Opening and closing mechanisms of the larynx.

Opening and closing of the larynx are determined by the intrinsic and extrinsic muscles acting on the elastic forces in the tongue, pharynx, larynx, and trachea. The pharynx is opened or closed by two mechanisms: (1) Contractions of the cricothyroid and of the intrinsic muscles of the larynx open and close the vocal cords. (2) The false cords, ventricle, and true cords accordion open or close in a bellows mechanism. We conclude that the posterior cricoarytenoid opens the laryngeal airway. The cricothyroid together with the posterior cricoarytenoid accentuates this opening. The larynx is also o

Masseter-facial neurorrhaphy for facial palsy reanimation: What happens after masseter denervation? Histomorphometric and stomatognathic functional analysis.

The aim of the present study was to analyse the consequences of masseter muscle denervation. In facial palsy surgical treatment, the masseteric nerve constitutes an important nerve source for facial reanimation due to its anatomical position and large amount of available axons. Computed tomography and/or magnetic resonance imaging were performed in 30 control subjects, and three radiologists separately measured the longitudinal diameter (LD), anteroposterior diameter (APD), transverse diameter (TD), and skeletal muscle area (SMA) of the masseter muscles as reference values. Regarding the facia…

Muscle histopathology in upper motor neuron-dominant amyotrophic lateral sclerosis.

The distinction between primary lateral sclerosis (PLS) and amyotrophic lateral sclerosis (ALS) still remains debated. Recently, PLS patient … nervation with still moderate reinnervation phenomena. This study suggests that in UMN-dominant ALS muscle denervation may be characterized by an early chronic impairment of a restricted number of LMNs. The extent rather than the presence of LMN signs may allow to categorize patients with motor neuron disease involving mainly UMN into distinct entities.

Effect of mitochondrial protein synthesis inhibitors on the trophic action of nerve stump in mice.

We studied the effect of mitochondrial protein synthesis inhibitors on the neurotrophic action of the nerve stump in mice. The sciatic nerve was cut as close to, or as far from the extensor digitorum longus muscles as possible. At 2 and 3 days after denervation, depolarization in the resting membrane potential of muscles with long nerve stumps (14-16 mm) was significantly smaller than in muscles with very short (less than 2 mm) nerve stumps. Chloramphenicol (100 mg/kg, p.o.), erythromycin (4 micrograms/kg, i.p.), ethidium bromide, (10 micrograms/kg, i.p.) or acridine (10 micrograms/kg, i.p.),

Sphincter denervation in anorectal incontinence and rectal prolapse.

Biopsies of the external anal sphincter, puborectalis, and levator ani muscles have been examined in 24 women and one man with long-standing anorectal incontinence, 18 of whom also had rectal prolapse, and in two men with rectal prolapse alone. In 16 of the women anorectal incontinence was of unknown cause, but in eight there was a history of difficult labour. Similar biopsies were examined in six control subjects. In all the incontinent patients there was histological evidence of denervation, which was most prominent in the external anal sphincter muscle biopsies, and least prominent in the l

Selective peripheral neurotomy (SPN) for spasticity in childhood.

OVERVIEW: Excess spasticity leads to disability that is marked by impaired locomotion, handicapping deformities and, if not controlled, discomfort and pain. Selective peripheral neurotomy in the child is indicated for severe focal spasticity, when botulinum toxin injections cannot delay surgery any longer. MATERIALS AND METHODS: Preoperative motor blocks mimicking the outcome of the surgical procedure are essential to establish the objectives of neurotomy. In the lower limb, obturator neurotomy is indicated for spasticity in the adductor muscles, hamstring neurotomy for the knee flexion and t

Abnormal signal intensity in skeletal muscle at MR imaging: patterns, pearls, and pitfalls.

Abnormal signal intensity within skeletal muscle is frequently encountered at magnetic resonance (MR) imaging. Potential causes are diverse, including traumatic, infectious, autoimmune, inflammatory, neoplastic, neurologic, and iatrogenic conditions. Alterations in muscle signal intensity seen in pathologic conditions usually fall into one of three recognizable patterns: muscle edema, fatty infiltration, and mass lesion. Muscle edema may be seen in polymyositis and dermatomyositis, mild injuries, infectious myositis, radiation therapy, subacute denervation, compartment syndrome, early myositis

[Electromyography in function disorders of the cricothyroid muscle following trauma and strumectomy].

32 patients with a disturbed function of the cricothyroid muscle were examined electromyographically and some also electroneurographically. Thyroid surgery had preceded in 22 patients and a traumatic lesion in 10 patients. Clinical observation showed that all patients suffered from an instability and lowering of the speaking voice as well as a reduction of vocal range. This was caused by a deficient tension of the vocal cords due to an insufficiently innervated cricothyroid muscle. In healthy subjects a continuous muscle activity of the cricothyroid muscle can be noted even in a resting positi

QUANTITATIVE HISTOLOGICAL ASSESSMENT OF SKELETAL MUSCLE HYPOTROPHY AFTER NEUROTOMY AND SCIATIC NERVE REPAIR IN RATS.

Reinnervation of skeletal muscles, wich occurs in time, is considered a factor in preventing muscle atrophy and potentially successful functional recovery. Morphometry of denervated muscles makes it possible to assess the dynamics of muscle atrophy after various methods of repairing of a damaged peripheral nerve. The aim - evaluate histological changes and morphometry of m. gastrocnemius in rats after complete neurotomy and nerve repair techniques. In rats the sciatic nerve was crossed and sutured with 4 epineural sutures, 2 sutures with DuraSeal, and 2 sutures with Tisseel. On the 14th, 30th,

Effect of carotid chemoreceptor denervation on breathing in utero and after birth.

The influence of the carotid bodies on fetal breathing activity in utero and on the establishment of postnatal breathing was studied. The electrocorticogram, the electromyogram of the lateral rectus and postural muscles, and tracheal and arterial pressures were monitored on chronically prepared intact and carotid sinus-denervated fetal lambs. The denervated fetuses required a longer time for recovery from the operation, but thereafter the sleeping and breathing behavior was similar to that of the control group. Four of the carotid sinus-denervated fetuses were allowed to be delivered spontaneo

Evidence for mast cells contributing to neuromuscular pathology in an inherited model of ALS.

Evidence indicates that neuroinflammation contributes to motor neuron degeneration in amyotrophic lateral sclerosis (ALS), a fatal neurodegenerative disease leading to progressive muscular paralysis. However, it remains elusive whether inflammatory cells can interact with degenerating distal motor axons, influencing the progressive denervation of neuromuscular junctions (NMJs). By analyzing the muscle extensor digitorum longus (EDL) following paralysis onset in the SOD1G93A rat model, we have observed a massive infiltration and degranulation of mast cells, starting after paralysis onset and co

Skeletal muscle reprogramming in peripheral nerve injury: mechanisms, therapeutic roles, and complication management.

Peripheral nerve injury (PNI) presents a significant clinical challenge, frequently leading to long-term neuromuscular dysfunction, muscle atrophy, fibrosis, and chronic pain. Traditional repair strategies, including microsurgical reconnection and neurotrophic support, often yield limited functional recovery, especially in cases of delayed or incomplete reinnervation. In this context, skeletal muscle reprogramming-defined as the intentional modulation of cellular fate, function, or metabolic state in muscle-resident cells-has emerged as a promising strategy to enhance regenerative outcomes. Denervation induces rapid bone degeneration concurrent to neurovascular and mechanical changes.

Denervation results in reduced bone quality, largely attributed to reduced loading due to concurrent muscle atrophy. However, sensory and sympathetic nerves also directly innervate bone, suggesting additional potential inputs into bone remodeling. A quantitative baseline of bone morphological and functional changes after nerve injury is lacking. We investigated structural, biomechanical, and histological outcomes for time points up to three months following peripheral nerve injury. Our primary objective was to establish timelines over which bone and muscle structure and biomechanical function

Muscle atrophy and regeneration associated with behavioural loss and recovery of function after sciatic nerve crush.

AIM: To resolve timing and coordination of denervation atrophy and the re-innervation recovery process to discern correlations indicative of common programs governing these processes. METHODS: Female Sprague-Dawley (SD) rats had a unilateral sciatic nerve crush. Based on longitudinal behavioural observations, the triceps surae muscle was analysed at different time points post-lesion. RESULTS: Crush results in a loss of muscle function and mass (-30%) followed by a recovery to almost pre-lesion status at 30 days post-crush (dpc). There was no loss of fibres nor any significant change in the n

Altered RNA splicing contributes to skeletal muscle pathology in Kennedy disease knock-in mice.

Here, we used a mouse model of Kennedy disease, a degenerative disorder caused by an expanded CAG repeat in the androgen receptor (AR) gene, to explore pathways leading to cellular dysfunction. We demonstrate that male mice containing a targeted Ar allele with 113 CAG repeats (AR113Q mice) exhibit hormone- and glutamine length-dependent missplicing of Clcn1 RNA in skeletal muscle. Changes in RNA splicing are associated with increased expression of the RNA-binding protein CUGBP1. Furthermore, we show that skeletal muscle denervation in the absence of a repeat expansion leads to increased CUGBP1…

Regulation of nicotinic acetylcholine receptor turnover by MuRF1 connects muscle activity to endo/lysosomal and atrophy pathways.

Muscle atrophy is a process of muscle wasting induced under a series of catabolic stress conditions, such as denervation, disuse, cancer cachexia, heart and renal failure, AIDS, and aging. Neuromuscular junctions (NMJs), the synapses between motor neurons and muscle fibers undergo major changes in atrophying muscles, ranging from mild morphological alterations to complete disintegration. In this study, we hypothesized that remodeling of NMJs and muscle atrophy could be linked together. To test this, we examined if a major atrophy-promoting E3 ubiquitin ligase, MuRF1, is involved in the mainten

[Electrophysiological investigation s in diagnosis and evaluation of ALS progress].

In diagnostics of ALS the electrophysiologic investigation should be able to evaluate two fundamental processes: the primary process of the loss of some motoneuros--denervation, and secondary process of reinnervation. The most important diagnostic methods include the quantitative electromyography (EMG) evaluating several parameters of the motor unit potential (MUP) and of the maximal effort pattern. The earliest features are the signs of spontaneous activity (denervation) and the elevation of MUP amplitude and area (reinnervation). Finding of spontaneous activity in the tongue muscles as well
