Impact of BCL-2 rs2279115 and rs3943258 variants on protein expression and clinical outcome of urothelial bladder carcinoma.

BACKGROUND: Urothelial bladder carcinoma (UBC) is the ninth most common malignancy worldwide and ranks thirteenth in cancer-related mortality. A significant challenge in managing non-muscle-invasive UBC (NMIBC) is the high recurrence rate, compounded by a paucity of robust prognostic biomarkers. Given that evasion of apoptosis is a hallmark of carcinogenesis, the apoptosis regulator BCL-2 oncogene plays a critical role by negatively regulating the intrinsic apoptotic pathway, often leading to protein overexpression and malignant cell immortalization. METHODS AND RESULTS: This study evaluated

FGFR inhibitors promote the autophagic degradation of IFN-γ-induced PD-L1 and alleviate the PD-L1-mediated transcriptional suppression of FGFR3-TACC3 in non-muscle-invasive bladder cancer.

Bladder cancer (BC) is the second most prevalent genitourinary malignancy worldwide. Treatment options remain limited for patients with Bacillus Calmette-Guérin (BCG)-unresponsive non-muscle-invasive bladder cancer (NMIBC). Up to 70% of NMIBC cases harbor fibroblast growth factor receptor 3 (FGFR3) alterations, and FGFR inhibition has shown potential to enhance the efficacy of immune checkpoint inhibitor (ICI). Interferon (IFN)-γ, a cytokine produced by activated T cells and associated with better response to immunotherapy in BC, is a key inducer of PD-L1 expression in the tumor microenvironme

Interpretable machine learning model of contrast-enhanced CT radiomics for predicting post-BCG recurrence in high-grade non-muscle-invasive bladder cancer.

BACKGROUND AND PURPOSE: While intravesical Bacillus Calmette-Guérin Vaccine (BCG) instillation remains standard adjuvant therapy for high-grade non-muscle-invasive bladder cancer (NMIBC) post-resection, marked interpatient response heterogeneity complicates recurrence prediction. This study develops a contrast-enhanced computed tomography (CT) radiomics-based machine learning model to quantify tumor heterogeneity and noninvasively predict 5-year recurrence in high-grade NMIBC. PATIENTS AND METHODS: This retrospective study included 136 patients with histopathologically confirmed high-grade NM

High and selective cytotoxicity of ex vivo expanded allogeneic human natural killer cells from peripheral blood against bladder cancer: implications for natural killer cell instillation after transurethral resection of bladder tumor.

BACKGROUND: Non-muscle-invasive bladder cancer (NMIBC) is treated with transurethral resection of bladder tumor (TURBT) followed by intravesical instillation of chemotherapy or Bacillus Calmette-Guérin therapy. However, these treatments have a high recurrence rate and side effects, emphasizing the need for alternative instillations. Previously, we revealed that expanded allogeneic human natural killer (NK) cells from peripheral blood are a promising cellular therapy for prostate cancer. However, whether NK cells exhibit a similar killing effect in bladder cancer (BCa) remains unknown. METHODS

Elevated serum Raftlin levels in non-muscle invasive bladder cancer: diagnostic implications and rs690037 genotype analysis.

OBJECTIVE: Non-muscle invasive bladder cancer is a common urological malignancy characterized by high recurrence and progression rates. There is a critical need for non-invasive biomarkers to support early detection and disease monitoring. Raftlin, a lipid raft-associated scaffold protein involved in immune regulation and signal transduction, has recently gained attention in cancer biology. The aim of this study was to investigate serum Raftlin levels and the rs690037 polymorphism of the Raftlin gene in non-muscle invasive bladder cancer patients. METHODS: A case-control study was conducted,

BCG Induces PD-1 Upregulation on Circulating CD8+ T Cells and IL-6 and IL-8 Secretion In Vitro.

Backgrounds: Bacillus Calmette-Guérin (BCG) remains the most effective adjuvant therapy for non-muscle-invasive bladder cancer (NMIBC); however, the immunological underpinnings of its efficacy remain incompletely understood. This study aims to elucidate the dual immunomodulatory roles of BCG by integrating systemic and tumor-intrinsic analyses, through determining the systemic effects of BCG instillation on immune checkpoint expression and direct inflammatory response in a previously established in vitro tumor model. Methods: We investigated systemic and tumor-intrinsic immune responses to BCG

Sarcopenia Weakens the Benefit of Bacillus Calmette-Guérin in Non-Muscle-Invasive Bladder Cancer: A Computed Tomography-Based Cohort Study.

OBJECTIVES: Non-muscle-invasive bladder cancer (NMIBC) commonly recurs despite intravesical Bacillus Calmette-Guérin (BCG) therapy. Host factors that modify the effectiveness of BCG have not been completely defined. We evaluated whether sarcopenia is associated with outcomes after BCG in Japanese patients. METHODS: We conducted a retrospective single-center cohort study (2010-2023) including high-grade (HG) NMIBC patients treated with BCG (n = 139) and those not receiving BCG (n = 100). Sarcopenia was defined a priori using sex- and BMI-specific skeletal muscle index (SMI) and psoas muscle in

Efficacy and safety of RC48 alone or combined with PD-1 inhibitors in high-risk non-muscle invasive bladder cancer.

High-risk non-muscle invasive bladder cancer (HR-NMIBC) presents a significant therapeutic challenge due to its high recurrence and progression rates, particularly in patients with human epidermal growth factor receptor 2 (HER2) expression who are unresponsive to or ineligible for Bacillus Calmette–Guérin (BCG) therapy. This study aimed to evaluate the efficacy and safety of the HER2-targeted antibody-drug conjugate (ADC), Disitamab Vedotin (RC48), alone or in combination with PD-1 inhibitors, as a systemic bladder-sparing approach in a real-world HR-NMIBC cohort. We conducted a multicenter, r

Preparation of photo-controlled release ROS-responsive Ce6/elemene co-loaded liposomes and study on the effect on enhancing apoptosis of NMIBC.

At present, chemotherapy combined with photodynamic therapy is exerting satisfactory therapeutic effects in the treatment of tumors. Chlorin e6 (Ce6) is a photosensitizer with high efficiency and low dark toxicity. At the same time, elemene (ELE) contains high-efficiency and low-toxicity anti-cancer active ingredients, which can effectively penetrate tumor tissue and inhibit its recovery and proliferation. Due to the poor water solubility of these two drugs, we prepared ELE/Ce6 co-loaded liposomes (Lipo-ELE/Ce6) to improve their water solubility, thereby enhancing the anti-tumor effect. The ch

Mycobacterial fusion protein REA eliciting anti-tumor activity through activation of innate immunity.

Mycobacterial components are widely used as immunoadjuvants. Mycobacterium bovis bacillus Calmette-Guérin (BCG) remains the gold standard for treating high-risk, non-muscle-invasive bladder cancer, and its cell wall skeleton has been used to treat several cancers. Therefore, we investigated the anti-tumor activity of mycobacterial fusion protein Rv2299cD2D3-ESAT-6-Ag85B (REA) and its potential role as a mycobacterial-derived immunomodulator. REA exhibited potent antitumor effects, driving robust innate immune activation that secondarily engages adaptive immune responses against tumor-derived a

SREBP2-RAB11A-ZDHHC20 axis orchestrates FGFR3 palmitoylation and membrane retention to drive bladder cancer progression.

BACKGROUND: Non-muscle-invasive bladder cancer (NMIBC) accounts for approximately 75% of all bladder cancer cases and is characterized by a high recurrence rate, requiring long-term surveillance and repeated treatments that impose substantial clinical burdens. While aberrant fibroblast growth factor receptor 3 (FGFR3) signaling is a well-established driver in the majority of NMIBC cases, the precise molecular mechanisms regulating FGFR3 plasma membrane stability and trafficking remain poorly defined, limiting targeted therapeutic development. This study aims to elucidate the molecular pathway

Prospective comparative evaluation of clinical efficacy and safety of Intravesical gemcitabine-docetaxel versus BCG in adjuvant management of BCG-naive non-muscle invasive urothelial bladder cancer in Indian population.

BACKGROUND: Intravesical Bacillus Calmette-Guérin (BCG) is the standard adjuvant therapy for intermediate/high-risk non-muscle-invasive bladder cancer (NMIBC) but is limited by failure, toxicity, and global supply constraints. Sequential Intravesical gemcitabine-docetaxel (Gem/Doce) can offer a rational chemotherapy alternative. METHODS: In this prospective randomized, parallel-arm interventional study comparing efficacy and safety of Gem/Doce versus BCG in BCG-naïve intermediate/high-risk NMIBC, patients aged 18-80 years with complete TURBT of primary localised bladder lesion(s) were include
