Altered somatosensory processing in trigeminal neuralgia.

Trigeminal neuralgia (TN) is a pain state characterized by intermittent unilateral pain attacks in one or several facial areas innervated by the trigeminal nerve. The somatosensory cortex is heavily involved in the perception of sensory features of pain, but it is also the primary target for thalamic input of nonpainful somatosensory information. Thus, pain and somatosensory processing are accomplished in overlapping cortical structures raising the question whether pain states are associated with alteration of somatosensory function itself. To test this hypothesis, we used functional magnetic …

[Vagoglossopharyngeal neuralgia treated with vascular decompression].

This case report describes a 63-year-old man referred with right-sided glossopharyngeal neuralgia. Magnetic resonance imaging (MRI) demonstrated asymmetry of the PICA vessels. A microvascular decompression was performed and complete remission of symptoms followed. After 14 months the neuralgia recurred, this time accompanied by sinus bradycardia/sinus arrest during attacks causing discomfort and syncopes. An MRI now demonstrated an arterial loop at the site of the vagoglossopharyngeal complex leaving the brainstem. After another microvascular decompression, complete remission of symptoms was o…

Herpes zoster (shingles) and postherpetic neuralgia.

Herpes zoster (HZ), commonly called shingles, is a distinctive syndrome caused by reactivation of varicella zoster virus (VZV). This reactivation occurs when immunity to VZV declines because of aging or immunosuppression. Herpes zoster can occur at any age but most commonly affects the elderly population. Postherpetic neuralgia (PHN), defined as pain persisting more than 3 months after the rash has healed, is a debilitating and difficult to manage consequence of HZ. The diagnosis of HZ is usually made clinically on the basis of the characteristic appearance of the rash. Early recognition and t…

Antiviral treatment for preventing postherpetic neuralgia.

BACKGROUND: Postherpetic neuralgia (PHN) is a painful and refractory complication of herpes zoster. Treatments are either partially or totally ineffective for many people with PHN. Antiviral agents, used at the time of the rash, are one of the best-established approaches that may prevent the development of PHN. OBJECTIVES: To investigate the effectiveness of antiviral agents in preventing PHN. SEARCH STRATEGY: We searched the Cochrane Neuromuscular Disease Group Trials Register (January 13 2009), the Cochrane Central Register of Controlled Trials (The Cochrane Library 2008, Issue 4), MEDLINE…

Managing herpes zoster and postherpetic neuralgia.

Treatment strategies for herpes zoster infection include limiting viral replication with anti-infective agents as well as limiting associated acute and chronic neuropathic pain with a variety of analgesics. The author outlines currently available pharmacotherapeutic options, from first-line (eg, anti-infective agents, tricyclic antidepressants, anticonvulsants, opioid analgesics, and topical agents) through adjuvant treatments (eg, oral corticosteroids). A summary of adverse event profiles is provided for each medication. Vaccination is recommended as a preventive measure.

Craniocervical junction schwannoma mimicking occipital neuralgia.

Occipital neuralgia is a very well recognized cause of posterior headache. Although hypoesthesia may be found in the individual occipital nerve's territory, the remaining neurologic exam is typically normal. An abnormal neurologic exam is an alert for potential underlying causes of symptoms.

Percutaneous microballoon compression for trigeminal neuralgia.

BACKGROUND: Percutaneous microballoon compression (PMC) for trigeminal neuralgia is an important therapeutic method. The aim of this study was to review the effects of PMC for trigeminal neuralgia in 276 patients. METHODS: From December 2000 to May 2003, 276 patients with trigeminal neuralgia were treated with PMC. The course of the disease ranged from 3 months to 38 years. Under the guidance of C-arm X-ray, 14# needle was placed into the foramen ovale using the classical Hakanson's technique. Fogarty balloon catheter was navigated into the Meckel's cave tenderly. A small amount of Omnipaque …

Trigeminal neuralgia: a modern-day review.

OBJECTIVE: To review the disorder of trigeminal neuralgia (TN) in the context of previously known data and recent advancements. METHODS: A PubMed search (November 25, 2006) was conducted, with review of existing literature related to this search. Review articles were accessed along with new advancements in diagnosis, pathophysiology, treatment, and prognosis. RESULTS: The diagnosis of TN is primarily based on clinical suspicion; ancillary tests can help rule out other associated pathology. Treatment includes medical and surgical treatment as previously described; recent radiosurgical treatme…

Nervus intermedius neuralgia: a case report.

Nervus intermedius neuralgia (NIN) is an uncommon disorder that affects a sensory branch of the facial nerve. This condition usually provokes a very intense and stabbing pain localized in the depth of the ear canal. Due to the close anatomical proximity, temporomandibular joint (TMJ) pathologies should be included in the differential diagnosis. The treatment of NIN has not been established, although it seems reasonable that the therapeutic approaches used in other more common craniofacial neuralgias, such as trigeminal neuralgia, should be effective. In this paper, the authors present a case r…

Subcutaneous sumatriptan for refractory trigeminal neuralgia.

BACKGROUND: Mechanical compression of the trigeminal root by an artery is thought to cause trigeminal neuralgia. The 5-HT(1B/1D) receptor agonist may inhibit neurogenic inflammation and vasodilation near the irritated trigeminal root. OBJECTIVE: To examine the effectiveness of a 5-HT(1B/1D) receptor agonist, sumatriptan, on a paroxysmal pain in trigeminal neuralgia. METHODS: Twenty-four patients with trigeminal neuralgia refractory to previous treatment were randomized to receive subcutaneously either 3 mg (1 mL) of sumatriptan or 1 mL of saline placebo. Following a 7-day period, patients cr…

Topiramate therapy for symptomatic trigeminal neuralgia.

OBJECTIVE: To evaluate patients' experience after flexible cystoscopy (FC), particularly concentrating on the prevalence and degree of symptoms, the frequency of visits to their General Practitioner (GP), subsequent antibiotic rates and the actual incidence of urinary tract infection (UTI). PATIENTS AND METHODS: Consecutive patients (420) presenting for FC were audited prospectively. A pain score for the procedure was recorded immediately afterward (linear scale 0-10) and a self-administered questionnaire completed at 7 days, to assess the objective and subjective symptoms and their duration,

Repeat radiosurgery for idiopathic trigeminal neuralgia.

PURPOSE: Although frequently performed, the efficacy and safety of repeat trigeminal neuralgia radiosurgery is not well described. METHODS AND MATERIALS: Between August 1997 and December 2002, 19 patients (9 men, 10 women) underwent repeat trigeminal neuralgia radiosurgery. The median interval between procedures was 16 months. The median dose (based on an output factor of 0.87 for the 4-mm collimator) for repeat radiosurgery was 76.1 Gy; the median additive dose was 163.1 Gy. Outcomes were defined as excellent (no pain, no medications), good (no pain, reduced medications), fair (>50% pain red…
