[Classification of haematogenous and post-traumatic osteomyelitis].

A classification of osteomyelitis must reflect the complexity of the disease and, moreover, provide conclusions for the treatment. The classification is based on the following eight parameters: source of infection (OM [osteomyelitis]/OT [post-traumatic OM]), anatomic region, stability of affected bone (continuity of bone), foreign material (internal fixation, prosthesis), range of infection (involved structures), activity of infection (acute, chronic, quiescent), causative microbes (unspecific and specific bacteria, fungi) and comorbidity (immunosuppressive diseases, general and local). In the…

Antibiotics for treating chronic osteomyelitis in adults.

BACKGROUND: Chronic osteomyelitis is generally treated with antibiotics and surgical debridement but can persist intermittently for years with frequent therapeutic failure or relapse. Despite advances in both antibiotic and surgical treatment, the long-term recurrence rate remains around 20%. This is an update of a Cochrane review first published in 2009. OBJECTIVES: To determine the effects of different systemic antibiotic treatment regimens for treating chronic osteomyelitis in adults. SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (Octobe…

Isolated cryptococcal osteomyelitis in an immunocompetent patient.

Cryptococcosis is a rare infection in HIV-negative individuals. While the lungs and the central nervous system are most commonly infected, skeletal cryptococcosis is uncommon and isolated osteomyelitis due to Cryptococcus neoformans is quite rare. To our knowledge, only 47 cases of isolated cryptococcal osteomyelitis have been reported from 1974 to 2005. We report a case of isolated cryptococcal osteomyelitis in an immunocompetent patient, who received 12 weeks of fluconazole with complete recovery.

Advances in the diagnosis and management of pediatric osteomyelitis.

Acute hematogenous osteomyelitis is a common disease that affects previously healthy children of all age groups. Despite its importance, there are limited data in the current literature to guide many aspects of the diagnosis and therapy of this infection. Over the last decade, our understanding of the etiology of this infection has changed, with increased recognition of Kingella kingae and the dramatic increase in community-associated Methicillin-resistant Staphylococcus aureus (CA-MRSA) infections. There is a need for the optimization of diagnostic strategies, such as MRI and serum inflammato…

A solitary skull lesion of syphilitic osteomyelitis.

We experienced a rare case of solitary syphilitic osteomyelitis of the skull without any other clinical signs or symptoms of syphilis. A 20-year-old man was referred due to intermittent headache and mild tenderness at the right parietal area of the skull with a palpable coin-sized lesion of softened cortical bone. On radiological studies, the lesion was a radiolucent well enhanced mass (17 mm in diameter). The erythrocyte sedimentation rate (52 mm/h) and C-reactive protein (2.24 mg/dL) were elevated on admission. Serum venereal disease research laboratory (VDRL) and Treponema pallidum haemaggl…

Case of localized scleroderma associated with osteomyelitis.

We report a 4-year-old girl presenting with progressive linear scleroderma affecting the right leg. Biopsy specimen disclosed typical histopathological findings of localized scleroderma. Right leg magnetic resonance imaging (MRI) showed high signal areas on T(2)-weighted images on the subcutaneous fatty tissue, muscles and bone marrow, suggesting that skin inflammation extended to the bone marrow. Oral corticosteroid therapy was instituted with improvement of both skin sclerosis and MRI findings. Our observations suggest that MRI examination should be considered in patients with localized scle

Osteomyelitis variolosa: a report of two cases.

We present 2 patients with osteomyelitis variolosa in both elbow joints--sequelae of smallpox. The condyles were elongated and the central portions of the distal humeri were absorbed and the elbow joints were unstable. One of the patients sustained a closed fracture of the distal humerus. The fracture united uneventfully following stabilisation and bone grafting. At the 13-year follow-up, the patient had satisfactory elbow function.

Bisphosphonates and osteomyelitis of the jaw: a pathogenic puzzle.

The maxillary and mandibular bones undergo high-turnover remodeling to maintain mechanical competence. Common dental or periodontal diseases … reased production of cytokines derived from the bone matrix, and the bone is exposed to the risk of osteomyelitis and necrosis. The pathogenic relationship between BPs and osteonecrosis of the jaw is unclear, but there is evidence to indicate an association between high-dose BP treatment and exposure to dental infections or oral surgical procedures. A better kn …

Differential diagnosis of Charcot arthropathy and osteomyelitis.

Foot problems are common causes of morbidity in patients with diabetes mellitus. Foot ulcers are the leading cause of hospitalization in diabetic patients. Bones may be involved in two different clinical conditions: osteomyelitis and Charcot osteoarthropathy. Osteomyelitis usually develops by spreading from contiguous soft tissue to underlying bone. Charcot foot is deformation of foot as a result of muscle athrophy, bone and joint structure changes in a joint as a secondary complication of neuropathy. To distinguish bone infection from non-infectious bone disorders as in Charcot joint may be d…

Primary subacute osteomyelitis of the greater trochanter.

Primary subacute osteomyelitis is difficult to diagnose, because of its insidious onset, mild symptoms, lack of a systemic reaction, and similarity with tumoral conditions. This condition is seen mostly in the epiphysis or metaphysis of tibia. Although it has been reported at various bones, its occurrence in the greater trochanter is rare. We describe two patients, 4 and 14 years old, with mild pain of the hip joint and limping without history of acute bone infection. Radiography showed a lytic lesion in the greater trochanter resembling Brodie abscess. Both patients were treated completely wi…

Acute mastoiditis and osteomyelitis of the temporal bone.

OBJECTIVE: Acute mastoiditis becomes clinically significant when infection spreads through the periosteum and induces periosteitis. This study describes an atypical complication of acute mastoiditis: osteomyelitis of the temporal bone. PATIENTS AND METHODS: The study sample included all patients admitted for acute mastoiditis between September 2001 and December 2003 who had symptoms, signs and imaging findings of osteomyelitis of the temporal bone beyond the mastoid area. The files were reviewed for diagnosis, work-up, radiographic findings and treatment. RESULTS: The study group included 6 …

[Expert opinion problems in the evaluation of osteomyelitis].

Cases of osteomyelitis can be differentiated according to hematogenous (endogenous), and post-traumatic (exogenous) forms. Both types can lead to considerable permanent damage or recur even after a fairly long time. In the formulation of an expert opinion to assess cases of osteomyelitis, the foremost question concerns a possible error in treatment or clarification of a connection to a postulated trauma. The present article describes aspects of expert opinions on cases of osteomyelitis of the long bones, spinal column, and in children. In addition to potential claims for compensation for harm …

Osteomyelitis and pyogenic sacroiliitis: A difficult diagnosis.

Pyogenic sacroiliitis is an uncommon infection often diagnosed late because of poor localization of symptoms and inadequate physical examination. We describe a 12-year-old girl whose osteomyelitis and pyogenic sacroiliitis was initially misdiagnosed and discuss examination, investigation and management of the condition. This case highlights the importance of thorough physical examination and the consideration of septic arthritis in an unusual joint.

"Cold" and "hot" osteomyelitis on bone scintigraphy.

Bone scanning is frequently used for the assessment of a child with pain around the hip joint. The authors present a child with septic hip arthritis in which a striking abnormality of complete absence of tracer uptake in an extensive region of the proximal femur was demonstrated on bone scintigraphy. Later during the course of his disease, markedly increased tracer uptake was seen along most of the femoral diaphysis with the exception of the avascular proximal bone. These scintigraphic findings represent extensive osteomyelitis of the femur with avascular necrosis of its proximal part.

Mandibular pyogranulomatous osteomyelitis in a Sannen goat.

Mandibular pyogranulomatous osteomyelitis was diagnosed in a female Sannen goat. The doe presented for difficulty prehending and chewing food. The left mandible was swollen and firm on palpation. Radiographs revealed changes consistent with osteomyelitis of the affected mandible. Arcanobacterium pyogenes was isolated from aspirates of swollen mandible. Despite antimicrobial therapy, the goat died. Histopathological findings were consistent with pyogranulomatous disease of the affected mandible. The histopathological findings were similar to those reported for actinomycosis, caused by Actinomyc…
