Long-term analgesia with epidural morphine (EM) is a new tool in the management of intractable cancer pain. Twenty-six out of 160 cancer patients referred to the Pain Division for pain assessment were selected for analgesia with long-term epidural morphine, so aiming to define its place amongst more traditional methods of treatment, such as drugs, nerve-blocks, neurosurgery or radiotherapy. The renal response to the acute administration of morphine was examined in conscious, chronically catheterized, nonhydrated rats. In 7 healthy volunteers 4% morphine eye-drops, when administered to one eye, caused a miosis limited to that eye. In 7 other healthy volunteers morphine was administered into one eye after bilateral instillation of 0.5% homatropine ophthalmic drops; the eye treated with morphine and homatropine showed a mydriasis less intense than the other eye treated only with homatropine. It is suggested that topical morphine locally affects sympathetic function by inhibiting noradrenaline release into the iris neuromuscular junction.

The involvement of the angiotensin-adrenal system as a possible mechanism in the potentiation of morphine analgesia by the angiotensin-converting enzyme inhibitor, captopril (SQ 14225), was studied in rats. Captopril pretreatment sensitized the animals to the analgesic effects of morphine while angiotensin II exerted an attenuating influence. Aggression, which is normally seen during withdrawal from narcotics, could not be produced in morphine-dependent rats by the administration of naloxone at doses which causes other signs of withdrawal. Apomorphine injected instead of naloxone was capable of producing aggression, without other withdrawal signs. Naturally occurring aggression (72-hr withdrawal) was enhanced by apomorphine and unaffected by naloxone.
