[Gilles de la Tourette's syndrome: a particular clinical form of tics? A case report].

Although it is a syndrome with sometimes spectacular symptoms, Gilles de la Tourette's syndrome still keeps, more than a century after its first description, many mysteries. Progress in neurobiology has led to various new findings-our case report is an opportunity to review the recent progress in clinical knowledge.

Shoot first, draw later peroneal nerve palsy: a sonographic study.

A 32-year-old man had an unusual gunshot wound to the leg, causing a peroneal nerve palsy. Sonography provided useful complementary findings to the electrodiagnostic localization of the injury.

Septuagenarian Sydenham's with secondary hypomania.

A 74-year-old man with a remote history of Sydenham's chorea but no personal or family history of mood disorder had simultaneous onset of severe, generalized chorea and a mild manic syndrome in clear consciousness. Both chorea and hypomania improved after treatment with sodium valproate and clonazepam. Agitated delirium or mood lability have been described in Sydenham's, but this case is the first report of criteria-based secondary mania with probable Sydenham's chorea, as well as the longest reported remission between symptomatic Sydenham's episodes. The parallel course of this patient's hypo

Neurologic anomalies of Perrault syndrome.

We report on an 18-year-old man with neurosensory hearing loss and his sister with neurosensory hearing loss, ovarian dysgenesis, mental retardation, generalized ataxia of the trunk and limbs, and saccadic dysmetria. A CT scan showed cerebellar hypoplasia. The cardinal manifestations of Perrault syndrome in females are neurosensory hearing loss and ovarian dysgenesis. Other anomalies, including neurologic and skeletal, have been reported in other individuals with Perrault syndrome. We review the neurologic anomalies in previous patients with Perrault syndrome. Neurologic data are available on

Ulnar nerve anatomy and compression.

Compression of the ulnar nerve can be understood in terms of the anatomic and dynamic factors. Although the ulnar nerve may be compressed at any point along its course, it is particularly susceptible at the elbow and the wrist. Clinically relevant anatomy will be reviewed in an attempt to provide the reader with a logical framework for successfully diagnosing and managing typical and atypical ulnar nerve compression lesions.

[Herpes zoster: follow-up, complications and therapy].

In clinical practice herpes zoster infections are common. The cause is the reactivation of the herpes varicella virus that persists in the sensory ganglia after an earlier primary infection with shingles. There are several neurological complications such as meningitis, ventriculitis, encephalitis, myelitis, cerebral angiitis, myositis, paresis of motor nerves, acute polyneuritis, and most commonly post-zoster neuralgia. A proposed reason for these complications is the direct infiltration of the virus or a hematogenous infection. Some of the complications can be treated symptomatically such as

Neuroendoscopic third ventriculostomy: a nursing perspective.

Endoscopic third ventriculostomy is an old technique that has gained greater acceptance in recent years with the advent of new endoscopic technology. This procedure, which involves fenestrating the floor of the third ventricle to communicate the basilar cisterns with the third ventricle offers new hope to patients with non-communicating hydrocephalus. Until recently, the only accepted treatment for these patients was the placement of a ventriculoperitoneal shunt to drain the accumulating cerebrospinal fluid. It is well known that shunting carries the risks of infection, blockage and overdraina

Blinking and the release reflexes: are they clinically useful?

OBJECTIVE: To review the clinical utility of blinking and the release reflexes, palmomental, glabellar, grasp, and snout, as an important part of the bedside neurological examination. DESIGN: Articles published from 1966 to 1993 that addressed blinking or the release reflexes were identified by searching the MEDLINE database. Thirty-seven references provided clinically useful and applicable information. RESULTS: There is controversy regarding the optimal method of clinical elicitation of these reflexes. The release reflexes are frequently seen as an incidental abnormality in the elderly. A p

[Therapeutic use of botulinum A toxin in neurology].

The highly potent neurotoxins produced by Clostridium botulinum lead to botulism when ingested in appreciable amounts. However, botulinum toxin injections delivered intramuscularly in very small quantities can produce a therapeutically intended focal paresis while producing only negligible local or systemic side effects. Over the past several years, various neurological disorders, especially those involving increased muscle tone and/or abnormal movements, have been successfully treated with local botulinum A toxin injections. The success of this method has led to a general change in the manage

Bilateral predominant radial nerve crutch palsy. A case report.

Axillary crutch palsy, although well described in the literature, is rarely seen in clinical practice. Little has been written in the contemporary orthopaedic literature. The purpose of this case report is to report on a well-documented axillary crutch palsy with bilateral, predominantly radial, nerve dysfunction that resolved spontaneously in eight weeks.

[Neurosurgical treatment of lumbar stenosis. A review of the literature].

Low back pain and claudicatio spinalis are typical for spinal stenosis. Neuroradiological diagnosis is best accomplished by a myelogram. The operative results in patients with a lumbar spinal canal stenosis are successful in 85.5%. Radicular pain responds better than lumbar pain. Severe complications such as new persistent neurological deficits are rare. If the operation is performed early the results are usually better than with delayed surgery. The decompression should be adapted to the type and extent of the stenosis. Additional removal of the disc as well as severe damage to the facet join

[Hip changes in infantile cerebral palsy].

Dislocation of the hip associated with cerebral motor disturbances differs considerably from that in children with normal motor activity. In patients with such disturbances ist is a relatively common and often serious complication. It occurs at an age when there are not usually any clear spastic signs, merely hypotonia with reflex-like movement patterns. Early dislocation is marked by early onset of acetabular dysplasia. In the final analysis, hip dislocation associated with cerebral motor disturbances is attributable to a more or less constant muscle imbalance. The prognosis for hip developme

Review of tests of standing balance abilities.

This review examines tests of standing balance abilities in patients with traumatic brain injury. Patients may compensate for vestibular dysfunction by using other sensory systems. As a result, subtle deficits in balance abilities may not be detected by traditional tests. The purpose of this paper is to provide a compilation of standing balance tests and to provide the clinician with the latest technological advances that can be used to assess balance abilities.

AIDS and the brain.

AIDS dementia complex is a major source of disability for many AIDS patients. Less commonly, the spinal cord or peripheral nerves are affected by HIV infection. Physicians working with AIDS patients must differentiate these manifestations from those caused by the infectious, neoplastic and vascular complications.

A quick and easy guide to neurological assessment.

A quick and easy guide with step-by-step pictorial descriptions has been developed for use as a uniform approach to neurological assessment by the nurse. It is the packaging of the information and the visual aids that may be useful, educational tools for nurses in performing a systematic assessment of the patient.

Cerebral vascular diseases.

Strokes remain an important cause of mortality and morbidity despite recent advances in neurologic intensive care. Although support of ventilation and circulation improves the outcome for stroke patients, the efficacy of cerebral resuscitative measures such as hyperventilation, osmotherapy, fluid restriction, steroids, intracranial pressure control, and barbiturates remains unproven. Current specific therapies (anticoagulation, surgery) are useful in carefully selected patients. New therapies now being researched include fluorocarbons, narcotic antagonists, and calcium blockers.
