Developing a moral compass: Themes from the Clinical Ethics Residency for Nurses' final essays.

BACKGROUND: The Clinical Ethics Residency for Nurses was offered selectively to nurses affiliated with two academic medical centers to increase confidence in ethical decision-making. RESEARCH QUESTION/AIM: To discover how effective the participants perceived the program and if their goals of participation had been met. RESEARCH DESIGN: A total of 65 end-of-course essays (from three cohorts) were analyzed using modified directed content analysis. In-depth and recursive readings of the essays by faculty were guided by six questions that had been posed to graduates. ETHICAL CONSIDERATIONS: Ins

Moral judgment modulation by disgust priming via altered fronto-temporal functional connectivity.

Moral judgments are not just the product of conscious reasoning, but also involve the integration of social and emotional information. Irrelevant disgust stimuli modulate moral judgments, with individual sensitivity determining the direction and size of effects across both hypothetical and incentive-compatible experimental designs. We investigated the neural circuitry underlying this modulation using fMRI in 19 individuals performing a moral judgment task with subliminal priming of disgust facial expressions. Our results indicate that individual changes in moral acceptability due to priming co

Update in Hospital Palliative Care: Symptom Management, Communication, Caregiver Outcomes, and Moral Distress.

BACKGROUND: Updated knowledge of the palliative care (PC) literature is needed to maintain competency and best address the PC needs of hospitalized patients. We critiqued the recent PC literature with the highest potential to impact hospital practice. METHODS: We reviewed articles published between January 2016 and December 2016, which were identified through a handsearch of leading journals and a MEDLINE search. The final 9 articles selected were determined by consensus based on scientific rigor, relevance to hospital medicine, and impact on practice. RESULTS: Key findings include the follo

Social Justice as the Moral Core of Family Medicine: A Perspective from the Keystone IV Conference.

A recurring conference theme was the essential place of social justice within family medicine, especially the need to focus on denominator populations, exalt the personal and caring qualities of doctoring, and address social determinants of health. Many expressed solidarity with "community," but it is not always easy to define community in our large and diverse nation. Exhortations for health advocacy were frequently voiced, but putting these into meaningful action agendas is a challenge. There was general agreement that medicine is in flux and that the many expressions of "commodity-centered

Assessing and addressing moral distress and ethical climate Part II: neonatal and pediatric perspectives.

Moral distress remains a pervasive and, at times, contested concept in nursing and other health care disciplines. Ethical climate, the conditions and practices in which ethical situations are identified, discussed, and decided, has been shown to exacerbate or ameliorate perceptions of moral distress. The purpose of this mixed-methods study was to explore perceptions of moral distress, moral residue, and ethical climate among registered nurses working in an academic medical center. Two versions of the Moral Distress Scale in addition to the Hospital Ethical Climate Survey were used, and partici

"It was only harmless banter!" The development and preliminary validation of the moral disengagement in sexual harassment scale.

Sexual harassment represents aggressive behavior that is often enacted instrumentally, in response to a threatened sense of masculinity and male identity. To date, however, theoretical attention to the social cognitive processes that regulate workplace harassment is scant. This article presents the development and preliminary validation of the Moral Disengagement in Sexual Harassment Scale (MDiSH); a self-report measure of moral disengagement in the context of hostile work environment harassment. Three studies (total n = 797) document the excellent psychometric properties of this new scale. Ma

The salience network causally influences default mode network activity during moral reasoning.

Large-scale brain networks are integral to the coordination of human behaviour, and their anatomy provides insights into the clinical presentation and progression of neurodegenerative illnesses such as Alzheimer's disease, which targets the default mode network, and behavioural variant frontotemporal dementia, which targets a more anterior salience network. Although the default mode network is recruited when healthy subjects deliberate about 'personal' moral dilemmas, patients with Alzheimer's disease give normal responses to these dilemmas whereas patients with behavioural variant frontotempo

Maternal incest as moral panic: envisioning futures without fathers in the South African lowveld.

During 2008, rumours about revolting incestuous encounters between sons and their mothers circulated in the Bushbuckridge municipality of the South African lowveld. This article views these rumours as expressing moral panic, paying particular attention to the historical contexts of their emergence and circulation, and to their temporal orientation. I locate these rumours in the periphery of South Africa's de-industrialising economy, marked by increased unemployment and criminality among men and by a growing prominence of women-headed households. They express a regressive temporalisation and pe

Moral landscapes and everyday life in families with Huntington's disease: aligning ethnographic description and bioethics.

This article is concerned with understanding moral aspects of everyday life in families with Huntington's Disease (HD). It draws on findings from an empirical research project in Denmark in 1998-2002 involving multi-sited ethnography to argue that medical genetics provides a particular framework for conducting life in an HD family. A framework that implies that being informed and making use of genetic services expresses greater moral responsibility than conducting life without drawing on these resources. The moral imperative of engagement in medical genetics is challenged here by two pieces of

The forsaken mental health of the Indigenous Peoples - a moral case of outrageous exclusion in Latin America.

BACKGROUND: Mental health is neglected in most parts of the world. For the Indigenous Peoples of Latin America, the plight is even more severe as there are no specific mental health services designed for them altogether. Given the high importance of mental health for general health, the status quo is unacceptable. Lack of research on the subject of Indigenous Peoples' mental health means that statistics are virtually unavailable. To illustrate their mental health status, one can nonetheless point to the high rates of poverty and extreme poverty in their communities, overcrowded housing, illite

Daily living with distress and enrichment: the moral experience of families with ventilator-assisted children at home.

OBJECTIVE: The growing shift toward home care services assumes that "being home is good" and that this is the most desirable option. Although ethical issues in medical decision-making have been examined in numerous contexts, home care decisions for technology-dependent children and the moral dilemmas that this population confronts remain virtually unknown. This study explored the moral dimension of family experience through detailed accounts of life with a child who requires assisted ventilation at home. This study involved an examination of moral phenomena inherent in (1) the individual exper

An up-to-date review of theories of shock therapies: are convulsive shock therapies "moral treatments"?

About four decades ago, empirically developed shock therapies were introduced as treatments for mental disorders. However, there still remains today a wide disagreement as to why these procedures are effective. It has long been known that the behavior in lower animals can be altered by the use of planned psychologcal methods. Likewise psychological procedures (employing such factors as suggestion and fear) had been used by preliterate man all through the ages in the treatment of mental illness. During the past century various types of pre-modern shock therapies using suggestion, fear or excita

The Longitudinal Associations Between Hostile Automatic Thoughts, Moral Disengagement, and Aggression: A Cross-Lagged Study.

Numerous studies have clearly identified that cognitive and moral factors are important predictors of aggression, but the longitudinal interaction between hostile automatic thoughts (HAT), moral disengagement (MD) and aggression has not been sufficiently explored. Based on the General Aggression Model (GAM) and script theory, this study proposes that HAT and MD may form a cognitive-moral system that is associated with aggression over time. Using a two-wave longitudinal research design with an interval of 6 months, we obtained data from 941 Chinese college students. Cross-lagged analyzes reveal

Professional Ethics Education and Moral Sensitivity in Public Health Students: A Pilot Quasi-Experimental Study.

BACKGROUND: Public health professionals frequently encounter complex ethical dilemmas, yet dedicated ethics courses are often absent from curricula. This pilot study aimed to explore potential associations between a structured professional ethics course and self-reported moral sensitivity in undergraduate public health students using a controlled but non-randomized design. Given the exploratory nature of this study, all findings should be interpreted as hypothesis-generating rather than conclusive. METHODS: A quasi-experimental, non-equivalent control group design was conducted with 53 underg

The harm that wasn't: moral residue, near-miss events, and the ethics of harmless error in procedural medicine.

A surgeon performing a retroperitoneal procedure inadvertently begins dissecting the wrong kidney. The error is recognized intraoperatively, the correct structure is identified, and the procedure is completed successfully. The patient suffers no measurable clinical harm. The institution classifies the event as a near-miss, and no disclosure is initiated. This paper argues that the classification, while legally defensible, is morally inadequate. The absence of measurable clinical harm does not eliminate the ethical obligations a serious procedural error generates; it displaces them onto forms o

End of life in the ICU: A scoping review of factors associated with moral distress in critical care nurses.

BACKGROUND: Moral distress is common among critical care nurses providing end-of-life (EOL) care, yet contributing factors are not fully synthesized. PURPOSE: To summarize the literature on factors influencing moral distress in adult ICU nurses delivering EOL care. METHODS: A scoping review of moral distress in adult ICU nurses providing EOL care, published 1984 to 2025, was conducted across five databases. Data on study design and findings were extracted and thematically analyzed. Study quality was assessed using the Mixed Methods Appraisal Tool. DISCUSSION: Moral distress among ICU nurses
