Foetal echocardiographic assessment of borderline small left ventricles can predict the need for postnatal intervention.

BACKGROUND: We sought to prospectively determine foetal echocardiographic factors associated with neonatal interventions in borderline hypoplastic left ventricles. METHODS: Foetuses were included who had a left ventricle that was 2-4 standard deviations below normal for length or diameter and had forward flow across the mitral and aortic valves. Factors associated with an intervention in the first month of life or no need for intervention were sought using univariate and multivariate logistic regression models. RESULTS: From 2005 to 2008, 47 foetuses meeting the criteria had an additional di

[The eastern Paris perinatal network (2008-2010). Defining the target population and assessing health needs].

BACKGROUND: Since the 1990s, governmental plans in France have imposed the regionalization of perinatal care to improve both safety and quality of care. The Eastern Paris Perinatal Network is under construction in health area 75-2, which includes the 11th, 12th, 13th and 20th arrondissements of Paris. A major issue is ensuring that the network can meet the needs of its target population. The objective is to define the network's target population of mothers and newborns. METHODS: We designed a matrix to help assess these needs and identify the data required to define the network's population.

Assessing the supportive care needs of parents with a child with a bleeding disorder using the Parental Needs Scale for Rare Diseases (PNS-RD): A single-centre pilot study.

INTRODUCTION: Inherited bleeding disorders are rare, for some such as haemophilia, there is a growing body of evidence about caregiver burden and associated support needs. Whilst for other disorders, which may be present with less clinically significant bleeding, there is a paucity of data about parents support needs. AIMS: To evaluate the supportive care needs of parents of children with a range of inherited bleeding disorders. METHODS: A single-centre pilot study using the validated Parental Needs Scale for Rare Diseases to measure the overall support needs of parents of children with a bl

Systematic Methods to Assess End-of-Life and Palliative Care Needs for Heart Failure Home Care.

Face, head, and neck (FHN) problems and surgical need assessment among refugees living in Tanzania.

Otolaryngology-head and neck surgery (OHNS) conditions comprise a considerable and underprioritized portion of the global burden of surgical disease. While OHNS conditions have been found to disproportionately impact low- and middle-income countries (LMICs), no studies to date have sought to characterize the burden of OHNS disease within chronic refugee settings in LMICs. The cluster-randomized, cross-sectional Surgeons Overseas Assessment of Surgical Need (SOSAS) survey tool was administered in Nyarugusu Refugee Camp in Makere, Tanzania in August and September of 2021 to estimate the prevalen

Digital rectal examination in rectal cancer response assessment: clinical performance and the need for standardized reporting.

BACKGROUND: Accurate assessment of treatment response after neoadjuvant therapy is crucial to select patients with rectal cancer for organ-preserving strategies. Digital rectal examination (DRE) remains a key component of response assessment, but its precise clinical role and the lack of standardized reporting limit its utility. This study aimed to assess variability in DRE reporting and to explore the association between DRE findings at first reassessment and subsequent local oncological outcome within a watch-and-wait pathway, rather than to evaluate DRE as a stand-alone diagnostic test. This study aimed to analyze the needs of individuals over 75 years of age living in a home environment. Methods: A cross-sectional study assessed support needs from the perspectives of the patient, caregiver, and researcher. The following research tools were used: Camberwell Assessment of Need for the Elderly (CANE), Mini-Mental State Examination (MMSE), Barthel Index, and Geriatric Depression Scale (GDS). Results: The ave

Synergizing Needs Assessments and Patient Preference Studies for Enhanced Patient-Centered Decision Making in Healthcare.

OBJECTIVES: Over the past decades, criticism has grown regarding the supply-driven approach of medicinal product development. In response, patient-centered methods have been developed to inform decision making. This perspective article aims to reflect on two of these methods, which are needs assessments and patient preference studies (PPSs). METHODS: A reflection was conducted based on existing literature and our collaborative experiences, proposing a conceptual framework that synergistically combines needs assessments and PPSs. RESULTS: Needs assessments identify unmet health-related needs …

A population health approach to assessing for and addressing palliative care needs in outpatients with advanced gynecologic malignancies.

OBJECTIVE: Describe a pilot system that 1) proactively identifies palliative care (PC) needs in a gynecology oncology population and 2) efficiently connects patients with targeted primary and specialty PC services. METHODS: Outpatients with stage 3 or 4 gynecologic malignancies received electronic surveys assessing PC needs in domains of physical symptoms, emotional or spiritual distress, care coordination, practical concerns, and advance care planning (ACP). Referrals to support groups, social work (SW), ACP websites/workshops, and specialty PC consultation were offered accordingly. RESULTS

Clinical Outcome Assessments in Parkinson's Disease: A Scoping Review of Current Rating Scales and Future Needs.

BACKGROUND: Clinical outcome assessments (COAs) are essential for evaluating symptom severity, treatment response, and disease progression in Parkinson's disease (PD). As clinical knowledge evolves, it is necessary to revisit the recommendation status on the COAs to ensure their continued relevance and validity. OBJECTIVES: To provide an updated overview of existing MDS-recommended COAs in PD and to identify specific constructs requiring revision or further validation to inform current clinical practice and research. METHODS: The Scientific Evaluation Committee of the Movement Disorder Socie

From the need to the knowledge. Feeding emotions and thoughts: Assessing emotion regulation strategies in food tribes.

Some people choose to follow a certain dietary habit because they are guided by strong beliefs, but also to pursue a healthy lifestyle. Food tribes are defined as social groups sharing common values and beliefs that underlie food and lifestyle choices. The current study aimed to investigate potential differences in emotion regulation between specific food tribes, and when comparing food tribes to people who conversely do not follow a dietary habit. Specifically, we examined the presence of rumination, psychological inflexibility, and emotional contagion as components of emotion dysregulation.

Lipoprofiling Assessed by NMR Spectroscopy in Patients with Acute Coronary Syndromes: Is There a Need for Fasting Prior to Sampling?

Most patients presenting in an emergency unit with acute coronary syndromes (ACS) (which include non-ST-elevation myocardial infarction (NSTEMI), ST-elevation MI (STEMI), and unstable angina) usually meet at least two cardiovascular risk factors, such as dyslipidemia, arterial hypertension, diabetes mellitus type 2, obesity, history of or current smoking, etc. Most ACS patients suffer from a type of dyslipidemia, and in addition to this there are ACS patients rushed into the emergency units for which the feeding status is unknown. Thus, we set out to evaluate the effect of fasting on 16 blood

Is an "Outpatient Arthroplasty Risk Assessment Score" Needed for Predicting Safe Selection of Outpatient Arthroplasty Candidates?

BACKGROUND: Total joint arthroplasty is rapidly shifting to the outpatient space. One of the challenges of same-day discharge adoption has been determining which patients are suitable candidates. Risk assessment tools have been developed, including the Outpatient Arthroplasty Risk Assessment (OARA) score. The purpose of this study was to assess its predictive utility. METHODS: A retrospective review was performed on all total joint arthroplasties performed at a single ambulatory surgery center in 2018, yielding a cohort of 1,105 patients (1,332 arthroplasties). The institution's outpatient cr
