Systolic anterior motion causing hemodynamic instability and pulmonary edema during bleeding.

Systolic anterior motion (SAM) of mitral valve is the prolapse of a mitral leaflet into the left ventricle outflow tract (LVOT) during systole, causing LVOT obstruction and mitral valve regurgitation. We report the case of a patient who developed SAM-induced hemodynamic instability during bleeding with a clinical picture resembling pulmonary edema. A 77-year-old woman was admitted to our emergency room for abdominal bleeding in polycystic renal disease. Upon arrival, she was normotensive, despite being anuric and acidotic. After infusion of fluids and packed red blood cells (total 3 680 mL in

Hazards in the theater.

The authors offer a survey of the myriad and unique safety and health hazards faced past and present by performers and theatrical workers, from preproduction work, through the show, and during the strike (dismantling). Special emphasis is given to health hazards posed by the many new plastic resin systems and adhesives used in set, prop, and costume construction; the hazards of special-effect fogs, smokes, haze, dusts, and pyrotechnic emissions; and theatrical makeup.

Low glucose metabolism during brain stimulation in older Down's syndrome subjects at risk for Alzheimer's disease prior to dementia.

OBJECTIVE: Down's syndrome is characterized by the genetically programmed accumulation of substantial Alzheimer's disease neuropathology after age 40 and the development of early dementia years later, providing a unique human model to investigate the preclinical phases of Alzheimer's disease. Older nondemented adults with Down's syndrome show normal rates of regional cerebral glucose metabolism at rest before the onset of dementia, indicating that their neurons maintain function at rest. The authors hypothesized that an audiovisual stimulation paradigm, acting as a stress test, would reveal ab

Long-term cinemicrography of erythroblasts from a patient with congenital dyserythropoietic anemia type III: direct observation of dysplastic erythroblast formation.

Erythroblasts from a patient with congenital dyserythropoietic anemia type III (CDA III) were observed by time-lapse phase-contrast cinemicrography. The erythroblasts were compressed with a new mechanical device; this procedure enhanced the visualization of their structures. Soret band micrography was carried out simultaneously to assess cytoplasmic hemoglobinization. The proliferation and differentiation course of the majority of CDA III erythroblasts was similar to that of normal erythroblasts. The formation of dysplastic erythroblasts showed two patterns of progression: (1) Usually, the pro

Virulence gene regulation in Pseudomonas aeruginosa is mediated by a novel hinge motion in the signaling helix of GacS histidine kinase.

Sensor kinases (SKs) play central roles in bacterial adaptation to environmental challenges. Signal perception by these membrane-embedded enzymes initiates phosphotransfer cascades that regulate gene expression through cognate transcription factors. Although SKs were thought to function independently, interwoven multi-kinase networks have revealed a more complex picture. The present study focuses on GacS, a major regulator of virulence gene expression in the opportunistic pathogen Pseudomonas aeruginosa. Molecular dynamics (MD) simulation revealed a novel hinge motion within the GacS S-helix r

Energy-selective confinement of fusion-born alpha particles during internal relaxations in a tokamak plasma.

Long-pulse operation of a self-sustained fusion reactor using toroidal magnetic containment requires control over the content of alpha particles produced by D-T fusion reactions. On the one hand, MeV-class alpha particles must stay confined to heat the plasma. On the other hand, decelerated helium ash must be expelled before diluting the fusion fuel. Here, we report results of kinetic-magnetohydrodynamic hybrid simulations of a large tokamak plasma that confirm the existence of a parameter window where such energy-selective confinement can be accomplished by exploiting internal relaxation even

Talar injuries--the orthopaedic challenge.

Injuries of the talus represents an important part of the foot and ankle trauma. Since talar bone connect the lower limb and foot, the sequelas of its trauma could have significant influence on the function of the whole lower limb and gait. The specific vascularization of the talus results in delayed union and even in the avascular necrosis. The diagnosis of the fractures of the talus can be made on the x-rays, but sometimes real picture of the fracture pattern can be seen only in the CT scans. Ocult fractures such as osteochondral fractures and avascular necrosis can be exactly detected on MR

Forward head posture and neck mobility in chronic tension-type headache: a blinded, controlled study.

Forward head posture (FHP) and neck mobility were objectively assessed in 25 patients with chronic tension-type headache (CTTH) and 25 healthy controls. Side-view pictures were taken in a sitting position to measure the cranio-vertebral angle. A cervical goniometer was employed to measure the range of all cervical motions. Patients with CTTH showed a smaller cranio-vertebral angle (45.3 degrees +/- 7.6 degrees) than controls (54.1 degrees +/- 6.3 degrees), thus presenting a greater FHP (P < 0.001). Patients also had lesser neck mobility for all cervical movements, except for right lateral flex

[Cancer on the big screen. How and when movies deal with oncological diseases].

Films that feature characters with cancer have become a familiar sight for movie-goers. 148 movies treating tumors were selected, produced all over the world since the Thirties, in which cancer had "prompt", "relevant", or "plot" character. In order to clearly understand each film's peculiar message about cancer, we recollected data such as genealogy, year and country of production, main characters' age and gender, and kind of tumor. Movies deal with cancer through very relevant questions, as well as themes and contexts that have great influence on oncologist's mind and consciousness. Speciall

Can regurgitant flow damage the left atrial endothelium in patients with prosthetic mechanical heart valves?

BACKGROUND AND AIM OF THE STUDY: Previous in-vitro studies of mechanical heart valves (MHVs) in the closed position demonstrated the formation of regurgitant flows, with bubbles and jets forming vortices during each systole. The study aim was to determine whether the regurgitant flow observed in patients with MHVs can damage the left atrial endothelium, due to shear stresses exerted on the endothelial layers. This objective has been accomplished by appropriate in-vitro simulation experiments. METHODS: In these experiments, leakage flow through several commercial MHVs was investigated. The geo

Laryngeal function in postpolio patients.

Of the 250,000 survivors of the polio epidemics, approximately 25% experience progressive muscle weakness known as postpolio syndrome (PPS). Laryngeal function in postpolio patients previously has not been studied. This paper presents data detailing laryngeal function in a group of postpolio patients who had been evaluated for swallowing complaints. Nine patients underwent comprehensive history and physical exam, acoustical voice analysis, and laryngeal videostroboscopic endoscopy. Three patients underwent laryngeal electromyography (EMG) evaluation. Results indicated some degree of phonatory

Fusiform function in children with an autism spectrum disorder is a matter of "who".

BACKGROUND: Despite the importance of face processing for normal social development, no functional magnetic resonance imaging studies of face processing in autism have focused exclusively on the childhood years. To fill this gap, 45 children aged 6-12 participated in practice scans. After exclusion due to motion, 11 children with an ASD and 11 age-matched normal control subjects were included in final analyses. METHODS: Stimuli consisted of pictures of a familiar adult, familiar child, stranger adult, stranger child, and objects. During the scan, children pressed a button in response to an id

Electronic screen media for persons with autism spectrum disorders: results of a survey.

Social and anecdotal reports suggest a predilection for visual media among individuals on the autism spectrum, yet no formal investigation has explored the extent of that use. Using a distributed questionnaire design, parents and caregivers report on time allotted toward media, including observable behaviors and communicative responses. More time was spent engaged with electronic screen media (ESM) than any other leisure activity. Television and movie viewing was more popular than computer usage. Across media platforms, animated programs were more highly preferred. Prevalent verbal and physica

Emotions are associated with the genesis of visually induced motion sickness in virtual reality.

Visually induced motion sickness (VIMS) is a well-known side effect of virtual reality (VR) immersion, with symptoms including nausea, disorientation, and oculomotor discomfort. Previous studies have shown that pleasant music, odor, and taste can mitigate VIMS symptomatology, but the mechanism by which this occurs remains unclear. We predicted that positive emotions influence the VIMS-reducing effects. To investigate this, we conducted an experimental study with 68 subjects divided into two groups. The groups were exposed to either positive or neutral emotions before and during the VIMS-provok

Composition-Dependent Li-Ion Transport in Mixed-Metal ZIF-62 Glasses Revealed by Machine-Learning Molecular Dynamics.

Understanding ion transport in metal-organic frameworks requires resolving the interplay between framework dynamics, local disorder, and thermally activated hopping on extended time and length scales. Here, we develop a robust deep neural network (DNN) interatomic potential, trained and validated against density functional theory data using the DP-GEN framework, to investigate Li-ion transport in pure and mixed-metal ZIF-62 across a wide range of Co/(Co + Zn) ratios. Large-scale deep potential molecular dynamics simulations at 450 K reveal that Li-ion motion is governed by localized vibrationa

Nonreciprocal charge transport in quantum materials: From superconductors to magnetic systems.

Nonreciprocal charge transport, where current flows preferentially in one direction, has become a key topic across quantum materials. This review covers some recent progress in bulk crystals, artificial heterostructures, van der Waals systems, high-temperature superconductors, and magnetic materials. The review discusses both intrinsic origins, such as band-structure effects, and extrinsic ones, including vortex motion, device geometry, and heating artifacts. A central focus is the superconducting diode effect due to its application potential, which shows different critical currents for opposi
