Exploring the role of circulating glial fibrillary acidic protein and neurofilament light chain in myasthenia gravis.

BACKGROUND: Myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular junction caused by antibodies against the acetylcholine receptor (AChR) or muscle-specific kinase (MuSK). Although these antibodies enable diagnosis, they correlate poorly with disease activity and prognosis. Emerging data suggest that MG involves broader neuroimmune mechanisms beyond the peripheral synapse. Glial fibrillary acidic protein (GFAP), a marker of astroglial activation, and neurofilament light chain (NfL), a marker of neuroaxonal injury, have been proposed as circulating biomarkers in several neurolog…

Predictive Factors for Non-response to First Use of Polyvalent Intravenous Immunoglobulin in Generalised Myasthenia Gravis.

INTRODUCTION: Myasthenia gravis (MG) is an autoimmune neurological disorder affecting the neuromuscular junction. Rapid clinical deterioration during a myasthenic crisis (MC) or severe exacerbation may be life-threatening due to respiratory or bulbar involvement. Plasma exchange (PLEX) and intravenous immunoglobulin (IVIg) are two rescue therapies that show comparable efficacy in this setting. However, the characteristics of patients who respond poorly to IVIg remain insufficiently described. The aim of this study was to identify predictive factors of non-response to IVIg administered during a…

Robot-Assisted Versus Video-Assisted Thymectomy: A Narrative Review of Current Evidence.

OBJECTIVES: Minimally invasive thymectomy has transformed the surgical management of thymic epithelial tumours and myasthenia gravis (MG). Video-assisted thoracoscopic surgery (VATS) is well established, whereas robot-assisted thoracoscopic surgery (RATS) provides technical and ergonomic advantages but at a higher cost. We reviewed current evidence to clarify the indications, oncological validity, perioperative performance, and economic implications of RATS compared with VATS and open thymectomy. METHODS: A structured narrative review was conducted, drawing on multi-institutional cohorts, reg…

Prevalence and characteristics of patients with seronegative myasthenia gravis.

Myasthenia gravis is an autoimmune disorder characterized by antibodies targeting the neuromuscular junction, causing fatigue of skeletal muscle. Natural history and epidemiological studies report that around 10% of patients diagnosed with myasthenia gravis do not have detectable levels of antibodies, categorized as seronegative. Theories of the pathophysiology in these patients are predominantly centered around undetectable antibody titers and antibodies against yet undefined antigens in the neuromuscular junction. This study aims to investigate the regional prevalence and characteristics of …

Effect of CXCR5, PD-1 and ICOS on B-cell responses and relevance to myasthenia gravis.

OBJECTIVES: T follicular helper (Tfh) cells regulate B-cell responses within germinal centres primarily via programmed cell death protein 1 (PD-1) and inducible T-cell costimulator (ICOS), whereas CXC chemokine receptor type 5 (CXCR5)-negative T peripheral helper (Tph) cells provide similar support extra-follicularly. Both subsets contribute to the pathogenesis of acetylcholine receptor-antibody-positive myasthenia gravis (AChR-MG). This study investigated the roles of PD-1, ICOS and CXCR5 in T-cell-mediated B-cell activation to identify therapeutic targets for MG. METHODS: CD4 T cells from 1…

Relative frequencies of muscle specific kinase antibody myasthenia in 46 centres worldwide.

Serum antibodies to muscle specific kinase (MuSK) are present in a proportion of patients with acetylcholine receptor antibody seronegative myasthenia gravis (SNMG), but their reported frequencies in different populations vary. From 2002, serum samples were sent to Oxford for MuSK antibody testing from 35 centres in 6 continents. MuSK antibodies were identified in 143/465 (30.8%) previously untested SNMG sera. They were not identified in 147 adult acetylcholine receptor antibody positive patients or in 63 adult ocular MG patients. As expected from subsequent reports, the MuSK antibody positive…
