Adverse childhood experiences and risk for suicidal behavior in male Iraq and Afghanistan veterans seeking PTSD treatment.

OBJECTIVE: Adverse childhood experiences (ACEs) are associated with increased risk for suicide and appear to occur in disproportionately high rates among men who served in the U.S. military. However, research has yet to examine a comprehensive range of ACEs among Iraq/Afghanistan veterans with combat-related posttraumatic stress disorder (PTSD) or whether these premilitary stressors may contribute to suicidal behavior in this highly vulnerable population. METHOD: A sample of 217 men entering a residential program for combat-related PTSD completed measures for ACEs, combat exposure, and lifeti

Chasing the care: soldiers experience following combat-related mild traumatic brain injury.

OBJECTIVE: One of the most common, yet most difficult to detect injuries sustained by U.S. soldiers in Iraq and Afghanistan is mild traumatic brain injury (mTBI). Left untreated, mTBI can negatively impact soldiers' postdeployment adjustment. This research describes the rehabilitation experiences of soldiers with a history of mTBI and their spouses. METHOD: Nine soldiers with mTBI and their spouses participated in the study. A total of 27 interviews were conducted exploring couples' post-mTBI rehabilitation experiences. Participants consisted of active duty soldiers with mTBI who had returned

Mental health among commando, airborne and other UK infantry personnel.

BACKGROUND: Despite having high levels of combat exposure, commando and airborne forces may be at less risk of mental ill-health than other troops. AIMS: To examine differences in mental health outcomes and occupational risk factors between Royal Marines Commandos (RMCs), paratroopers (PARAs) and other army infantry (INF). METHODS: Three groups of personnel (275 RMCs, 202 PARAs and 572 INF) were generated from a UK military cohort study of personnel serving at the time of the 2003 Iraq war. Participants completed a questionnaire about their mental health and experiences on deployment. Differ

Quarantine in a COVID-19 Pandemic: Lessons from a Deployed Role I.

The coronavirus (COVID-19) pandemic has changed the world; and the US military changed with it. Although this virus presents with a wide spectrum of disease progression (no symptoms to acute respiratory distress syndrome leading to death), its impact extends beyond health outcomes. At the time of this study, numerous research and development projects were underway to develop a COVID-19 vaccine or other treatment modalities; however, there were no Federal Drug Administration (FDA) approved vaccines or medical therapeutics that definitively provided a cure. Instead, public health officials relie

Can military trauma promote psychological growth in combat veterans? Results from the national health and resilience in veterans study.

BACKGROUND: Military combat trauma is often associated with negative outcomes, including high rates of posttraumatic stress disorder (PTSD). Less is known, however, about whether military combat trauma may foster posttraumatic growth (PTG), which has been observed in relation to other trauma types, in representative samples of veterans. METHODS: We analyzed data from veterans who participated in the National Health and Resilience in veterans Study who reported a military-related trauma (n = 210). Participants completed measures of trauma history, combat exposure, PTSD symptoms, PTG, functioni

Predeployment and in-theater diagnoses of American military personnel serving in Iraq.

OBJECTIVE: This purpose of this study was to determine the incidence and types of preexisting mental disorders among military personnel who received mental health services in an Iraqi war zone. METHODS: The study examined psychiatric histories of 1,078 American military personnel (Marines, 65%; Army, 23%; Navy, 11%; and Air Force, <1%) deployed to Iraq and seen by in-theater mental health providers between January 2006 and February 2007. RESULTS: Among the 1,078 patients, the most frequent in-theater diagnoses were anxiety (24%), adjustment (23%), and mood (19%) disorders. Twenty-nine percen

Pivmecillinam for Shigellosis With Limited Oral Therapeutic Alternatives: A 3-Case Series and Narrative Review of Published In Vivo Clinical Evidence.

Shigellosis remains highly relevant to military medicine because acute diarrheal illness during travel and deployment can impair operational readiness, reduce physical performance, and increase lost duty days. At the same time, oral treatment options are increasingly constrained by antimicrobial resistance, intolerance, contraindications, and occasional supply limitations in operational or austere settings. We report 3 adults with microbiologically documented shigellosis treated with oral pivmecillinam at our institution and summarize the limited published in vivo clinical evidence. In all 3 c

Navigating the management complexity in long-term asymptomatic immunodeficiency.

Background: Common variable immunodeficiency disorder (CVID) is a condition associated with recurrent infections and non-infectious outcomes, including lung disease like bronchiectasis and granulomatous and lymphocytic interstitial lung diseases (GLILD), autoimmune disease, enteropathy, and lymphoma. Treatment involves initiation of replacement immunoglobulin (Ig), which is a lifelong commitment. Prior to Ig replacement, life expectancy for patients with CVID was less than 15 years. With replacement Ig, it has improved to over 50 years. In most cases, patients present to a clinician with a his

Reproductive Health of Active Duty Women in Medically Austere Environments.

One in seven of the approximately 2.2 million Department of Defense active duty military personnel are women. Among active duty servicewomen, about 40% are under 26 years old, and almost half are young, lower ranking enlisted personnel. This article will include a review of the literature on military women's health topics such including contraception access, pregnancy, and pregnancy outcomes after environmental exposures. In these early adult years, contraception use may not be consistent, leading to higher rates of unintended pregnancy that is similar to their civilian counterparts, but it ma

Gastrointestinal infections, active component, U.S. Armed Forces, 2002-2012.

Acute gastroenteritis and other infectious disorders of the gastrointestinal system are common in civilian and military populations. During the years 2002 through 2012, there were 286,305 cases of gastrointestinal infection (GI) diagnosed among members of the active component of the U.S. Armed Forces. The distribution of presumed causes of these illnesses (as reported in administrative medical records) was bacterial (29%), viral (68%), and parasitic (3%). Most recorded diagnoses did not specify an etiologic agent. In addition, there were 379,509 other healthcare encounters in which the recorde

Risky driving among regular armed forces personnel from the United Kingdom.

BACKGROUND: Road traffic accidents are the leading cause of death for service personnel from the United Kingdom (UK). Little is known about the pattern of risky driving by these service personnel. METHODS: Cross-sectional data (collected postdeployment, between June 2004 and March 2006) were analyzed from a large, randomly selected cohort of military personnel from the UK. These analyses were limited to regular-service personnel who were drivers (n=8,127; 7,443 men and 684 women). "Risky driving" (not wearing a seatbelt, speeding, or both) was examined. Analyses were then repeated but restric

Cohort profile: a migratory cohort study of US Marines who train in Australia.

PURPOSE: In 2012, US Marines and Sailors began annual deployments to Australia to participate in joint training exercises with the Australian Defence Force and other partners in the region. During their training, US service members are exposed to a variety of infectious disease threats not normally encountered by American citizens. This paper describes a cohort of US Marines and Sailors enrolled during five rotations to Australia between 2016 and 2020. PARTICIPANTS: Study participation is strictly voluntary. Group informational sessions are held prior to deployment to describe the study struc

Prevalence and predictors of anxiety disorder courses in the Canadian Armed Forces.

Few studies have examined the longitudinal courses of anxiety disorders in military members. This study examined the prevalence and predictors of courses of any anxiety disorder in members and veterans of the Canadian Armed Forces, including no lifetime, remitted, new onset, and persistent/recurrent anxiety disorder. The 2018 Canadian Armed Forces Members and Veterans Mental Health Follow-up Survey is a 16-year follow-up of n = 2941 participants from the Canadian Community Health Survey: Canadian Forces Supplement in 2002. Diagnoses of any DSM-IV anxiety disorder (i.e., generalized anxiety, so

A Systematic Review of the Effect of Dietary Supplements on Cognitive Performance in Healthy Young Adults and Military Personnel.

Intake of dietary supplements has increased, despite evidence that some of these have adverse side effects and uncertainty about their effectiveness. This systematic review examined the evidence for the cognitive benefits of a wide range of dietary supplements in healthy young adult samples; the aim was to identify if any might be useful for optimising cognitive performance during deployment in military personnel. Searches were conducted in 9 databases and 13 grey literature repositories for relevant studies published between January 2000 and June 2017. Eligible studies recruited healthy young

Age moderates the association of depressive symptoms and unhealthy alcohol use in the National Guard.

Unhealthy drinking is a significant problem contributing to poor health and performance of military personnel. The Iowa Army National Guard and the Iowa Department of Public Health have collaborated with the Substance Abuse and Mental Health Administration to better identify unhealthy substance use via Screening, Brief Intervention, and Referral to Treatment program (SBIRT). Yet, little research has been conducted on the Guard's use of SBIRT. This study examined depression, age, deployment status, and sex as factors contributing to unhealthy drinking. Of the Guardsmen who took part in SBIRT, 3

Sunburn among active component service members, U.S. Armed Forces, 2002-2013.

Sunburn is caused by acute overexposure to ultraviolet (UV) radiation directly from the sun or from artificial UV sources. Service members are at risk of excessive exposure to sunlight due to the nature of their military duties, which often involve working and training outdoors, and deployment to environments where UV radiation is more intense. From January 2002 through December 2013, a total of 19,172 incident cases of clinically significant sunburn were diagnosed among active component service members. Most of the cases (80.2%) were first degree sunburn. The incidence rates of sunburn diagno
