Open reduction and internal fixation of fractured mandibular condyles by a retromandibular approach: surgical morbidity and informed consent.

Treatment of fractured mandibular condyles remains controversial. In this paper we present data collected prospectively on 42 fractured condyles treated by open reduction and internal fixation. The central issue, which is sometimes forgotten, is the informed consent of our patients. To this end we have looked at the surgical morbidity related to the retromandibular approach.

Preliminary results and complications following limited open reduction and percutaneous screw fixation of displaced fractures of the acetabulum.

Twenty-four patients underwent attempted closed or limited open reduction of displaced acetabular fractures. If reduction was successful, the fractures were stabilized with percutaneous screws. Group 1 was composed of elderly patients with complex fractures and radiographic findings that were felt to be predictive of post-traumatic arthritis. In these patients, percutaneous screw fixation was used to improve fracture anatomy, allow mobilization and total hip replacement later, if necessary. In group 1, anatomical reduction was not felt to be a necessity. Group 2 was composed of young patients

Fractures of the calcaneus: open reduction and internal fixation from the medial side a 21-year prospective study.

Since 1974, 61 displaced fractures of the calcaneus have been treated by open reduction and internal fixation by a modified medial approach technique. Surgery was performed through a 5-cm incision posterior to the neurovascular bundle. A single threaded pin was passed longitudinally through the tuberosity and into the sustentacular fragment, giving stable fixation. Reduction of the depressed posterior facet fragments was accomplished from the medial side in 77% of cases, occasionally assisted by fluoroscopy. Postoperatively all fractures were immobilized in a cast for 4 weeks. At the end of 4

Open reduction and internal fixation of two-part displaced fractures of the greater tuberosity of the proximal part of the humerus.

Twelve patients, ranging in age from thirty-four to seventy-two years (average, fifty-three years), were evaluated an average of five years (range, two to eight years) after open reduction and internal fixation of a two-part displaced fracture of the greater tuberosity of the proximal part of the humerus. The indication for operative reduction was one centimeter or more of displacement of the fracture as seen on the diagnostic radiographs. The anterosuperior deltoid-splitting approach, combined with rotation of the humerus, allowed adequate exposure of the retracted tuberosity. Internal fixati

Posterior Malleolus Fractures of the Tibia: Open Reduction and Buttress Plating versus Percutaneous Fixation with an Anteroposterior Screw.

OBJECTIVE: The posterior malleolar fragment (PMF) in ankle fractures can be fixed with closed reduction and percutaneous fixation with an anteroposterior screw (Closed Reduction Internal Fixation - CRIF), or with open reduction using a posterior approach and buttress plating (Open Reduction Internal Fixation - ORIF). The objective of this study was to compare these two techniques with respect to postoperative complications and patient-reported outcome measures (PROMs). MATERIALS AND METHODS: In this retrospective study, data were analysed from 106 consecutive ankle fracture cases involving th

Clinical and functional outcomes of open reduction and internal fixation for scapular fractures using the modified Judet approach.

OBJECTIVE: Displaced scapular fractures are uncommon injuries, and the optimal surgical approach remains controversial. Muscle-preserving techniques may improve post-operative shoulder function by minimizing soft-tissue damage. The purpose of the study was to evaluate the clinical and functional outcomes of open reduction and internal fixation (ORIF) using a muscle-preserving modified Judet approach in displaced scapular fractures and to assess the effect of surgical timing on post-operative shoulder function. METHODS: This retrospective study included 12 adult patients who underwent ORIF for

Return to riding after open reduction internal fixation of acute clavicle fractures in professional horse jockeys.

BACKGROUND: High-velocity, traumatic fracture of the clavicle is one of the most common injuries suffered by professional horse jockeys. Operative fixation represents the mainstay of treatment in this cohort. Despite this, there remains a paucity of evidence to guide post-operative recovery and time to return to riding. Accordingly, the aim of the present study was to establish the safety and efficacy of open reduction internal fixation of acute clavicle fractures in professional horse jockeys. METHODS: A prospectively maintained database was used to retrospectively identify professional hors

Evaluation of surgical outcomes after open reduction and internal fixation of condylar fractures: An ambispective cohort study.

BACKGROUND AND AIM: Mandibular condylar fractures can cause functional impairment, malocclusion, and temporomandibular joint (TMJ) dysfunction; consequently, open reduction and internal fixation (ORIF) is increasingly adopted. This study evaluated the clinical, functional, and radiographic outcomes of ORIF for condylar fractures over varying postoperative follow-up periods. STUDY DESIGN: This ambispective cohort study included 60 patients who underwent ORIF for unilateral or bilateral mandibular condylar fractures between 2021 and 2025. Clinical evaluation included mandibular mobility, TMJ dy

The clinical outcomes of open reduction and internal fixation for Mason-Johnston type IV fractures of the radial head.

BACKGROUND: The treatment of displaced or comminuted Mason-Johnston type IV radial head fractures is challenging. These fractures often involve complex injuries to the ligaments surrounding the radial head, necessitating careful consideration of treatment strategies that prioritize ligament repair while preserving the integrity of the radial head whenever feasible. The primary objective of this study was to assess the clinical outcomes of open reduction and internal fixation (ORIF) in managing complex radial head fractures associated with transolecranon fractures, Monteggia fractures, and terr

Comparison of a New Minimally Invasive Reduction Technique Using Open Wiring in the Treatment of Subtrochanteric Femur Fractures.

OBJECTIVE: Minimally invasive reduction and intramedullary nailing (IMN) are the most common strategies used to treat subtrochanteric femur fractures (SFF). We report a new minimally invasive surgical technique for the successful reduction of SFF and maintenance of fragmental position after longitudinal traction. This study compared the radiological and clinical effects of these two reduction techniques when using IMN to treat SFF. METHODS: All 43 patients who underwent SFF at our hospital were included in this retrospective study. Twenty-two patients were treated using the conservative metho

Safety of Accelerated Return to Play After Anatomic Open Reduction and Internal Fixation of Adolescent Clavicle Fractures.

BACKGROUND: There is a paucity of data on the safety of an accelerated return-to-play (RTP) timeline after open reduction internal fixation (ORIF) of clavicular fractures in adolescent athletes, and whether patients may safely RTP before the traditional 10- to 14-week time point is unclear. PURPOSE: To assess the safety of an accelerated RTP timeline (<8 weeks) after ORIF of adolescent clavicle fractures. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: This was a dual-institution, retrospective cohort study of consecutive adolescent patients (age, 10-17 years) undergoing ORIF of

Antibiotic administration for open reduction internal fixation of closed ankle fractures: Is one preoperative dose enough?

Although preoperative antibiotics are used routinely in ankle fracture surgery, there is considerable variability in postoperative antibiotic dosing regimens amongst surgeons. The aim of this study is to compare the rate of surgical site infection (SSI) in patients receiving one dose of preoperative antibiotics to patients who received more than one dose of antibiotics. A single-center Level I Trauma Center retrospective review of patients with isolated closed ankle fractures who underwent open reduction internal fixation over a 10-year period was performed. Demographics data and risk factors

An Innovative Use of Maxillofacial Implants for the Open Reduction and Internal Fixation of a Displaced Sternal Fracture: A Case Report.

Sternal fractures resulting from blunt chest trauma often present unique surgical challenges. While conservative management is common, cases with significant displacement, delayed union, or painful dyspnea may require surgical intervention to improve structural stability and relieve symptoms. Here, we report the case of a 46-year-old man who sustained a displaced sternal fracture in a motor vehicle accident. The fracture was complicated by posterior displacement of the proximal bone fragment, causing persistent chest pain and deformity that did not resolve with conservative treatment. The pati

Outcomes of open reduction and internal fixation of calcaneus fractures: A database study comparing patients with and without diabetes.

Treatment of calcaneal fractures in patients with diabetes mellitus (DM) is challenging. The purpose of this study was to compare post-operative outcomes after open reduction and internal fixation (ORIF) for calcaneus fracture in patients with complicated DM, uncomplicated DM, and patients without DM. A commercially available de-identified database was queried for all calcaneus fracture diagnoses undergoing ORIF from 2010 to 2021. The patients were separated into three groups for analysis: patients without DM (10,951, 82.6%), uncomplicated DM (1,500, 11.3%) and complicated DM (802, 6.1%). At 1

Median Nerve Palsy in Pediatric Supracondylar Humerus Fractures Recovers Faster With Open Than Closed Reduction.

BACKGROUND: Management of supracondylar humerus fractures (SCHF) with coexisting median nerve injury is controversial. Although many nerve injuries improve with the reduction and stabilization of the fracture, the speed and completeness of recovery are unclear. This study investigates median nerve recovery time using the serial examination. METHODS: A prospectively maintained database of SCHF-related nerve injuries referred to a tertiary hand therapy unit between 2017 and 2021 was interrogated. Factors related to the injury (vascularity, Gartland grade, open vs. closed fracture) and treatment

A technique for introducing broad-spectrum topical antibiotics during open reduction and internal fixation of acetabular fractures.

Postoperative surgical site infections remain a significant and prevalent complication after open reduction and internal fixation of acetabular fractures. Local antibiotics have been shown to decrease risk of postoperative infection, although recent evidence is conflicting. We provide a consistent and replicable technique for delivering intraoperative broad-spectrum antibiotics in the form of a putty applied directly to surgical implants. With this technique, systemic levels of those antibiotics remain safe and stable.
