[Vertebral pedicle screw-rods system for correcting paralytic scoliosis].

OBJECTIVE: To explore some operative problems of correcting paralytic scoliosis (PS) by using vertebral pedicle screws-rods system. METHODS: From May 2000 to May 2005, 18 patients with PS were corrected by screws-rods system which were made of titanium alloy. There were 10 males and 8 females, aging from 11 to 26 years. The primary disease included poliomyelitis in 13 patients and myelodysplasia (MS) in 5 patients (2 cases for second correction) with scoliosis of an average 85 degrees Cobb angle (55-125 degrees). The pelvic obliquity was found in all patients with an average 24 degrees angle

Traumatic shoulder instability. Nonsurgical management versus surgical intervention.

Shoulder instability is classified as either traumatic or atraumatic based on the mechanism of injury. Traditional treatment for both forms of instability involves a nonsurgical approach, consisting of immobilization, rehabilitation, and a delay in the return to vigorous activities. This treatment is often quite successful in preventing recurrent dislocations in the patient with atraumatic instability. However, those patients with traumatic instability often experience further dislocations or subluxations, with recurrence rates as high as 94% in patients younger than 20 years. These recurrent

Does instrumentation removal cause curve progression in patients with adolescent idiopathic scoliosis?

Adolescent idiopathic scoliosis (AIS) is usually instrumented using a posterior approach. Hardware removal may be performed for specific clinical reasons. Little data is available on whether removal influences curve magnitude. The aim of the paper is to evaluate the impact of instrumentation removal on curve progression, and the safety and efficacy of the procedure. We analyzed 59 patients who underwent instrumentation removal. Curve types, reasons for removal, period between procedures, and Cobb angles: at baseline, immediately after correction, after removal and in follow-up were evaluated.

Late implant infections caused by Propionibacterium acnes in scoliosis surgery.

One hundred and one consecutive adolescent scoliosis patients instrumented from the posterior between 1995 and 2002, with a minimum follow-up of 24 months (maximum 106 months), were reviewed for incidence of infection. Stainless steel implants with pedicle screws were used in the thoracic and lumbar spine of all patients. All were operated on by a single surgeon. There were no early infections. Incidence of late infection was 6.9% (seven patients). Clinical indicators for infection were the sudden onset of local pain and swelling without fever after an initial pain-free interval. There were no

[Rehabilitation of multiple injured patients].

Severe multiple trauma raises difficult problems of rehabilitation. In the most usual cases, other injuries are associated with head injuries and coma, and they must be systematically screened by a very complete radiological assessment and a thorough clinical examination. Rehabilitation will be passive as long as the injured person is comatose; however, it is very important as it allows preventing bedsores and the reduction of joint amplitude that may have serious consequences later on. The associated fracture of the limbs require orthopedic or surgical solutions. External fixation devices, wh

Limb salvage reconstruction of the lower limb with complex ankle arthrodesis and magnetic internal lengthening nail.

PURPOSE: With advances in orthopedic implants, the use of intramedullary lengthening devices has gained increasing popularity as an alternative technique compared to lengthening with external fixators, with alleged comparable or better outcomes. The aim of this study is to report our single-center technique and outcomes of combined ankle arthrodesis and proximal tibial lengthening using external fixator with a motorized intramedullary nail, respectively. METHOD: Fourteen patients with post-traumatic advanced ankle arthritis underwent staged ankle arthrodesis with external fixator and proximal

Systematic review of valgus intertrochanteric osteotomy in the setting of femoral neck fractures.

INTRODUCTION: Valgus intertrochanteric osteotomy (VITO) is a fixation procedure to manage neglected femoral neck fractures (FNF) in young adults. While several studies have evaluated the efficacy of VITO, there remains variability in intraoperative technique and fixation devices. The study aims to systematically review the current orthopedic literature to summarize the patient outcomes and determine the efficacy of VITO in young adults with neglected FNFs. METHODS: All peer-reviewed studies written in English about VITO for neglected FNFs from 2000 to 2022 were searched using the National Cen

A comparative study of three commonly used fixation techniques for isolated medial malleolus fracture.

OBJECTIVES: This study aims to compare the three most commonly used fixation techniques: tension-band wire fixation, partially threaded cannulated screws, and fully threaded cannulated headless compression screws. PATIENTS AND METHODS: Ninety patients with medial malleolus fractures were included in the study. Patients were divided into three groups. Group A included patients who underwent tension-band wire fixation (n=26), Group B partially threaded cannulated compression screws (n=32), and Group C fully threaded cannulated headless compression screws (n=32). The type of medial malleolus fra

[Choice of surgical procedure of the distal radius. Bore wire versus plate].

At the distal radius surgical treatment is indicated in fractures with defect, compression zone or re-dislocation, fractures with joint depression, open fractures and fractures with neurological and circulatory impairment. K-wire transfixation is mainly indicated in unstable fractures of elderly patients, in juvenile fractures with involvement of the epiphyseal line, and in simple intraarticular fractures, if the articular surface can be restored easily. T-plate osteosynthesis is the method of choice in fractures with volar displacement (Smith-Goyrand), in dislocated unstable intraarticular fr

Anterior fixation for fractures of the thoracic and lumbar spine with or without neurologic involvement.

The anterior approach is useful for treatment of early and late thoracic and lumbar spine burst fractures but is especially useful for a post-traumatic kyphosis. The indications for dural decompression include acute neurologic injury in which CT scanning shows evidence of significant canal intrusion of the spinal cord. The use of anterior fixation devices (and, in particular, the anterior Kostuik-Harrington distraction device), supplemented with Dwyer screws and a solid Hall rod, precludes the necessity of any posterior approach. Anterior surgery has been performed in 49 cases for burst injuri

[Comparison of Blount's sling and Kirschner wire fixation in the treatment of a dislocated supracondylar humeral fracture in childhood].

From 1979 to 1982 we were treating 63 cases of displaced supracondylar fractures of the humerus. Patients underwent either closed reduction and fixation by collar and cuff immobilization (32 cases), or fixation by means of Kirschner wires following closed or (more often) open reduction (31 cases). Follow-up controls after an average time of three years clearly showed the more favourable results of the conservative method.

The treatment of congenital dislocation and subluxation of the hip in the older child.

Two additional risks are involved in the treatment of congenital dislocation of the hip when diagnosis is made too late: avascular necrosis following the reduction, and residual coxo-femoral dysplasia. Our study and the experience that we acquired within the past 10 years on 142 hips show the value of closed reduction by slow progressive traction with abduction and internal rotation, and the value of innominate osteotomy. These are two strong points on which we can base our treatment in order to further improve the results of our efforts in this field.

Compressed Fixation Combined with Vacuum-Assisted Closure for Treating Acute Injury of the Heel Fat Pad.

BACKGROUND Treating acute injury of the heel fat pad is different from treating common soft tissue damage. Due to the paucity of literature on the topic, we described our initial experience treating acute injury of the heel fat pad to determine the ideal treatment method. MATERIAL AND METHODS A total of 53 patients with acute injury of the heel fat pad admitted to our hospital were selected for the study and were randomly divided into 2 groups: the compressed fixation combined with vacuum-assisted closure group and the only reimplanted and sewn group. Twenty-seven of the heel fat pads were com

In vivo deformation of anatomically pre-bent rods in thoracic adolescent idiopathic scoliosis.

Some surgical strategies can maintain or restore thoracic kyphosis (TK); however, next-generation surgical schemes for adolescent idiopathic scoliosis (AIS) should consider anatomical corrections. A four-dimensional correction could be actively achieved by curving the rod. Thus, anatomically designed rods have been developed as notch-free, pre-bent rods for easier anatomical reconstruction. This study aimed to compare the initial curve corrections obtained using notch-free rods and manually bent, notched rods for the anatomical reconstruction of thoracic AIS. Two consecutive series of 60 patie

A biomechanical assessment of a novel double endobutton technique versus a coracoid cerclage sling for acromioclavicular and coracoclavicular injuries.

PURPOSE: Recently, many acromioclavicular-coracoclavicular (AC-CC) ligament reconstruction techniques address only the CC ligament. However, many of these techniques are costly, time-consuming, and require the use of allogenic grafts, making them prone to creep and failure or novel devices making them challenging for orthopaedic surgeons. The purpose of this study was to compare the biomechanical characteristics of a double endobutton technique using a standard endobutton CL with those of a coracoid cerclage sling (CS) for reconstruction of the CC ligaments. METHODS: Anterior-posterior (AP) t

Modified technique of presurgical infant maxillary orthopedics for complete bilateral cleft lip and palate.

This article introduces technical modifications to the conventional presurgical infant maxillary orthopedics device for newborns with complete bilateral cleft lip and palate, providing procedural simplicity and efficiency as well as therapeutic efficacy. The modifications incorporate a wax block-out on the stone model prior to device fabrication in a manner that the need for periodic acrylic addition and removal is not required, and thus eliminates the risk of natural maxillary growth restriction during infant maxillary orthopedics treatment. The premaxilla is completely excluded from the acry
