[Paget disease, complicated by Ki-1 lymphoma with G-CSF growth factor secretion].

Development of lysis within an area of pagetic bone suggests a sarcoma. We report the case of a 61-year-old woman who had increasingly severe pain in her right hip and a decline in general health. The roentgenogram of the pelvis showed previously known pagetic lesions of the right half of the pelvis and osteolysis of the roof of the acetabulum. Additional imaging studies (ultrasonography, computed tomography, and magnetic resonance imaging) disclosed a pelvic tumor extending on either side of the right ilium. The peripheral white blood cell count was markedly elevated (93,000/mm3) as a result

Ipriflavone: pharmacological properties and usefulness in postmenopausal osteoporosis.

Ipriflavone (IP) is an isoflavone derivative available in several countries for investigational and/or therapeutic use. Inhibition of bone resorption was demonstrated in several models, both in vitro and in vivo for IP and its metabolites. Their mechanisms of action on bone are not yet fully elucidated but some of them are widely accepted. IP does not possess, per se, any estrogenic activity. It appears that IP-related inhibition of bone resorption might be mediated by an indirect effect on osteoclast and related to an inhibition of recruitment and/or differentiation of pre-osteoclast, maybe t

Relationships between quantitative histological measurements and noninvasive assessments of bone mass.

We performed a comprehensive analysis of the relationships between histologic indices in the iliac crest (cancellous bone volume, trabecular structural indices, cortical width, and core width) and bone density in the spine, hip, and wrist in 81 patients with various metabolic bone diseases including osteoporosis, osteomalacia, hyperparathyroidism, and Paget's disease. In the whole group, all of the histologic indices correlated significantly with bone mineral density (BMD) of the spine and the three regions of the hip (r = 0.28-0.73), with the exception of cortical width which correlated with

Bone hypertrophy and trabecular generation in Paget's disease and in fluoride-treated osteoporosis.

The replacement of lost trabeculae characteristic of postmenopausal osteoporosis is problematic, since a biological pathway has not been established for trabecular regeneration de novo in the healthy, intact, mature skeleton. Possible pathways for trabecular replacement may occur under pathological conditions, in particular those associated with bone hypertrophy. The topography of trabecular hypertrophy was compared in two groups of subjects with disease- or treatment-induced osteosclerosis following a period of atrophy. In Paget's disease and fluoride-treated osteoporosis a thickening of rare

Radiology of Paget's disease.

A review of Paget's disease produces few surprises; however, several good articles have recently been published on its appearance, on MR imaging, sarcomatous degeneration, and the disease-related occurrence of multicentric giant-cell tumors. One recent paper illustrated the possible progression of fibrous dysplasia occurring in the skull after adolescence, a finding that does not occur elsewhere. An excellent review article on the normal and abnormal calcaneus is reviewed, and two brief reports on skeletal angiomatosis are discussed. Condensing osteitis of the clavicle is well described in one

Treatment of Paget's disease with intermittent low-dose infusions of disodium pamidronate (APD).

Disodium pamidronate (APD) is a potent inhibitor of bone resorption, with less risk of defective mineralization than earlier bisphosphonates. We assessed the response to six spaced low-dose intravenous infusions of APD given at intervals of approximately 6 weeks followed by weekly infusions if bone turnover remained abnormal. Three groups of 10 patients were studied, each group receiving infusions of 15, 30, or 45 mg. Hydroxyproline excretion fell by 62% and alkaline phosphatase was reduced by 72%, with no difference between the dose levels. A total of 21 patients (70%) achieved normal levels

Characteristic CT manifestations of uncommon spinal disorders.

This article identifies many disorders seen on CT scans of the spine that should be recognized to avoid submitting the patient to additional diagnostic tests or even surgery. These disorders are not easily categorized, however, and could best be described as normal variants and pitfalls in CT diagnosis of the spine.

Focal osteomalacia due to low-dose diphosphonate therapy in Paget's disease.

Transiliac bone biopsies carried out on 13 patients with Paget's disease to evaluate the effects of low-dose diphosphonate (disodium etidronate) therapy showed focal osteomalacia in the 9 patients in whom post-therapy specimens were taken through pagetic bone. Active bone resorption persisted in 5 of these. A mineralisation defect not amounting to osteomalacia--ie, osteoid of increased thickness but of normal extent--was present in the 4 specimens taken through non-pagetic bone. Although 9 patients experienced symptomatic improvement, 2 suffered fissure fractures in affected lower limbs. In Pa

Relationship of the activity of the bone isoenzyme of serum alkaline phosphatase to urinary hydroxyproline excretion in metabolic and neoplastic bone diseases.

A significant correlation between the activity of the bone isoenzyme or serum alkaline phosphatase and the urinary hydroxyproline excretion in osteomalacia, osteoporosis, primary hyperparathyroidism with osteodystrophy, Paget's disease, secondary bone tumours, and in a control group was found (P less than 0.001). This close correlation was not observed between these variables in patients with active acromegaly. Diagnosis determined from these indices of formation and turnover of bone matrix agreed with that established by histological and histochemical examination of bone, by X-ray investigati

Paget's disease. Radiological changes occurring in untreated patients and those on therapy with salmon calcitonin during two years' observation.

Radiological observations over two years were made in 23 patients with Paget's disease. Radiological criteria for progression and regression of disease are defined. Fourteen of the patients were treated with salmon calcitonin. Of these treated patients four showed regression of the disease and only two showed progression, both in the skull vault. Of the remaining nine untreated patients none showed regression but two showed progression. The radiological findings confirm that calcitonin will prevent the extension and sometimes induce healing of Paget's disease in long bones but that the skull a

Ossification in extradural fat in Paget's disease of the spine.

A case of cauda equina compression by ossification in extradural fat at a level above the site of Paget's disease in a vertebra is described. Good recovery followed laminectomy and exposure of the dura mater.
