Writers' cramp-a focal dystonia.

We have examined 29 subjects with writers' cramp (and 4 with typists' and one with pianists' cramp) and have noted two major groupings, simple and dystonic. We have observed spread from one to the other. We have seen repeatedly, in patients with isolated simple writers' cramp certain subtle physical signs which are found also in other basal ganglia diseases. We have noted also the frequent association of other features of segmental and generalized dystonia in patients with dystonic writers' cramp. We have demonstrated that patients with isolated writers' cramp have no higher an incidence of ps

[Nocturnal leg cramps--their causes and treatment].

Organic causes for nocturnal or recumbency leg cramps as venous, arterial, statical, nervous, rheumatical and metabolic diseases such as tetany, diabetes and gout must be excluded. This means also electrolyte disturbances during a medical treatment, for example with diuretics. In addition there is a very high portion of idiopathic cramps. In 22 patients with nocturnal leg cramps a four week double blind study with a combination of quinine sulphate and aminophylline (Limptar) was started. Unequivocally, Limptar significantly reduced the number and the intensity of nocturnal or recumbency leg cr

Leptospirosis presenting as atypical pneumonia, respiratory failure and pyogenic meningitis.

A 20-year-old man who looked after a pack of hounds was admitted with a short history of headache, fever, haemoptysis and muscle cramps. Investigations showed type I respiratory failure with diffuse pneumonitis, pyogenic meningitis and raised liver enzymes. Leptospirosis was suspected and treated with the appropriate antibiotics leading to a full recovery. The diagnosis was confirmed by a titre of I/2560 to Leptospira icterohaemorrhagiae. The case illustrates an unusual presentation of this infection and argues for early antibiotic treatment.

The Effect of Hyperventilation Syndrome on Ionized and Serum Calcium: A Case Presentation in the Emergency Department.

Hyperventilation syndrome is described as a constellation of symptoms that are typically benign but can become a medical emergency in the setting of severe metabolic derangement secondary to shifts in a patient's pH. A 36-year-old male with a history of intravenous (IV) drug abuse presented to the emergency department (ED) in distress, complaining of diffuse muscle cramping with tetany and peripheral vasospasm. This case report discusses his initial ED testing and treatment when ionized calcium was low and serum calcium was high.

CACNA1S Variant Associated With a Myalgic Myopathy Phenotype.

BACKGROUND AND OBJECTIVES: This study aimed to characterize the phenotype of a novel myalgic myopathy encountered in a Finnish family. METHODS: Four symptomatic and 3 asymptomatic individuals from 2 generations underwent clinical, neurophysiologic, imaging, and muscle biopsy examinations. Targeted sequencing of all known myopathy genes was performed. RESULTS: A very rare CACNA1S gene variant c.2893G>C (p.E965Q) was identified in the family. The symptomatic patients presented with exercise-induced myalgia, cramping, muscle stiffness, and fatigue and eventually developed muscle weakness. Exami

The ischemic exercise test in normal adults and in patients with weakness and cramps.

Data from 23 normal men and women were used to derive 95% confidence limits for maximum changes in ammonia and lactate values following ischemic forearm exercise. Most normal subjects raised serum lactate and ammonia concentrations more than 20 mg/dl and 100 micrograms/dl, respectively, over baseline values. No significant correlations were found among age, sex, duration of exercise, or estimate of work performed and the maximum ammonia and lactate values achieved. When 70 patients with complaints of weakness, fatigue, or cramps were evaluated, the ischemic exercise test identified 5 patients

[Chinese expert consensus on the diagnosis and treatment of Becker muscular dystrophy].

Becker muscular dystrophy (BMD) is an X-linked recessive myopathy caused by pathogenic variants of the dystrophin-encoding DMD gene. The clinical presentation of BMD encompasses four clinical subtypes: limb-girdle muscle weakness, quadriceps myopathy, isolated cramp-pain syndrome, and asymptomatic hyper-creatine kinase-emia. Patients may also present with cardiopulmonary involvement, neuropsychological issues, joint contractures, and spinal deformities. Diagnosis primarily relies on genetic testing and/or muscle biopsies. The treatment and management of BMD requires comprehensive and multidisc

Spontaneous electrical activity: an overview.

Spontaneous electrical activity of muscle originates either from one, a few, or a large number of muscle fibers; the responses from the latter are similar to those of motor units. Responses from one or a few fibers are those arising in the end-plate zone of normal muscle and the electrical activity is associated with denervation and myotonia. The motor unit-like activity may either be discharged at randomly varying intervals, as in fasciculations, or at intervals similar to the discharges during voluntary effort, as in hemifacial spasm, Isaacs' syndrome, and in infantile spinal muscular atroph

Cramp-Fasciculation Syndrome Associated with Natural and Added Chemicals in Popular Food Items.

Cramp-fasciculation syndrome (CFS) is a rare and benign neuromuscular disorder that may initially masquerade as motor neuron disease/amyotrophic lateral sclerosis. While CFS may have a familial disposition, we report on cases associated with high consumption of popular food items. One set of patients reversibly experienced acute onset of headache, flushing, muscle stiffness and fasciculations following the consumption of umami-flavored food containing a large concentration of monosodium glutamate. A second group of patients consuming food derived from lupin seed developed acute cholinergic tox

[Motor Neuron Involvement in RFC1 CANVAS/Spectrum Disorders].

Cerebellar ataxia with neuropathy and vestibular areflexia syndrome (CANVAS) is characterized by the triad of cerebellar ataxia, bilateral vestibular impairment, and sensory neuropathy. The responsible anatomical region for the sensory disturbance in CANVAS is reportedly the dorsal root ganglion, which suggests neuronopathy rather than neuropathy as the pathomechanism of this peripheral nervous system disorder. Early on, motor neuron involvement was considered rare in CANVAS. The etiology of CANVAS includes the homozygous pentanucleotide repeat expansion within the RFC1 gene, resulting in dive

Phenotypic variation of Val1589Met mutation in a four-generation Chinese pedigree with mild paramyotonia congenitia: case report.

Four generations of a Chinese family with a mild form of paramyotonia congenital was characterized in phenotype and genotype. For each member, clinical history, physical examination, laboratory tests, electrophysiological and gene analyses were recorded and carried out. A potassium loading, exercise and cold provocation were further tested to diagnose the clinical differentiation. All members shared the characteristics of mild muscle cramp and stiffness induced by exercise or exposed to cold. The symptoms were relieved after rest and warming. A Val1589Met mutation at exon 24 of the SCN4A gene …

[Treatment of movement disorders using botulinum toxin].

Intramuscular injections of botulinum toxin (Botox) are followed by a dose-dependent focal paresis which can be used to treat several focal movement disorders. Botox injections are recommended as effective for the treatment of blepharospasm, hemifacial spasm, and cervical dystonia (torticollis). Focal dystonias elsewhere (for example, writer's cramp) can often be treated with similar success. Others, such as oromandibular dystonia, are more difficult to treat. In the case of more generalized dystonias, some focal muscle spasms can be treated with success by local intramuscular injections. New

A real-world evidence study of nabiximols in multiple sclerosis patients with resistant spasticity: Analysis in relation to the newly described 'spasticity-plus syndrome'.

BACKGROUND AND PURPOSE: Nabiximols is a therapeutic option for patients with multiple sclerosis (MS) spasticity whose symptoms are poorly controlled by conventional oral first-line medications. This study aimed to assess the relationship between changes in spasticity severity (measured on the 0-10 numeric rating scale [NRS]) and the presence of associated symptoms in patients treated with nabiximols, and to investigate the presence of the newly described 'spasticity-plus syndrome'. METHODS: We analyzed real-world data from the Italian Medicines Agency e-Registry on 1138 patients with MS spast
