Breathing, voice, and movement therapy: applications to breathing disorders.

Elsa Gindler (1885-1961) developed a holistic approach to the human body and psyche via the movement of breath. Gindler experimented with movements to strengthen the deeper layers of the muscular system and improve the circulation of oxygen, movements that reduced tensions that had been preventing the breathing muscles from functioning properly. Subsequently, she founded a school for breathing and body awareness. The biggest breathing muscle in the human body is the diaphragm, the lowering of which can only take place when the jaw and the throat are relaxed, the belly is free, and the psoas (m

Tardive stereotypy and other movement disorders in tardive dyskinesias.

We reviewed the medical records and videotapes of 100 patients with tardive dyskinesia (TD) referred to our movement disorders clinic to characterize the spectrum of hyperkinetic movement disorders caused by dopamine receptor blocking drugs (DRBD). Tardive stereotypy, present in 78 patients, was the most common type of TD, followed by tardive dystonia, akathisia, tremor, chorea, and myoclonus. Sixty-four had a combination of these hyperkinesias. In a second study, a "blind" review of videotapes of patients with a variety of movement disorders found that DRBD were the cause of stereotypic movem…

Association of psychosis and movement disorders in the elderly.

There are a number of different relationships among aging, psychosis and movement disorders, most of which have been proposed to involve the neurotransmitter dopamine. Dopamine content and dopamine receptors have been shown to decrease with age, which may relate to the time of onset of different motor and psychotic disorders, as well as to the appearance of these disorders. For example, some so-called senile movement disorders, such as senile tremor and senile chorea, may relate to alterations in dopaminergic transmission with age, as might the general findings of increased slowing of movement…

Eye-movement disorders in brain-stem and cerebellar stroke.

Vertebrobasilar strokes can yield varied disturbances of eye movements, by affecting specific centers and pathways contained in the brain stem and cerebellum. Unique disorders combining supranuclear, nuclear, and infranuclear syndromes may occur. Some eye-movement abnormalities are useful localizing signs (eg, gaze palsies, rotatory nystagmus, and ipsilateral saccadic bias), but many others are not. The use of techniques such as magnetic resonance imaging may provide new insights in clinicotopographic correlations in patients with good recovery, in the absence of pathologic verification.

Eye movement disorders in multiple sclerosis and optic neuritis.

Horizontal saccadic and smooth pursuit eye movements were studied in 84 patients with multiple sclerosis (MS) and 21 patients with optic neuritis (ON). The MS patients were clinically classified as 'definite', 'probable', or 'possible'; subclinical eye movement disorder was found in 80 per cent of the definite, 74 per cent of the probable and 60 per cent of the possible category. Five of the ON patients (25 per cent) showed a subclinical eye movement deficit; these 5 were young patients with a recent history of ON. In a group of 27 MS patients with symptoms of spinal cord involvement only, 14

Eccentric head positions reveal disorders of conjugate eye movement.

The effect of head position on conjugate horizontal gaze was studied in healthy adults, in patients with multiple sclerosis without eye movement signs, and in patients with downbeat nystagmus indicative of low brain stem lesions. Displacements of gaze from primary position to 30 degrees left and right were recorded using the electro-oculogram, with the head in the primary position, and turned voluntarily to the left and right (in yaw). The quality of eye movements was noted and peak velocities of saccades were measured. The head turning test trebled the incidence of abnormal eye movements foun

Are movement disorders and sensorimotor injuries pathologic synergies? When normal multi-joint movement synergies become pathologic.

The intact nervous system has an exquisite ability to modulate the activity of multiple muscles acting at one or more joints to produce an enormous range of actions. Seemingly simple tasks, such as reaching for an object or walking, in fact rely on very complex spatial and temporal patterns of muscle activations. Neurological disorders such as stroke and focal dystonia affect the ability to coordinate multi-joint movements. This article reviews the state of the art of research of muscle synergies in the intact and damaged nervous system, their implications for recovery and rehabilitation, and

Is normal pressure hydrocephalus a movement disorder?

Idiopathic normal pressure hydrocephalus (iNPH) represents a nosographic entity characterized by phenotypic variability. In this context, th … ary/atypical parkinsonism, indicating iNPH as a neurological condition that should be considered by movement disorders specialists. We will also propose a preliminary diagnostic algorithm combining clinical, imaging and biological markers leading to a multidimensional diagnosis of iNPH associated with parkinsonism.

Gaps and Controversies in Catatonia as a Movement Disorder.

The term "catatonia" was introduced by German psychiatrist Karl Kahlbaum in 1874. Although historically tied to schizophrenia, catatonia exhibits a diverse range of phenotypes and has been observed in various medical and neuropsychiatric conditions. Its intrinsic movement characteristics and association with hypokinetic and hyperkinetic phenomenologies place catatonia within the purview of movement disorders. Despite the presence of catatonia in psychiatry literature for over 150 years, many gaps and controversies persist regarding its etiopathogenesis, phenomenology, diagnostic criteria, and …

Neuropsychiatric phenotypes in functional movement disorder.

OBJECTIVE: Functional movement disorder (FMD), the motor-dominant subtype of functional neurological disorder, is a complex neuropsychiatric condition. Patients with FMD also manifest non-motor symptoms. Given that patients with FMD are diagnosed based on motor phenotype, the contribution of non-motor features to the neuropsychiatric syndrome is not well characterized. The objective of this hypothesis-generating study was to explore potential novel, neuropsychiatric FMD phenotypes by combining movement disorder presentations with non-motor comorbidities including somatic symptoms, psychiatric

Vergence eye movements in bipolar disorder.

OBJECTIVES: With respect to bipolar disorder (BD), previous research have demonstrated saccadic eye movements abnormalities, manifested mainly as an increase in reaction time (latency) in both prosaccadic and antisaccadic task. So far, there were no studies related to vergence eye movements in subjects with BD. Our primary aim was to evaluate vergence tracking performance in this clinical group. METHODS: 30 patients with BD in remission and 23 healthy controls were enrolled. Subjects underwent optometric examination where near point of convergence was measured by the use of Wolff Wand. Stereotypic movement disorder: easily missed.

AIM: To expand the understanding of stereotypic movement disorder (SMD) and its differentiation from tics and autistic stereotypies. METHOD: Forty-two children (31 males, mean age 6y 3mo, SD 2y 8mo; 11 females, mean age 6y 7mo, SD 1y 9mo) consecutively diagnosed with SMD, without-self-injurious behavior, intellectual disability, sensory impairment, or an autistic spectrum disorder (ASD), were assessed in a neuropsychiatry clinic. A list of probe questions on the nature of the stereotypy was administered to parents (and to children if developmentally ready). Questionnaires administered include

Disorders of head and eye movements in children.

Disturbances of the normal relationship of head and eye movements occur in several pathological conditions, including disorders of rapid eye movements and various forms of congenital nystagmus. In disorders of rapid eye movements the abnormal head movements may be a learned response using various techniques to improve eye velocity and amplitude. In congenital nystagmus with head shaking, the head movements may be compensatory, of common genesis, or suppressive. The vestibulo-ocular reflex may be manipulated in certain circumstances to achieve eye stability or to increase eye movement velocity.

Dr Samuel Johnson's movement disorder.

Dr Samuel Johnson was noted by his friends to have almost constant tics and gesticulations, which startled those who met him for the first time. He also made noises and whistling sounds; he made repeated sounds and words and irregular or blowing respiratory noises. Further, he often carried out pronounced compulsive acts, such as touching posts, measuring his footsteps on leaving a room, and performing peculiar complex gestures and steps before crossing a threshold. His symptoms of (a) involuntary muscle jerking movements and complex motor acts, (b) involuntary vocalisation, and (c) compulsive
