Autonomic cardiovascular regulation in patients with obstructive sleep apnoea: a study based on spectral analysis of heart rate variability.

The aim of this study was to assess the function of the autonomic nervous system in patients with obstructive sleep apnoea syndrome (OSAS). The study was designed as a cross-sectional case-control study. Fifty-one patients were included, and the findings were compared with those in 66 controls. Spectral analysis of heart rate variability (HRV) during supine rest, during controlled breathing and after tilting was performed in each patient and control case. The patients performed overnight sleep recordings the night before the HRV recordings. Individuals with an apnoea-hypopnoea index (AHI) abov

Neurocardiac dysregulation and neurogenic arrhythmias in a transgenic mouse model of Huntington's disease.

Huntington's disease (HD) is a heritable neurodegenerative disorder, with heart disease implicated as one major cause of death. While the responsible mechanism remains unknown, autonomic nervous system (ANS) dysfunction may play a role. We studied the cardiac phenotype in R6/1 transgenic mice at early (3 months old) and advanced (7 months old) stages of HD. While exhibiting a modest reduction in cardiomyocyte diameter, R6/1 mice had preserved baseline cardiac function. Conscious ECG telemetry revealed the absence of 24-h variation of heart rate (HR), and higher HR levels than wild-type litterm

Development and significance of a fetal electrocardiogram recorded by signal-averaged high-amplification electrocardiography.

Although ultrasonic diagnostic imaging and fetal heart monitors have undergone great technological improvements, the development and use of fetal electrocardiograms to evaluate fetal arrhythmias and autonomic nervous activity have not been fully established. We verified the clinical significance of the novel signal-averaged vector-projected high amplification ECG (SAVP-ECG) method in fetuses from 48 gravidas at 32-41 weeks of gestation and in 34 neonates. SAVP-ECGs from fetuses and newborns were recorded using a modified XYZ-leads system. Once noise and maternal QRS waves were removed, the P,

[Autonomic nervous system function of alcoholics during acupuncture treatment].

Clinical investigation was performed in 24 male Grade II alcoholics and a control age-matched groups. Autonomic disorders were polymorphic both in cardiovascular and other systems. Corporal-auricular acupuncture changed the autonomic nervous control that was evidenced in 12-13 sessions of electrical acupuncture point measurement. Acupuncture increased the parasympathetic influences. The data reflect the stabilization of adaptive mechanisms which were deteriorated in alcoholic patients.

Dysautonomia and Postural Orthostatic Tachycardia Syndrome: A Critical Analysis of Dysautonomia: How to Diagnose and Treat.

A significant number of physicians are unclear of the vast clinical manifestations of dysautonomia and imbalance of the autonomic nervous system, specifically the parasympathetic and sympathetic nervous systems. The major obstacle has been an inability to determine the mechanism of action as well as multisystem dysfunction and a lack of clear-cut testing. Dysautonomia, a pathophysiological malfunction of the sympathetic and parasympathetic nerves in our bodies, can present as altered clinical functions of heart rate (tachycardia/bradycardia), altered breathing patterns, blood pressure (hyperte

Simultaneous degeneration of myenteric plexuses and pelvic parasympathetic colonic nerve in slow transit constipation: A case report.

RATIONALE: Slow transit constipation (STC) is a common disease of which the etiology is still not clear. Multiple hypotheses have been proposed to explain STC, including autonomic neuropathy, disorders of the enteric nervous system and so forth. Morphological abnormalities of the enteric nerves of the colon in patients with STC have been extensively reported, while there have been no morphological reports focusing on extrinsic extramural fibers from the pelvic plexus to the distal colon (i.e., pelvic parasympathetic colonic nerve) in patients with STC. PATIENT CONCERNS: Whether morphological

Differential expression of SNAP-25a and SNAP-25b RNA transcripts in cranial nerve nuclei.

Synaptosomal-associated protein of 25 kDa (SNAP-25) is involved in the molecular regulation of neurotransmitter release. SNAP-25 exists in two isoforms, SNAP-25a and SNAP-25b, which arise from alternate splicing and which are differentially expressed throughout the nervous system. In situ hybridization was used to examine the presence and subcellular localization of SNAP-25a and SNAP-25b RNA transcript expression in motor and parasympathetic nuclei associated with cranial nerves. SNAP-25a RNA transcripts were strongly expressed in the parasympathetic Edinger-Westphal nucleus and dorsal motor n

Impaired vagal adaptation to an exercise task in women with gestational diabetes mellitus versus women with uncomplicated pregnancies.

PURPOSE: Gestational diabetes mellitus (GDM) is associated with an increased risk for maternal and fetal complications. Patients with GDM have an increased cardiovascular risk in later life. The aim of this study was to investigate cardiac autonomic nervous system (ANS) function at rest and during exercise in women with GDM vs. women with uncomplicated pregnancies. METHODS: Thirty-six normotensive pregnant women (21 with GDM and 15 age- and parity-matched women with an uncomplicated pregnancy) were enrolled in this case-control study. Continuous beat-by-beat blood pressure (BP) measurements w

Sympathetic and parasympathetic nerves regulate postsynaptic alpha-2 adrenoceptor in salivary glands.

The effects of sympathetic denervation or parasympathetic decentralization on the inhibitory effects of postsynaptic alpha-2 adrenoceptors were studied in the submaxillary and the sublingual gland of the rat. Chronic sympathetic denervation enhanced by a factor of 10 the potency of clonidine to inhibit the secretory responses of the submaxillary gland to either norepinephrine or methacholine. In denervated glands, clonidine (1 microgram/kg), reduced markedly the response to norepinephrine, but potentiated this response in control glands. Blockade of postsynaptic alpha-2 adrenoceptors with idaz

Structure and Functions of the Vagus Nerve in Mammals.

We review the structure and function of the vagus nerve, drawing on information obtained in humans and experimental animals. The vagus nerve is the largest and longest cranial nerve, supplying structures in the neck, thorax, and abdomen. It is also the only cranial nerve in which the vast majority of its innervation territory resides outside the head. While belonging to the parasympathetic division of the autonomic nervous system, the nerve is primarily sensory-it is dominated by sensory axons. We discuss the macroscopic and microscopic features of the nerve, including a detailed description o

The autonomic nervous system and the origins of neurocardiology.

Following new concepts by Bichat in the early 19th century, of organic and animal life centered around the ganglionic nervous system, over 100 years of anatomic studies and physiologic experimentation eventually resulted in Gaskell's 1916 book entitled "The Involuntary Nervous System" and Langley's 1921 book entitled "The Autonomic Nervous System." Neurology and cardiology emerged as specialties of medicine in the early 20th century. Although neurology made several prominent discoveries in neurophysiology during the first half of the 20th century, cardiology developed coronary care units and c

The effect of autonomic nervous system activity on gastric myoelectrical activity: does the spectral reserve hypothesis hold for the stomach?

Previous studies have associated changes in gastric myoelectrical activity during motion sickness, as indexed using the electrogastrogram (EGG), with changes in autonomic nervous system (ANS) activity. Subjects who did not report nausea in a rotating optokinetic drum were characterized by normal 3 cycles per minute (cpm) gastric myoelectrical activity, strong parasympathetic nervous system (PNS) activity, and low sympathetic nervous system (SNS) activity. The goals of this study were: (1) to determine whether tasks previously shown to alter ANS activity as indexed by changes in cardiac inter-b…

Ca(2+) channel alpha(1B) subunit (Ca(V) 2.2) knockout mouse reveals a predominant role of N-type channels in the sympathetic regulation of the circulatory system.

N-type voltage-dependent Ca(2+) channels (VDCCs), predominantly localized in the nervous system, have been proposed to play vital roles in a variety of neuronal functions such as neurotransmitter release at sympathetic nerve terminals. To directly approach the elucidation of the physiological significance of N-type VDCCs in the autonomic nervous system, alpha(1B) subunit (Ca(V) 2.2)-deficient mice were generated, in which peripheral neurons showed a complete and selective elimination of N-type channel currents sensitive to omega-conotoxin GVIA (the peptide toxin from the fish-hunting cone snai

Contribution of the sympathetic nervous system to hypertensive response to insulin excess in spontaneously hypertensive rats.

Our previous studies demonstrate that chronic insulin administration exacerbates hypertension in spontaneously hypertensive rats (SHR). In the present study, we tested the hypothesis that the pressor effect of insulin in SHR is medicated by sympathetic nervous system overactivity. Male SHR (7 weeks old) were given daily subcutaneous injection of insulin or vehicle for 3 days, after which each rat received an intravenous infusion of the peripheral ganglionic blocker hexamethonium. Two days later, in a second experiment, the infusion protocol was repeated with the alpha 2-adrenoceptor agonist cl

[Cardiovascular impact of the autonomic neuropathy of diabetes mellitus].

The neuropathic complications related to Diabetes may affect the somatic, sympathetic and parasympathetic nervous system. As a result, there are several clinical manifestations of diabetic neuropathy. They can be related to nervous system lesions of the genital, urinary, gastro-intestinal, skin and cardiovascular tissues. The results of these alterations are loss in the quality of life as well as increase of mortality indexes related to sudden death with cardiac arrhythmias and other causes. The cardiovascular autonomic neuropathy probably contributes to the bad prognosis of the coronary heart…

Autonomic asymmetry in migraine: augmented parasympathetic activation in left unilateral migraineurs.

Brain autonomic control is asymmetrical, the left hemisphere affecting predominantly parasympathetic function and the right hemisphere affecting predominantly sympathetic function. It is not known whether the extent of autonomic activation is altered in migraine, although the fact that some migraineurs express parasympathetic features such as facial flushing, lacrimation and rhinorrhoea might suggest increased parasympathetic activation. We instilled diluted soapy eyedrops and measured (i) the trigemino-parasympathetic reflex by the vasodilator response of forehead skin bilaterally using photo

Heart rate changes in diabetes mellitus.

Resting heart rates were measured lying, sitting, and standing in 61 diabetics with varying degrees of cardiovascular reflex abnormalities. Those with parasympathetic abnormalities alone had the highest heart rates, while those with both parasympathetic and sympathetic involvement had slightly less rapid heart rates, which were still faster than those in diabetics with normal cardiovascular reflexes. 38 other diabetics in whom autonomic function tests had been done at least three times had a similar pattern of resting heart rate. 25 had unchanged tests: those with parasympathetic involvement a
