Primary left ventricular neuroendocrine tumor in a middle-aged female: a case report.

Primary neuroendocrine tumors (NETs) in the heart are exceptionally rare. Here, we report a case found at our hospital of a 51-year-old woman with a primary left ventricular NET. The patient presented with non-causal recurrent diarrhea for 2 years, abdominal pain, and vomiting. Endoscopy revealed chronic proctitis and duodenal ulcer but found no explanation for the clinical symptoms. Comprehensive cardiovascular tests were conducted, and a mass measuring 26 mm × 41 mm × 30 mm was detected in the left ventricle. The patient underwent complete surgical resection to remove the tumor. Postoperativ

Identifying Key Risk Factors for Dizziness Handicap in Middle-Aged and Older People.

OBJECTIVES: More than 10% of people aged 50 years and older report dizziness. Despite available treatments, dizziness remains unresolved for many people due in part to suboptimal assessment. We aimed to identify factors associated with dizziness handicap in middle-aged and older people to identify targets for intervention to address this debilitating problem. A secondary aim was to determine whether factors associated with dizziness differed between middle-aged (<70 years) and older people (≥ 70 years). DESIGN: Secondary analysis of baseline and prospective data from a randomized controlled t

'Organic Anxiety' in a Middle-aged Man Presenting with Dyspnoea: a Case Report.

We report a case of pulmonary embolism in a patient who presented with repeated anxiety attacks and psychotic symptoms and was misdiagnosed as having withdrawal seizure or anxiety disorder not otherwise specified. This case highlighted the nonspecific clinical features of pulmonary embolism and the principles in making psychiatric diagnosis. Careful history taking, thorough physical examination, appropriate investigation, and a high index of suspicion led to the correct diagnosis. The principle of hierarchy of psychiatric diagnosis (ie, organic over non-organic) and the possibility of comorbid

Cigarette smoking and cerebral blood flow in a cohort of middle-aged adults.

Cigarette smoking is often associated with dementia. This association is thought to be mediated by hypoperfusion; however, how smoking behavior relates to cerebral blood flow (CBF) remains unclear. Using data from the Coronary Artery Risk Development in Young Adults (CARDIA) cohort (mean age = 50; n = 522), we examined the association between smoking behavior (status, cumulative pack-years, age at smoking initiation, and years since cessation) and CBF (arterial spin labeling) in brain lobes and regions linked to dementia. We used adjusted linear regression models and tested whether association

Functionality in Middle-Aged and Older Overweight and Obese Individuals with Knee Osteoarthritis.

Patients with knee osteoarthritis (OA) suffer from immobility and pain. The objective of this cross-sectional study was to investigate the relationship between pain and functionality in middle-aged and older overweight and obese individuals with mild-to-moderate knee OA. Overall pattern, physical activity, and total energy expenditure (TEE) were assessed in 83 participants. The Western Ontario McMaster Universities Arthritis Index (WOMAC) was used to assess lower extremity pain and function. The six-minute walk test (6-MWT) and range of motion (ROM) were also assessed. Results indicated that a

Association between metabolic syndrome and osteoporosis in Taiwanese middle-aged and elderly participants.

UNLABELLED: This study examined the association between metabolic syndrome and osteoporosis among middle-aged and elderly Taiwanese participants. After controlling for body mass index, age, liver and renal functions, and nutrition and exercise statuses, we found no significant association between MS and osteoporosis in either gender. PURPOSE: The term metabolic syndrome (MS) encompasses different abnormalities with independent effects on bone metabolism, which has led to inconsistencies in the association between MS and osteoporosis. This study evaluated this association among middle-aged and

Sub-Clinical Cognitive Decline and Resting Cerebral Blood Flow in Middle Aged Men.

BACKGROUND: Although dementia is associated with both global and regional cerebral blood flow (CBF) changes, little is known about cerebral perfusion in the early pre-clinical stages of cognitive decline preceding overt cognitive dysfunction. The aim of this study was to investigate the association of early sub-clinical cognitive decline with CBF. MATERIALS AND METHODS: The study participants were recruited from a cohort of Danish men born in 1953. Based on a regression model we selected men who performed better (Group A, n = 94) and poorer (Group B, n = 95) on cognitive testing at age 57 tha

Physical activity and hippocampal volume in middle-aged patients with type 1 diabetes.

OBJECTIVE: To examine the cross-sectional association between physical activity (PA) and hippocampal volume in middle-aged adults with childhood-onset type 1 diabetes (T1D), and whether hyperglycemia and insulin sensitivity contribute to this relationship. METHODS: We analyzed neuroimaging and self-reported PA data from 79 adults with T1D from the Pittsburgh Epidemiology of Diabetes Complications Study (mean age 50 years, mean duration 41 years) and 122 similarly aged adults without T1D (mean age 48 years). Linear regression models, controlling for intracranial volume, sex, education, and age

Functional neuroimaging findings in healthy middle-aged adults at risk of Alzheimer's disease.

It is well established that the neurodegenerative process of Alzheimer's disease (AD) begins many years before symptom onset. This preclinical phase provides a crucial time-window for therapeutic intervention, though this requires biomarkers that could evaluate the efficacy of future disease-modification treatments in asymptomatic individuals. The last decade has witnessed a proliferation of studies characterizing the temporal sequence of the earliest functional and structural brain imaging changes in AD. These efforts have focused on studying individuals who are highly vulnerable to develop A

Preterm Delivery and Maternal Cardiovascular Disease in Young and Middle-Aged Adult Women.

BACKGROUND: Preterm delivery has been shown to be associated with increased risk of cardiovascular disease (CVD), but it is unknown whether this risk remains after adjustment for prepregnancy lifestyle and CVD risk factors. METHODS: We examined the association between history of having delivered an infant preterm (<37 weeks) and CVD in 70 182 parous women in the Nurses' Health Study II. Multivariable Cox proportional-hazards models were used to estimate hazards ratios (HRs) and 95% confidence intervals (CIs) for CVD events (myocardial infarction and stroke, n=949); we also adjusted for interm

Speech evoked auditory brainstem response and gap detection threshold in middle-aged individual.

This study aimed at characterizing the gap detection threshold (GDT) and speech evoked ABR (SABR) in younger and middle-aged individuals. Two groups of subjects were participated in the study which includes 15 young adults in the age range of 15-25 years and 15 middle-aged individuals in the age range of 40-60 years. SABR with stimulus/da/of 40 ms and GDT were investigated on both groups. For SABR, Mann-Whitney U test revealed that ageing has significantly adverse effect on the encoding of F1 and F2 at brainstem level. However, no significant effect of ageing (till middle age) on the encoding

Post-renal Transplantation de novo Renal Cell Carcinoma in a Middle-aged Man.

Renal cell carcinoma is usually seen in the native kidney but may be seen in the renal allograft. We report a rare case of renal cell carcinoma in a 56-year-old renal allograft recipient who was transplanted for end-stage renal disease induced by analgesic nephropathy. This complication developed after 13 years of renal transplantation. Patient was investigated for hematuria and abdominal pain with a normal renal function. Computed tomography depicted a mass sized 9.0×7.3×6.8 cm that involved the upper pole of the transplant. There was no metastasis. The patient underwent radical allograft nep

Dynapenic Obesity and Prevalence of Type 2 Diabetes in Middle-Aged Japanese Men.

BACKGROUND: The independent and combined associations of muscle strength and obesity on the prevalence of type 2 diabetes in Japanese men remain unclear. METHODS: Hand grip strength was cross-sectionally evaluated between 2011 and 2013 to assess muscle strength in 5039 male workers aged 40 to 64 years. Weight and height were measured, and overweight/obesity was defined as a body mass index ≥25 kg/m(2). The prevalence of type 2 diabetes, defined as fasting plasma glucose ≥126 mg/dL and/or hemoglobin A1c ≥6.5% and/or self-reported physician-diagnosed diabetes, was evaluated. Odds ratios (OR) an

Atrioventricular block as the initial manifestation of cardiac sarcoidosis in middle-aged adults.

INTRODUCTION: Atrioventricular block (AVB) can be caused by several conditions, including cardiac sarcoidosis (CS). The prevalence of CS causing this presentation in a North American population has not been investigated and was the purpose of this study. METHODS: We prospectively evaluated patients aged 18-60 years presenting with unexplained 2nd or 3rd degree AVB and no previous history of sarcoidosis in any organ. All patients had fluorodeoxyglucose-positron emission tomography (FDG-PET) scans for the evaluation of CS. Japanese Ministry of Health Welfare (JMHW) criteria and biopsy results w

Calcific periarthritis as the only clinical manifestation of hypophosphatasia in middle-aged sisters.

Hypophosphatasia (HPP) is the inborn error of metabolism that features low serum alkaline phosphatase (ALP) activity caused by loss-of-function mutation(s) within the gene for the tissue nonspecific isoenzyme of ALP (TNSALP). In HPP, extracellular accumulation of inorganic pyrophosphate (PPi), a TNSALP substrate and inhibitor of mineralization, leads frequently to premature tooth loss and often to rickets or osteomalacia. In affected adults, the excess PPi sometimes also causes calcium pyrophosphate dihydrate (CPPD) deposition, PPi arthropathy, or pseudogout, or seemingly paradoxical depositio
