Central oculomotor disturbances and nystagmus: a window into the brainstem and cerebellum.

BACKGROUND: Oculomotor disturbances and nystagmus are seen in many diseases of the nervous system, the vestibular apparatus, and the eyes, as well as in toxic and metabolic disorders. They often indicate a specific underlying cause. The key to diagnosis is systematic clinical examination of the patient's eye movements. This review deals mainly with central oculomotor disturbances, i.e., those involving smooth pursuit, saccades, gaze-holding, and central types of nystagmus. METHODS: We searched the current literature for relevant publications on the diagnosis and treatment of oculomotor distur

[Eye muscle surgery in persistent bilateral trochlear paralysis and concomitant Brown phenomenon after indirect craniocerebral trauma].

BACKGROUND: Rarely, an additional Brown's syndrome develops spontaneously in cases of posttraumatic bilateral superior oblique palsy. Surgical measures are controversial and problematic. CASE REPORT: We here report on a 40-year-old patient who, 2 years after blunt head trauma, presented with residual bilateral VIth nerve palsy with 12 degrees excyclotropia in downgaze, abnormal head postures of 20 degrees chin down position in binocular viewing and reduced elevation in adduction. His binocular field of gaze ranged from 10 degrees upgaze to 25 degrees upgaze. After thorough discussion of the s

[Benign paroxysmal tonic upward gaze syndrome].

INTRODUCTION: The benign paroxysmal tonic upward gaze syndrome (BPTUG) is a rare condition. We present two new cases analyzing the clinical, genetic, evolution and therapeutic aspects. CLINICAL CASES: Case 1. A 2 year-old girl with no family history of similar disorders started, at the age of 6 months, to have episodes of upward deviation of gaze with hyperextension of the neck and vertical nystagmus of fixation, increased by nervousness and episodes of fever. Some episodes caused the patient to fall in spite of there being no alteration of consciousness. Case 2. From the age of five months a

[Congenital cranial dysinnervation disorders (CCDD)].

Knowledge about hereditary eye diseases has been substantially increased by means of genetic testing during the last decade. This has resulted in a new classification of a number of disease patterns, which are characterised by non-progressive restrictive disorders of the oculomotor system, formerly classified as "congenital fibrosis syndromes". Based on the results of genetic testing, these ocular motility disorders are now referred to as "congenital cranial dysinnervation disorders" (CCDDs). They are caused by an impaired innervation of extraocular muscles because of a dysgenesis of the nucle…

Longstanding diplopia after ethmoidal artery ligation for epistaxis.

PURPOSE: In the rare case of intractable, posterior, non-sphenopalatine artery epistaxis, ligation of ethmoidal arteries using an external approach like a Lynch-type incision is required. Orbital complications, especially extra-ocular motility disorders with diplopia, are known, but in the literature rarely described. Our aim was to analyse the complication type, rate, and outcome of ethmoidal artery ligation for epistaxis. MATERIALS AND METHODS: Data between 2012 and 2017 of patients treated with ethmoidal artery ligation were analysed retrospectively and through a telephone interview using …

[Thomsen myotonia congenita and strabismus: recording saccades in myotonic diseases].

INTRODUCTION: Thomsen myotonia is an autosomal dominant, but not dystrophic myotonia. We report a family case associating congenital Thomsen's myotonia, strabismus, and ocular myopathy. We successively studied seven isolated patients presenting a myotonic disease and two controls and recorded the ocular saccades of these patients (amplitude, velocity) as a comparison for our family case. PATIENTS AND METHODS: Four women from the same family presented Thomsen myotonia associated with strabismus. In addition to the clinical manifestations, the electro-oculographic analysis showed saccade altera

Surgical management of hypotropia and exotropia in high myopia: a case report.

Myopic strabismus, also known as heavy eye syndrome, is a rare ocular motility disorder, typically presenting with bilateral esotropia and hypotropia. We present the case of a 42-year-old man who presented with progressive hypotropia and exotropia rather than esotropia. Orbital imaging revealed a superonasal staphyloma with pathological changes in the extraocular muscles. Surgical correction involved loop myopexy between the superior rectus and medial rectus muscles combined with lateral rectus recession.

Neural substrates of the oculomotor vergence network for concussion-related convergence insufficiency within the CONCUSS study.

Concussion-related convergence insufficiency (CONC-CI) participants were compared to binocularly normal controls (BNC) using an fMRI stimulus-induced vergence oculomotor task to assess group-level differences in neural substrates. The hypothesis tested was whether the CONC-CI and BNC groups would show significant differences in the vergence oculomotor neural substrates, providing comparative data for the CONCUSS randomized clinical trial or future therapeutic longitudinal studies. A pediatrician diagnosed concussions, and an optometrist diagnosed convergence insufficiency. The CONC-CI group ex

'Staircase' square-wave jerks in early Parkinson's disease.

BACKGROUND/AIMS: Visually guided saccades and gaze-fixation ability were recorded in patients with early Parkinson's disease (PD). METHODS: Magnetic search coil system was used to measure horizontal and vertical eye positions. RESULTS: 'Staircase' visually guided saccades (multiple hypometric saccades separated by an intersaccadic interval) and 'staircase' square-wave jerks (SWJ) were present in all patients. The SWJ amplitude (total amplitude of the series of hypometric saccades comprising the SWJ) was abnormally large (mean 2°). SWJ frequency was also abnormally high (50-70 intrusions/min)

Reading-related oculomotor testing and training protocols for acquired brain injury in humans.

Many individuals with acquired brain injury (ABI) report reading problems of oculomotor origin. These may include frequent loss of place, skipping of lines and difficulty shifting to the next line of print. We describe two protocols for the testing and training of reading-related eye movements in adult individuals with ABI (traumatic brain injury [TBI] and stroke with hemianopia), who experience oculomotor-based symptoms when reading. These protocols use objective eye movement recording techniques and computer-based stimulus presentation and analysis. One protocol tests and the other trains ba

Convergence insufficiency in thyroid eye disease.

The incidence of convergence insufficiency in patients with thyroid ophthalmopathy and their response to convergence exercise is unknown. Twenty-seven consecutive patients with thyroid eye disease (TED) were evaluated prospectively. Six patients with symptomatic convergence insufficiency obtained varying degrees of subjective and objective benefit from treatment. None of the six had other TED-related complaints that required therapeutic intervention. The study findings should alert the clinician to consider convergence insufficiency in patients with TED and nonspecific ophthalmic symptoms.

Synergistic divergence and jaw-winking phenomenon.

Synergistic divergence is a congenital syndrome of monocular adduction deficit, with simultaneous abduction of both eyes on attempted gaze into the field of action of the paretic medial rectus muscle. Based on electromyographic data, it has been speculated that it is a neural miswiring disorder related to Duane's retraction syndrome. The association between Duane's retraction syndrome and the Marcus Gunn jaw-winking phenomenon, another neural miswiring disorder, has been previously reported. The authors describe a patient with both bilateral synergistic divergence and Marcus Gunn jaw-winking p

Near and distance vergence facility provides complementary clinical information in concussion-related convergence insufficiency.

PURPOSE: To compare near and distance vergence facility testing in adolescents and young adults with concussion-related convergence insufficiency and evaluate changes following office-based vergence/accommodative therapy (OBVAM). METHODS: This secondary analysis of the CONCUSS randomized clinical trial evaluated vergence facility at near (40 cm) and distance (4 m) using a 12Δ base out/3Δ base in prism flipper. Participants aged 11-25 years with concussion-related convergence insufficiency were randomized to immediate or 6 weeks delayed OBVAM. Vergence facility was assessed at baseline, outcom

Effect of Vision Therapy on Accommodative Lag in Myopic Children: A Randomized Clinical Trial.

SIGNIFICANCE: Accommodative dysfunction has been suggested to be related to the development and progression of myopia. Office-based accommodative/vergence therapy (OBAVT) improved accommodative facility in Chinese myopic children, but it is unclear if such improvement has a role in decreasing myopic progression. PURPOSE: The purpose of this study was to compare the effects of OBAVT with home reinforcement and office-based placebo therapy (OBPT) as a treatment to improve accommodative functions (i.e., lag, amplitude, and facility) in myopic children with poor accommodative accuracy. METHODS:

Post-traumatic pseudomyopia.

BACKGROUND: Many clinicians have noted that patients demonstrate a myopic refractive change following Traumatic Brain Injury (TBI). This apparent myopic shift disappears with cycloplegia, yet stubbornly reappears as soon as the pharmaceutical effect wears off. We propose that this shift is secondary to an irritative lesion that affects the parasympathetic innervation, resulting in ciliary body contracture. The dilemma for the clinician is whether to provide the immediate relief of clear distance vision by prescribing additional minus lenses, or to work toward attempting to re-establish the bas

Post-COVID simultaneous onset of Graves' disease and ocular myasthenia gravis in a patient with a complex ocular motility impairment.

A 74-years-old man experienced severe diplopia one month after recovery from an uncomplicated SARS-CoV-2 infection. Neurological examination was normal whereas ophthalmological examination showed bilateral exophthalmos with a complex ocular motility disorder characterized by a pseudo-internuclear ophthalmoplegia after fatigue associated to impairment of elevation and infraduction. Antibodies against TSH and acetylcholine receptors were positive; subsequent hormonal tests, ultrasonography of thyroid gland, single fiber electromyography and orbit MRI confirmed the diagnosis of concomitant Graves
