Vascular compression of cranial nerves. I. History of the microvascular decompression operation.

The development of the microvascular decompression (MVD) operation is reviewed. It is stressed that a few innovative neurosurgeons discovered the role of vascular compression of cranial nerves V and VII in trigeminal neuralgia (TGN) and hemifacial spasm (HFS) and developed an operation, later to be known as the MVD operation. While the understanding of the pathophysiology of these disorders has improved, the surgical procedure has undergone little change since Gardner described the operation about 1960.

On the evaluation of cranio-cervical decompression: normalisation of the cerebral spinal fluid circulation in MRI?

1) By means of "cine-MRI" a semi-quantitative analysis of CSF pulsations in the aqueduct of Sylvius can be carried out. 2) This investigation enables a qualitative evaluation of the cranio-spinal CSF flow phenomena. On the one hand this information is a valuable aid for neurosurgeons for diagnosis and on the other, cine-MRI progress controls allow an evaluation of the success of an operation in the sense of a quality control.

Characterizing Visual Neurosurgical Expertise in Brain MRI Visualization Using Eye-Tracking and 3D Fractal Dimension Analysis.

Eye-tracking has been utilized to characterize visual behavior in medical image visualization and interpretation, yet neurosurgeons remain underrepresented. Characterizing neurosurgery-specific visual expertise is important for understanding expert search strategies, informing training, and developing computational models. This study examined gaze behavior in naïve observers (Np = 29), neurosurgery registrars (Np = 16), and consultant neurosurgeons (Np = 24), viewing normal (Np = 20) and pathological (Np = 19) brain MR images under a free-viewing paradigm. To capture expertise-related characte…

The "chameleon" sellar lesions: a case report of unexpected sellar lesions.

INTRODUCTION: The sellar region and its boundaries represent a challenging area, harboring a variety of tissues of different linings. Theref … oma, case 2 had a primary neuroendocrine tumor, and case 3 had cavernous malformation. CONCLUSION: Neurosurgeons should consider "unexpected" lesions of the sellar/suprasellar region in the preoperative differential diagnosis. A multidisciplinary approach with the collaboration of neurosurgeons, neuroradiologists, and pathologists plays a fundamental role. The …

INTRODUCTION: We present a case of a 60-year-old female that underwent surgery for clipping a right vertebrobasilar junction aneurysm (VBJA) … in that case resulted in a safe and effective technique. It is an interesting tool that could help neurosurgeons deal with certain selected cases of VBJA. Intraoperative angiography offers the possibility to reposition a misplaced clip in the same surgery. Good collaboration between interventional neuroradiologists and vascular neurosurgeons helps in achieving …

Undergraduate teaching of neurosurgery - what is the current practice in the UK and is there a need for improvement?

There has been concern that the move towards generalism means that specialties, such as neurosurgery, are being pushed out of the undergraduate syllabus. Surveys were created, along with the Society of British Neurological Surgeons, and sent to medical school representatives (MSRs) and neurosurgery programme directors (NPDs) in the United Kingdom (UK). 60% of MSRs and 71% of NPDs responded. Neurosurgical topics were taught by a variety of specialties, and in one medical school, a MSR said that neurosurgery was not taught at all. 83% of MSRs and 80% of NPDs said that neurosurgeons should be mor…

[Chemotherapy of chiasmal gliomas in children].

Chiasmal gliomas are rare tumors occurring predominantly in childhood. Their optimal treatment is still controversial. In the past only neurosurgeons (performing partial or subtotal removal of the tumor, biopsy or shunting procedure in hydrocephalus) and radiotherapeutists participated in their treatment. In the middle of the eighties there was only a single article dealing with chemotherapy of these tumors (Rosenstock, 1985). Since that time there was an increased number of articles about harmful effects of radiotherapy on the developing child's brain. Neurosurgeons are aware that they will n…

[Case report: a 5-year-old girl with spinal, epidural empyema - an uncommon but successful therapy performed by anaesthetists in Malawi].

A five-year-old girl suffering from sepsis and acute paraplegia was diagnosed with spinal epidural empyema due to a superinfected tuberculous osteomyelitis of a vertebra. In the absence of a neurosurgeon the anaesthetists opted for an epidural procedure. To this effect they aspirated, drained and irrigated the epidural space on several levels via epidural Tuohy cannulas - followed by an antibiotic instillation.

Mechanical Complications of Sophysa SM8 Shunt in Adult Hydrocephalus: A Monocentric Experience.

BACKGROUND: Sophysa SM8 is widely used by neurosurgeons in France. Published studies report shunt malfunction rates in adults between 18% and 29%. However, these studies included multiple valve types and thus entailed a serious confounding factor. OBJECTIVE: To ascertain the incidence the Sophysa SM8 cerebrospinal fluid (CSF) shunt malfunctions in adults. METHODS: We present a retrospective series of adult patients who underwent CSF shunt placement between 2000 and 2013 with Sophysa SM8. RESULTS: In total, 599 patients (329 males and 270 females) were included. The mean age at surgery was 6…

Stroke Microsurgical Thrombectomy Human Placenta Simulator.

BACKGROUND: Stroke microsurgical cerebrovascular thrombectomy reports are limited, although this technique could be used in many centers as a primary treatment or a salvage intervention option. It requires great ability, so our aim is to describe and validate a stroke microsurgical thrombectomy ex vivo simulator with operative nuances analysis. METHODS: Human placenta (HP) models simulated middle cerebral artery vessels with intraluminal thrombus to be microsurgically excised. Six neurosurgeons performed 1-mm and 2-mm longitudinal and transverse arteriotomy in different arteries to remove a 1…

Diversity in Neurosurgery.

Over the past century, the field of neurosurgery has evolved and expanded in various directions. Neurosurgeons have continuously pushed the boundaries of the specialty with scientific discovery and innovation. A diverse array of neurosurgical techniques, treatments, and new areas of study have emerged within the field. Meanwhile, the neurosurgical workforce has stayed demographically homogeneous throughout time. Certain groups remain underrepresented owing to systemic barriers based on social identities and categorizations, including gender, race, ability status, and others. In this article, w…

A history of the Jannetta procedure.

Peter Jannetta was a neurosurgery resident when he proposed the neurovascular compression theory. He built upon the astute observations of Dandy, Gardner, and others who, in the era before the operating microscope, had successfully ventured into the posterior fossa. In 1965, Jannetta performed cranial nerve microdissections for dental students and identified the trigeminal portio intermedia. He proposed that preservation of these sensory fibers may avoid complete facial numbness, and together with Robert Rand developed a subtemporal transtentorial approach for selective rhizotomy for trigemina

Current practice of cervical disc arthroplasty: a survey among 383 AOSpine International members.

OBJECTIVE The use of cervical disc arthroplasty (CDA) in spinal practice is controversial. This may be explained by the lack of studies with a large sample size and long-term outcomes. With this survey the authors aimed to evaluate the opinions of spine surgeons on the use of CDA in the current treatment of cervical disc herniation (CDH). METHODS A web-based survey was sent to all members of AOSpine International by email using SurveyMonkey on July 18, 2016. A single reminder was sent on August 18, 2016. Questions included geographic location; specialty; associated practice model; number of di

Traumatic Brain Injury Studies in Britain during World War II.

As a result of the wartime urgency to understand, prevent, and treat patients with traumatic brain injury (TBI) during World War II (WWII), clinicians and basic scientists in Great Britain collaborated on research projects that included accident investigations, epidemiologic studies, and development of animal and physical models. Very quickly, investigators from different disciplines shared information and ideas that not only led to new insights into the mechanisms of TBI but also provided very practical approaches for preventing or ameliorating at least some forms of TBI.
