A Quantitative Approach for the Objective Assessment of Coupling Efficiency for an Active Middle Ear Implant by Recording Auditory Steady-state Responses.

OBJECTIVE: The coupling efficiency of a semi-implantable active middle ear implant with an electromagnetically driven floating mass transducer coupled to a middle ear ossicle or the round window can only be quantified postoperatively in cooperative patients by measuring behavioral vibroplasty in situ thresholds in comparison with bone conduction thresholds. The objective of the study was to develop a method to objectively determine the vibroplasty in situ thresholds by determining calibration factors from the relation between the objective and behavioral vibroplasty in situ thresholds. Use of Lasers in Otosclerosis Surgery.

Lasers were introduced as an atraumatic modality for accomplishing several of the crucial steps in otosclerosis surgery. Advances in laser technology have spurred coevolution of refinements in the technique of the operation. Several varieties of laser systems are available to suit individual preference and to augment a surgeon's armamentarium; however, a clear advantage in terms of surgical outcome or patient safety remains to be demonstrated.

General history of stapedectomy.

The article gives an overview of the historical development of stapedectomy beginning with Kessel in 1876. Then, from the beginning to the middle of the 20th century, surgery on the oval window became obsolete, opening the way for an era of fenestration operations until Shea in 1956 performed the first modern stapedectomy using a Teflon stapes replacement prosthesis. Since then, numerous surgeons worldwide have used this procedure with great success. Many of them have contributed towards progressively refining the surgical techniques, e.g. by changing the total removal of the footplate for the

Composite autograft "shield" reconstruction of remnant tympanic membranes.

The authors present their combined experience with large cartilage-perichondrial composite grafts used to reconstruct total tympanic membrane perforations in 294 ears. Patients chosen for this procedure had failed earlier tympanoplasty surgery or were identified as poor candidates for conventional facial tympanoplasty because of the perforation size. Successful perforation closure was achieved in 97% of ears with chronic otitis media characterized by absence of the tympanic membrane, including portions of the anterior annular ligament. Hearing results in general were good, considering the adva

Small fenestra stapedotomies with and without KTP laser: a comparison.

The results of 33 small fenestra stapedotomies performed using conventional techniques were compared with the results of 33 stapedotomies performed using the argon or KTP laser. The ossicular chain was reconstructed using a Teflon wire piston of 0.6 mm diameter, and follow-up was at least 1 year. Over-closure of the air-bone gap or closure to within 10 dB was accomplished in 91% of the laser-treated group versus 72% of the conventionally treated group (p less than 0.10). The hearing results were statistically better in the laser group (p less than 0.05). Transient delayed vestibular symptoms,

The use of a small fenestra technique with the Fisch piston in the surgical treatment of otosclerosis.

The modern surgical treatment of otosclerosis consists of replacement of the sound conducting function of the stapes by a prosthesis. The results obtained in 100 consecutive patients using the small fenestra technique and a 0.4 mm. Teflon and steel wire prosthesis are reported. The surgical technique is described. The hearing was improved in all patients. In 92 per cent of the patients an optimal hearing gain was found after an observation time of 1 to 4 years. Five patients required re-operation during the observation time. The small fenestra technique and the Fisch prosthesis were considered

Design of Piezoelectric Dual-Bandwidth Accelerometers for Completely Implantable Auditory Prostheses.

For the last 20 years, researchers have developed accelerometers to function as ossicular vibration sensors in order to eliminate the external components of hearing aid and cochlear implant systems. To date, no accelerometer has met all of the stringent performance requirements necessary to function in this capacity. In this work, we present an accelerometer design with an equivalent noise floor less than 20 phon equal-loudness-level over a 0.1-8 kHz bandwidth in a package small enough to be implanted in the middle ear. Our approach uses a dual-bandwidth (two sensing elements) microelectromech

Ossiculoplasty in chronic otitis media: Surgical results and prognostic factors of surgical success.

BACKGROUND AND OBJECTIVES: The goal of ossiculoplasty is to improve hearing. Successful ossiculoplasty depends on several factors. This retrospective study was carried out to analyze hearing results of ossiculoplasty in ears with chronic otitis media (COM) and evaluate clinical outcomes and factors predictive of hearing improvement. SUBJECTS AND METHODS: We reviewed the results of 153 patients with COM (with cholesteatoma (COMC) and without cholesteatoma (COMWC)) who underwent ossiculoplasty between January of 2002 to December of 2011. Several potential prognostic factors were evaluated: chol

[Use of a body acoustic probe and the conduction of brain stem potentials for the determination of sound conduction during tympanoplasty].

A new procedure of evoking brain stem potentials is introduced. Contrary to evoking brain stem potentials by acoustic stimulation via earphone or loudspeaker, we touch the middle ear apparatus at various locations using a piezoelectric transducer with a screwed-on probe, thus causing physiological movements within the sound conducting structures (Fig. 3). As a reaction to this kind of stimulation--1000 clicks with a duration of 0.1 ms were given--far field brain stem potentials can be recorded. The calibration of this special sound probe--not bigger than a pencil (Fig. 1)--was made by equalizi

Early activation of active middle ear implants: a prospective study.

PURPOSE: Vibrant Soundbridge (VSB) is an active middle ear implant that serves as a solution for people unable to use conventional hearing aids properly. Surgical techniques for VSB implantation have progressively advanced, improving its outcomes. Traditionally, VSB processor activation transpires around four weeks after the surgery to provide enough healing; however, contemporary practices in cochlear implant and bone-anchored hearing device activations indicate reduced waiting times. The purpose of this research is to explore the feasibility, safety, and advantages of early VSB activation.

Experimental quantification of the vibrant soundbridge stapes head (SH-) coupler in human cadaveric temporal bones.

BACKGROUND: The "floating mass transducer" (FMT) of the Vibrant Soundbridge was initially attached to the long process of the incus (LP). In patients missing an incus, coupling to the stapes head using the Vibroplasty CliP-coupler is used. Its main limitation is the height, leading to potential contact with the tympanic membrane. The new stapes head (SH)-coupler reduces the functional height but tilts the stimulation angle by 30°. We investigated the SH-coupler's magnitude and sound transmission quality compared to the LP-coupler. METHODS: Laser Doppler vibrometry was used on 10 cadaveric hum

ACTIVE MIDDLE EAR VIBRANT SOUNDBRIDGE SOUND IMPLANT.

The Vibrant Soundbridge represents a new approach to hearing improvement in the form of active implantable middle ear hearing device. Unlike conventional acoustic hearing aids, which increase the volume of sound that goes to the eardrum, the Vibrant Soundbridge bypasses the ear canal and eardrum by directly vibrating the small bones in the middle ear. Because of its design, no portion of the device is placed in the ear canal itself. The Vibrant Soundbridge has been approved by the FDA as a safe and effective treatment option for adults with moderate to severe sensorineural, conductive or mixed

Optical Clearing Agents for Optical Imaging Through Cartilage Tympanoplasties: A Preclinical Feasibility Study.

HYPOTHESIS: Optical clearing agents (OCAs) can render cartilage tympanoplasty grafts sufficiently transparent to permit visualization of middle ear structures in an operated ear using optical coherence tomography (OCT) imaging. METHODS: Pieces of human tragal cartilage were treated with glycerol, a commonly used OCA. A reference reflector was imaged with OCT through the tympanoplasty as it cleared and the optical attenuation of the graft was measured. The reversibility of clearing and the dimensional changes associated with glycerol absorption were also measured. In a separate experiment, a h
