MTP joint plantar plate tears are most commonly located at the 2nd M…

The effectiveness of shoe modifications and orthotics in the conservative treatment of Civinini-Morton syndrome: state of art.

Civinini Morton's Syndrome (CMS), better known as Morton's Neuroma, is a benign enlargement that typically affects the third common digital branch of the plantar nerve. It is a common cause of metatarsalgia leading to debilitating pain. It prefers the female gender, with a female to male ratio of 5:1 and an average age of 50 years at time of surgery. Precise aetiology remains under debate, with four etiopathogenetic theories often cited in the literature. Clinical symptoms, physical exam and instrumental evidence are important in assessing and grading the disease. Biomechanics seem to play an …
