Mycobacterium tuberculosis (Mtb), a highly infectious intracellular pathogen, is the primary causative agent of tuberculosis (TB), which ranks as the leading cause of death from individual infectious agents. In this context, we developed a model using macrophages infected with M. smegmatis mc2155 and Mtb H37Ra to elucidate the role of Mn2+ in host defense against tuberculosis infection. Treatment with a combination of anti-tuberculosis drugs and Mn2+ for 36 h markedly reduced the survival of intracellular bacteria, and the addition of Mn2+ significantly enhanced the secretion of TNF-α and IL-1…

Tuberculosis (TB) remains endemic in Oman, with incidence exceeding 5 per 10⁵ population since 2016 and a rising proportion of cases among expatriates (> 40% of the population). This study investigated the spatiotemporal diversity and structure of Mycobacterium tuberculosis (MTB) in Oman between 2008 and 2022. A total of 735 MTB isolates from nationals and expatriates across provinces were genotyped by spoligotyping and 17-locus MIRU-VNTR; 96 underwent whole-genome sequencing (WGS). Genetic diversity (expected heterozygosity, h) and differentiation (FST) were estimated, and phylogenetic analys…

Tuberculosis (TB) is a highly infectious disease caused by Mycobacterium tuberculosis. The World Health Organization (WHO) has reported more than 8 million new or relapse TB cases and 1.25 million deaths in 2023. Even though the global End TB strategy implemented by WHO has reduced the TB incidence rate between the years 2015 and 2023 up to 8.3%, TB remains one of the leading infectious causes of death worldwide. Active tuberculosis can affect any part of the body, and the clinical symptoms include fever, weight loss, and night sweats, whereas latent TB does not show any clinical symptoms. One…

BACKGROUND: Several discoveries about leprosy indicate that Mycobacterium leprae transmission mainly occurs by inhalation, and the nose is a major port of entry and exit. Molecular probes have shown certain potential for the detection and identification of M. leprae in patients. The aim of this study was to identify M. leprae in nasal swab specimens using polymerase chain reaction (PCR)-based assays followed by gene sequencing methods. This observational study examines 64 anterior nasal swab samples taken from pretreatment leprosy patients, on-treatment and completed leprosy treatment in Buluk…

Mycobacterium tuberculosis (Mtb) is a leading cause of death, with an escalating global occurrence of drug-resistant infections that are partially attributed to cell wall mycolic acids derived from type II fatty acid biosynthesis (FAS-II). Here, the central acyl carrier protein, AcpM, contributes to the regulation of complex and specific protein-protein interactions (PPIs), though the orchestration of these events remain largely unresolved due to unique features of AcpM. Limitations include complexities in generating modified AcpM in a single state. Herein, we report a streamlined method to ge…

Mycobacterium kansasii infection has been reported to be about 20 percent of non-tuberculous mycobacteriosis, and its disseminated type is uncommon and the prognosis is reported to be generally poor. We experienced one case of disseminated Mycobacterium kansasii infection. A 81 year-old man who had been short-bowel syndrome due to the operation for superior mesenteric artery occlusion since 1998 was admitted on April 24th, 2001 to our hospital because of slowly progressive consciousness disturbance and anorexia. He had shown progressive productive cough and respiratory failure and laboratory f…

Non-tuberculous mycobacterium is a prevalent pathogen that-despite myriad systemic presentations, particularly among immunocompromised hosts-rarely presents in the periocular and adnexal region. The authors present here the first reported case of pyomyositis caused by Mycobacterium avium complex in an immunocompetent patient presenting with progressive, insidious proptosis, found to have a "cold abscess" in the inferior rectus warranting surgical drainage and prolonged medical treatment. In addition to its novelty, this case reviews pyomyositis of the extraocular muscles and NTM infections in …

Heterogeneity during Mycobacterium tuberculosis (Mtb) infection greatly impacts disease outcome and complicates treatment. This heterogeneity encompasses many facets, spanning local differences in the host immune response to Mtb and the environment experienced by the bacterium, to nonuniformity in Mtb replication state. All of these facets are interlinked and each can affect Mtb susceptibility to antibiotic treatment. In-depth spatiotemporal understanding of Mtb-host interactions is thus critical to both fundamental comprehension of Mtb infection biology and for the development of effective th…

Mycobacterium haemophilum is a nontuberculous mycobacteria that primarily affects immunocompromised patients. It can lead to a wide variety of clinical manifestations including infections of the skin, soft tissue and joints. Due to the significant heterogeneity in clinical presentation and difficulty isolating the organism, diagnosis can be difficult and is often delayed. Our patient's course was further complicated by the development of haemophagocytic lymphohistiocytosis (HLH). Although M. tuberculosis infection is recognized as a potential association, HLH driven by a disseminated M. haemop…

MmpL3 is a promising new target for antitubercular drugs, but the microbiological properties of MmpL3 inhibitors are not fully understood. We compared the activity and mode of action of 11 structurally diverse compound series that target MmpL3. We confirmed the activity was via MmpL3 using strains with differential expression of MmpL3. MmpL3 inhibitors had potent activity against replicating M. tuberculosis, with increased activity against intramacrophage bacilli and were rapidly bactericidal. MmpL3 inhibition induced cell wall stress concomitantly with a boost in the ATP levels in M. tubercul

Mycobacterium marinum serves as an ideal model organism for studying tuberculosis due to its genetic similarity to Mycobacterium tuberculosis. However, there is a need for more suitable animal models to study M. marinum infections. In this study, we established a novel infection model using red-eared slider turtles (Trachemys scripta elegans). The turtles were infected with M. marinum via subcutaneous injection in the hind limb. Inoculation with >106 CFU of M. marinum resulted in acute infection, causing mortality in at least 80% of turtles within five weeks, whereas 105 CFU caused only 10% mo…

Nontuberculous mycobacteria (NTM) are opportunistic pathogens that cause pulmonary disease (PD) in people with bronchiectasis and other chronic airways diseases. Difficulty treating and eradicating NTM-PD highlights the need for improved understanding of bacterial mechanisms to establish chronic infections. People with the genetic disorder cystic fibrosis (CF) develop bronchiectasis and are the population at highest risk of NTM-PD, caused mainly by Mycobacterium avium or Mycobacterium abscessus (Mabsc). The majority of people with CF (pwCF) and bronchiectasis develop chronic Pseudomonas aerugi…

Tuberculosis of the cerebral nervous system is a diagnostic and therapeutic emergency that can lead to death and sometimes disabling sequelae. It can be complicated by stroke in the context of vasculitis. We report a pediatric case. We report the case of a 13-year-old child who presented with a sudden onset of altered consciousness in the context of fever. He had been experiencing unexamined cervical lymphadenopathy and a cough for a year. On day 4 of hospitalisation, he developed a right pyramidal syndrome. Cerebral magnetic resonance imaging revealed a deep middle cerebral artery infarction.

Paradoxical reactions occur when an infection has acute worsening in response to antibiotic therapy. Here, we describe a patient with chronic cutaneous ulcerative lymphangitis that acutely worsened following initiation of antibiotic therapy. The infection was caused by Mycobacterium marinum, a species which has not previously been associated with paradoxical reaction in immunocompetent persons. In this case report, we describe our patient's diagnosis and management, review the management of Mycobacterium marinum infection, and discuss paradoxical reactions in mycobacterial disease.

Mycobacterium chelonae (M. chelonae) is a member of the rapidly growing non-tuberous mycobacteria and can cause disseminated tissue infection, particularly, in the limbs. We reviewed medical records of two kidney transplant patients. We describe their background disease and transplantation details, with the use of immunosuppressive medication. We also discuss the presentation of M. chelonae infection and treatment. Both patients received deceased brain-dead donor kidney transplants for end-stage kidney disease. Both developed cutaneous manifestations of M. chelonae, progressing to disseminated…

BACKGROUND: The low rate of nontuberculous mycobacteria (NTM) among Brazilian patients with cystic fibrosis (CF) may be due to cross-reactive Bacille Calmette-Guérin (BCG) vaccination. In the present pilot study, we aimed to compare the lymphocyte responses against Mycobacterium tuberculosis(Mtb) and Mycobacterium bovis (BCG) in BCG-vaccinated CF patients and healthy controls.
METHODS: The lymphocyte responses of CF patients (n = 10) and healthy controls (n = 10) were assessed in terms of lymphocyte proliferation index (LPI), using flow cytometry. Median rates of each cell subtype - CD4, CD8,…
