Diastolic heart dysfunction is correlated with CTG repeat length in myotonic dystrophy type 1.

The aims of this study were to investigate the correlations of tri-nucleotide (CTG) repeat length with detailed echocardiography (ECHO) parameters that represent myocardial function and to find a relationship between heart function and CTG repeat length in adult-onset myotonic dystrophy type 1 (DM1). In this study, clinical data for patients with DM1, including age, onset age, CTG repeat length, Medical Research Council sum score (MRCSS), and 6-min walking test (6MWT), were recorded. In addition, ECHO parameters and cardiac conduction abnormalities were evaluated. Among the cardiac parameters,…

Short antisense-locked nucleic acids (all-LNAs) correct alternative splicing abnormalities in myotonic dystrophy.

Myotonic dystrophy type 1 (DM1) is an autosomal dominant multisystemic disorder caused by expansion of CTG triplet repeats in 3'-untranslated region of DMPK gene. The pathomechanism of DM1 is driven by accumulation of toxic transcripts containing expanded CUG repeats (CUG(exp)) in nuclear foci which sequester several factors regulating RNA metabolism, such as Muscleblind-like proteins (MBNLs). In this work, we utilized very short chemically modified antisense oligonucleotides composed exclusively of locked nucleic acids (all-LNAs) complementary to CUG repeats, as potential therapeutic agents a…

Brugada-like cardiac disease in myotonic dystrophy type 2: report of two unrelated patients.

BACKGROUND: Myotonic dystrophy type 2 (DM2) is an autosomal dominant multisystem disorder caused by CCTG repeat expansions within intron 1 of the ZNF9 gene on chromosome 3q. Cardiac conduction disturbances, supraventricular arrhythmias, and cardiomyopathy are described in DM2 but Brugada-like features have not yet been reported. Brugada syndrome (BS) is a genetically heterogeneous cardiac conduction disorder which is characterized by a significant ST-segment elevation upon ECG evaluations and bears an increased risk for sudden cardiac death. CASE REPORTS: We report two unrelated patients with…

Cardiac surgery in type-1-myotonic muscular dystrophy (Steinert syndrome) associated to Barlow disease.

No data exist in the English-language literature about patients with Barlow disease associated to Steinert syndrome and little is known about the employment of hypothermic cardiopulmonary bypass (CPB) and hyperkalemic cardioplegia in these patients. We present our experience with six patients affected by myxomatous degeneration associated to Steinert disease undergoing complex mitral valve repair. In all patients we employed mild hypothermic CPB (31 degrees C) and myocardial protection was achieved, in the entire cohort, by the use of blood hyperkalemic cold cardioplegia. The postoperative cou

Myotonic dystrophy transgenic mice exhibit pathologic abnormalities in diaphragm neuromuscular junctions and phrenic nerves.

Myotonic dystrophy Type 1 (DM-1) is caused by abnormal expansion of a (CTG) repeat located in the DM protein kinase gene. Respiratory problems have long been recognized to be a major feature of this disorder. Because respiratory failure can be associated with dysfunction of phrenic nerves and diaphragm muscle, we examined the diaphragm and respiratory neural network in transgenic mice carrying the human genomic DM-1 region with expanded repeats of more than 300 CTG, a valid model of the human disease. Morphologic and morphometric analyses revealed distal denervation of diaphragm neuromuscular …

Myotonic dystrophy: does it affect ovarian follicular status and responsiveness to controlled ovarian stimulation?

BACKGROUND: Myotonic dystrophy (MD) is characterized by myotonia, multisystemic lesions and hypogonadism. In women, the relationship between MD and infertility remains controversial. This study investigated the ovarian status and response to controlled ovarian stimulation (COS) in MD women entering our preimplantation genetic diagnosis programme. METHODS: We elected to compare MD patients with X-linked disorders (XLD) carriers, given that XLD have not been shown to affect ovarian status. On the one hand, we analysed all the cycles performed and, on the other hand, we conducted a subanalysis b…

[Intracellular signal transduction of endocrine organs and expanded DNA fragment size in myotonic dystrophy].

The Ellsworth-Howard (EH) test was performed in 16 patients with myotonic dystrophy (DM), who were divided into two groups according to serum calcium level; Group I showing normal serum calcium (8 patients) and Group II with hypocalcemia (8 patients). Patients in Group II were recognized as having pseudohypoparathyroidism (PHP) type II and those in Group I as normal. Therefore, it was suggested that an abnormality of A kinase might be present in Group II patients. We additionally performed the thyrotropin releasing hormone (TRH) tolerance test in the same patients and 7 normal controls to exam…

The genetic landscape of myotonic dystrophy type 1: insights from nearly one million genomes.

Myotonic Dystrophy type 1 (DM1) is a multisystem disorder caused by CTG repeat expansions in the dystrophia myotonica protein kinase (DMPK) gene. Current prevalence estimates of 1 in 3,000-8,000 have traditionally relied on clinically ascertained and geographically restricted cohorts, underestimating population burden and failing to capture ancestry diversity. Using large-scale whole-genome sequencing (WGS) datasets, we aimed to identify DMPK repeat expansions across nearly one million participants, define their ancestry-specific distribution, and evaluate associated clinical phenotypes, with …

Modeling myotonic dystrophy type 1 with hiPSCs-derived cardiac organoids reveals key disease mechanisms.

BACKGROUND: Myotonic dystrophy type 1 (DM1) is a multisystemic disorder characterized by conduction defects and arrhythmias that contribute significantly to cardiac morbidity and sudden death. Human-induced pluripotent stem cell (hiPSC)-derived cardiomyocytes provide a powerful platform to model disease-specific cardiac dysfunction; however, immature 2-dimensional cultures may not fully capture late-stage pathological mechanisms. OBJECTIVE: This study aimed to investigate the temporal evolution of cardiac dysfunction in DM1 using developmentally mature 3-dimensional cardioids derived from pat…

Shared decision-making about prolonged invasive ventilation for adults with myotonic dystrophy type 1.

The option of tracheostomy and prolonged invasive mechanical ventilation (IMV) for chronic respiratory failure in myotonic dystrophy type 1 (DM1) poses challenges in decision-making. The Myotonic Dystrophy Foundation consensus recommendations on respiratory management do not directly address the implications of this intervention given the progressive physical and cognitive manifestations of DM1. Here, we consider IMV in DM1 using the ethical principles of beneficence, non-maleficence, autonomy and justice. We call for further research on post-tracheostomy morbidity, mortality, and quality of l…

Differential pathology and susceptibility to MBNL loss across muscles in myotonic dystrophy mouse models.

There are 2 subtypes of myotonic dystrophy, DM1 and DM2, each caused by repeat expansion mutations. The leading pathogenic mechanism is RNA-mediated toxicity, whereby (C)CUG expansions sequester the muscleblind-like (MBNL) family of RNA binding proteins. However, key differences exist in muscle involvement patterns and histopathology between DM1 and DM2. The cause of these disparities both in how the muscles are affected within each disease and between the 2 diseases is unknown, and it is unclear if current DM mouse models recapitulate these differences or develop differential muscle susceptib…
