Rituximab Is Ineffective for Treatment of Fatigue in Primary Biliary Cholangitis: A Phase 2 Randomized Controlled Trial.

Primary biliary cholangitis (PBC) is a chronic cholestatic liver disease. Half of patients experience debilitating fatigue, which is currently untreatable. Previous studies have shown muscle bioenergetic abnormalities in PBC, including increased muscle acidosis with exercise linked to the antimitochondrial antibody (AMA) diagnostic of the disease, and reduced anaerobic threshold. In this study we addressed the hypothesis that fatigue in PBC is driven by muscle bioenergetic abnormality related to AMA, and that AMA reduction with B-cell depletion therapy will improve fatigue. In our single-cente

Research on rehabilitation robot control based on port-Hamiltonian systems and fatigue dissipation port compensation.

INTRODUCTION: Upper-limb rehabilitation robots have been demonstrated to effectively promote motor recovery in stroke patients. However, in active training modes, control instability may be induced by the nonlinear and time-varying characteristics of muscle fatigue, increasing the risks of physical human-robot interaction and ultimately limiting rehabilitation outcomes. METHODS: A novel control strategy within the port-Hamiltonian framework, incorporating a dynamic muscle fatigue model. Fatigue levels were assessed in real time using surface electromyography (sEMG) signals and mapped to dampi…

Muscular function in late polio and the role of exercise in post-polio patients.

Many post-polio individuals note new musculoskeletal and neuromuscular symptoms. In general, post-polio individuals are found to be weaker than non-postpolio individuals. Muscle weakness appears to play a role in functional limitations in post-polio individuals, especially for such activities as walking and stair climbing. Many post-polio individuals also have deficits in muscular work capacity and strength recovery following activity. Importantly, post-polio individuals are known to have normal perception of local muscle fatigue during activity. The perception of fatigue within the working mu…

Motor variability as an index of fatigue in dynamic actions: a perspective from the optimal movement variability theory.

Fatigue is a complex process that affects both force production and movement execution. Traditional measures, such as countermovement jump (CMJ) height, assess performance decrements but fail to capture compensatory movement adaptations. Nonlinear analysis of motor variability derived from acceleration signals provides a novel approach to monitoring fatigue by detecting subtle changes in movement execution. This study examined fatigue induced by three resistance training modalities-power, hypertrophy, and maximal strength-on motor variability during squats. Forty-four participants performed 10

Effect of mental fatigue on hand force production capacities.

Mental fatigue is common in society, but its effects on force production capacities remain unclear. This study aimed to investigate the impact of mental fatigue on maximal force production, rate of force development-scaling factor (RFD-SF), and force steadiness during handgrip contractions. Fourteen participants performed two randomized sessions, during which they either carried out a cognitively demanding task (i.e., a visual attention task) or a cognitively nondemanding task (i.e., documentary watching for 62 min). The mental fatigue was evaluated subjectively and objectively (performances a

Inhibition of carbonic anhydrases in type I muscle fibers influences contractility.

We tested the effects of inhibiting the carbonic anhydrase activity of rat soleus and extensor digitorum longus muscles on the isometric contractile properties and the resistance to fatigue. SOL and EDL muscles from female rats were incubated in vitro in the presence of methazolamide, a specific inhibitor of carbonic anhydrase, before determining their contractile properties. Methazolamide had no effects on the contractile properties of the soleus muscle (10(-5) or 10(-3) M) and extensor digitorum longus (10(-3) M), except for the half-relaxation time of the soleus muscle which increased signi

The effects of neuromuscular fatigue on task performance during repetitive goal-directed movements.

Proper movement timing is essential to the successful execution of many motor tasks and may be adversely affected by muscle fatigue. This study quantified how muscle fatigue affected task performance during a repetitive upper extremity task. A total of 14 healthy young adults pushed a low load back and forth along a low-friction horizontal track in time with a metronome until volitional exhaustion. Kinematic, force, and electromyography (EMG) data were measured continuously throughout the task. The first and last 3.5 min were analyzed to represent "early" and "late" fatigue. Means and standard…

Sex differences in the rate of fatigue development and recovery.

BACKGROUND: Many musculoskeletal injuries in the workplace have been attributed to the repetitive loading of muscle and soft tissues. It is not disputed that muscular fatigue is a risk factor for musculoskeletal injury, however the disparity between gender with respect to muscular fatigability and rate of recovery is not well understood. Current health and safety guidelines do not account for sex differences in fatiguability and may be predisposing one gender to greater risk. The purpose of this study was to quantify the sex differences in fatigue development and recovery rate of lower and upp

Effects of hypoxia on exercise-induced diaphragm fatigue in healthy males and females.

Following high-intensity, normoxic exercise there is evidence to show that healthy females, on average, exhibit less fatigue of the diaphragm relative to males. In the present study, we combined hypoxia with exercise to test the hypothesis that males and females would develop a similar degree of diaphragm fatigue following cycle exercise at the same relative exercise intensity. Healthy young participants (n = 10 male; n = 10 female) with a high aerobic capacity (120% predicted) performed two time-to-exhaustion (TTE; ~85% maximum) cycle tests on separate days breathing either a normoxic or hypo

Effect of theophylline on diaphragmatic and other skeletal muscle function.

Methylxanthines and particularly caffeine are known to increase skeletal muscle contractility. Recently, it has been shown that theophylline improves diaphragmatic contractility of the respiratory muscles both in isolated muscle preparations and in animals and normal human beings. Furthermore, it has been demonstrated that theophylline reverses diaphragmatic fatigue and prevents fatigue of the diaphragm when it is given prophylactically. Finally, recent evidence indicates that theophylline improves diaphragmatic function in patients with chronic obstructive pulmonary disease, all of whom retai

Diaphragmatic breathing exercises in recovery from fatigue-induced changes in spinal mobility and postural stability: a study protocol.

Prolonged periods of sitting at work can increase trunk muscle fatigue from the continuous contraction of deep trunk muscles. Insufficient activity of these muscles can decrease muscular support to the spine and increases stress on its passive structures. This can lead to reduced spinal mobility and impaired postural stability. It may also stimulate nociceptor activity leading to pain. However, frequently used recovery modalities such as muscle strengthening and stretching exercises, can be time-consuming, impractical, and difficult to implement in the workplace. Diaphragmatic breathing exerci…

Eight weeks of focal muscle vibration shows improvements in muscle co-ordination and co-contraction in cerebral palsy children: a randomized control trial.

BACKGROUND: Muscle spasticity, impaired coordination and fatigue are common in cerebral palsy (CP) children. This study investigated frequency-specific effects of focal muscle vibration (FMV) on neuromuscular activation, fatigue, and agonist-antagonist coordination. We hypothesized that 8 weeks of FMV would improve lower limb muscle activation, reduce fatigue, and optimize co-contraction compared to control, with differential effects between 80 and 100 Hz stimulation. METHODS: In a double-blind randomized controlled trial, 65 spastic diplegic CP children (GMFCS II-IV) were allocated to Contro

Inspiratory muscle weakness further impairs exercise capacity and respiratory functions and increases dyspnea perception in patients with heart failure.

Thirty to fifty percent of patients with heart failure (HF) exhibit inspiratory muscle weakness (IMW). However, it is not adequately known whether IMW further impairs exercise capacity, pulmonary function (PF), lower and upper extremity muscle strength, perception of dyspnea and fatigue, physical activity (PA) level, and quality of life (QoL) in HF patients. The study aimed to compare the abovementioned outcomes in HF patients with IMW, preserved inspiratory muscle strength (PIMS), and healthy controls (HC). In this retrospective, cross-sectional study, 41 HF patients with IMW, 30 with PIMS, a
