Close surgical margin after conservative parotidectomy in early stage low-/intermediate-grade parotid carcinoma: Outcome of watch and wait policy.

AIM: The aim of the study was to assess the watch&wait strategy for management of patients with close surgical margin after conservative parotidectomy due to early low/intermediate grade parotid carcinoma. PATIENTS AND METHODS: Out of 78 patients operated on due to primary parotid gland cancer we selected 32 patients with a history of parotidectomy, and preservation of 7th nerve, with negative (≥1mm), but close (≤5mm) surgical margin and who did not receive supplementary radiotherapy due to other indications. Margins ranged from 1 to 3mm (in 27 cases it was 1mm, in 3 cases - 2mm and in 2 case

Oncocytic papillary cystadenoma of the parotid gland: characteristic multiparametric MRI signature.

INTRODUCTION: Oncocytic papillary cystadenoma is a rare benign epithelial neoplasm of the salivary glands, infrequently arising in the parotid gland. Its preoperative imaging characteristics are poorly described, and the diagnosis is often established only after surgical excision and histopathologic examination. CASE PRESENTATION: We report two female patients (62 and 77 years old) who presented with parotid masses. Multiparametric MRI demonstrated multiloculated cystic lesions with distinctive features: (1) markedly low signal intensity (“signal void”) on T2-weighted and diffusion-weighted im

Atypical Lipomatous Tumor/Well-Differentiated Liposarcoma of the Salivary Gland and Its Diagnostic Challenges: A Case Report and Literature Review.

Atypical lipomatous tumor/well-differentiated liposarcoma (ALT/WDLPS) is a low-grade adipocytic neoplasm composed of mature adipocytes with at least focal nuclear atypia in adipocytes and stromal cells. The terms "atypical lipomatous tumor" and "well-differentiated liposarcoma" are site-dependent terms for the same morphological and genetic entity. Amplification of MDM2 and/or CDK4 is almost always present and is a defining molecular feature. It usually arises in deep soft tissue. Primary head-and-neck involvement is rare, and salivary gland tumors are exceptionally uncommon. Only a few saliva

Primary Synovial Sarcoma of the Parotid Gland: A Diagnostic Challenge With Cyto-Histological Correlation and Review of Literature.

Synovial sarcoma is a malignant mesenchymal tumor of uncertain histogenesis that most commonly affects young adults' distal extremities, with the parotid being a very uncommon site. With fewer than 50 cases being reported in the literature and morphological similarities to many benign and malignant parotid lesions, it is challenging to diagnose this lesion. We report a 30-year-old female who presented with a rapidly enlarging, painful mass in the left parotid region with facial nerve palsy. The initial FNAC performed outside was interpreted as a pleomorphic adenoma. Repeat FNAC at our institut

Agressive small cell neuroendocrine carcinoma of the neck: A case report.

INTRODUCTION: The location in the neck of small cell neuroendocrine carcinoma (SCNEC) is rare and aggressive with a poor prognosis. We herein describe an extremely uncommon example of cervical SCNEC probably arising in the parotid gland, diagnosed at an advanced metastatic stage. PRESENTATION OF CASE: A 76 year old man, non-smoking, non-alcoholic presented a left cervical and rapidly progressive mass. It was suspicious of malignancy and located in the parotid space and II, III, IV and V lymph node groups. In two weeks, the patient had presented a rapid increase in size, with extensive skin pe

Predictors of distant metastasis in acinic cell carcinoma of the parotid gland.

BACKGROUND: AiCC is a primarily indolent disease process. Our aim with this study is to determine characteristics consistent with rapidly progressive AiCC of the parotid gland. AIM: To report on patients with metastatic lung disease from AiCC and potential correlative factors. METHODS: Single-institution retrospective review of patients treated at the University of Michigan between 2000 and 2017. Univariate analyses were performed. RESULTS: A total of 55 patients were identified. There were 6 patients (10.9%) with primary AiCC of the parotid gland who developed lung metastases. The mean age

The Role of Real-Time Sonoelastography in the Differentiation of Benign From Malignant Parotid Gland Tumors.

OBJECTIVE: The aims of this study are to evaluate the utility of sonoelastography for parotid gland masses and to determine which cutoff strain ratio (SR) would be best for the differentiation between benign and malignant lesions. METHODS: From August 2015 to December 2016, 39 parotid gland masses were examined prospectively by ultrasonography and strain sonoelastography. Elastographic scores were determined by a 4-point scoring method. Interventional procedures were performed on all patients after sonographic examinations. The lesions were divided into groups as benign or malignant according

Mucoepidermoid carcinoma as a secondary malignancy in pediatric sarcoma.

PURPOSE: Children diagnosed with osteosarcoma (OS) and Ewing sarcoma (ES) have greatly benefited from the addition of alkylator therapy. However, with greater numbers of long-term survivors, the rising incidence of secondary malignant neoplasms (SMNs) is concerning. Herein we report on 2 patients with sarcoma who developed a case of secondary mucoepidermoid carcinoma after chemotherapy treatment without associated radiation therapy. To our knowledge, this is the first series of mucoepidermoid carcinomas arising in pediatric patients treated for sarcoma without radiotherapy. METHODS: Long-term

Pleomorphic Adenoma of the Palatal Minor Salivary Gland: An Unusual Case With Imaging and Cytological Correlation.

Pleomorphic adenoma is the most common salivary gland neoplasm, accounting for a significant proportion of both major and minor salivary gland tumors. While it most frequently arises from the parotid or submandibular glands, it can occasionally present as an intraoral mass over the palate or lip when originating from the minor salivary glands. A palatal pleomorphic adenoma typically presents as a painless, gradually enlarging mass on the posterolateral aspect of the palate. This case highlights the importance of considering pleomorphic adenoma in the differential diagnosis of palatal swellings

Salivary duct carcinoma arising from the inferior turbinate.

Salivary duct carcinoma (SDC) is an uncommon, aggressive tumour, which carries a poor prognosis. SDC affects the major salivary glands, usually occurring in the parotid gland. A 76-year-old male patient presented with right-sided nasal obstruction and rhinorrhoea. A polypoidal mass was seen in the right inferior turbinate mimicking a benign neoplasm. Histological examination following wide excision led to a diagnosis of SDC. There are no therapeutic guidelines and management is based on experience with SDC of the major salivary glands. Given the aggressive nature and poor prognosis of the dise

Intracapsular (in situ) carcinoma ex pleomorphic adenoma with unusual clinical and histological features.

Carcinoma ex pleomorphic adenoma poses a challenge to diagnosis and treatment. Herein we describe an extremely unusual case, which presented initially as an intracapsular carcinoma ex pleomorphic adenoma in the right parotid gland. The first recurrence, after an interval of 8 years, showed only recurrence of the benign component. Five years later, a myoepithelial carcinoma arose from the same site. No ductal carcinoma as seen in the initial intracapsular carcinoma ex pleomorphic adenoma was identified. This case report elucidates the atypical clinical behaviour and interesting histological fea

[Lipomatous pleomorphic adenoma of the parotid].

Lipomatous tumors rarely occur in the salivary glands. We report an unusual case of lipomatous pleomorphic adenoma in a 47-year-old man. The patient had no significant medical history and presented with a well circumscribed nodule measuring 3 cm in the right parotid. Histologically, the tumour was predominantly composed of sheets of mature fat cells. Rare myoepithelial cells and exceptional tubules were intermingled with the mature adipose tissue. One year after surgery the patient was alive without recurrence. In conclusion, it is a rare neoplasm who needs to be recognized and discussed with

Oncocytic lipoadenoma of the submandibular gland.

We report a case of oncocytic lipoadenoma of the submandibular gland, previously unrecognized benign tumor of the salivary gland. The patient was a 66-year-old Japanese woman with a left submandibular mass, measuring 11 x 7.5 x 5 cm. Microscopically, the mass was completely surrounded by a thin fibrous connective tissue capsule, and was composed of an intimate admixture of mature fat cells and oncocytes. There have been no English reports of such a neoplasm in the salivary gland with the exception of one recent case report of lipoadenoma of the parotid gland without oncocytic features. We prop

Intravascular small cell neuroendocrine tumor in the neck.

A 40-year-old man presented with a painless enlarging right neck mass over several weeks without a history of trauma or infection. Ultrasound, contrast-enhanced CT, and MRI showed a mass in an expanded vein extending into the parotid gland. Blood-stained fine-needle aspiration biopsy demonstrated atypical lymphoid cells, but there was insufficient material for a diagnosis. The mass was intensely avid on 18F-FDG PET/CT and was suggestive of a neoplasm. Excision confirmed a mass within the external jugular vein with areas of invasion through the vessel wall. A diagnosis of small cell neuroendocr

Sclerosing mucoepidermoid carcinoma of the parotid gland: case report.

Mucoepidermoid carcinoma is the most common malignant salivary gland neoplasm. Its sclerosing morphologic variant, however, is an extremely rare entity. Only 7 cases have been reported in the literature, and only 2 of those reports have discussed this unusual malignancy in detail. This report describes a case involving a 57-year-old woman with an intermediate-grade sclerosing mucoepidermoid carcinoma of the parotid gland. She underwent superficial parotidectomy and radiation therapy with excellent results. Diagnosis and options for treatment are discussed, as well as a review of the literature

Cat-scratch disease simulating malignant lymphoma.

A six-year-old girl with induration, swelling and discoloration of the lower eyelid, a temporal mass, preauricular adenopathy and enlarged parotid gland, underwent biopsy, She was initially diagnosed as having a malignant disorder of histiocytic origin. All lesions resolved without therapy. Further evaluation revealed that the child had oculoglandular cat-scratch disease. Cat-scratch disease should be added to the list of nonmalignant disorders which may simulate a malignant neoplasm in its clinical and histologic appearance. Recognition of this fact is important in order to avoid erroneous di

Renal Clear Cell Carcinoma With Late Metastasis to the Submandibular Gland: A Case Report and Literature Review.

Metastasis of renal cell carcinoma (RCC) to the head and neck region is rare, accounting for approximately 8%-14% of all RCC metastases. The thyroid gland is the most common site of metastasis in this region. However, RCC is known for its ability to metastasize to unusual anatomical sites, including the salivary glands. Approximately 0.1% of metastatic salivary gland neoplasms originate from renal malignancies, and the parotid gland is the most frequently involved site. Metastases to the submandibular and sublingual glands are considerably less common. In this report, we present a 62-year-old

Parathyroid adenomas following irradiation.

Parathyroid adenomas have been demonstrated to occur following external head and neck irradiation. The median latency interval is 30 years. In a series of 74 consecutive patients with histologically diagnosed parathyroid adenomas, 25% gave a history of prior radiation exposure. When compared to a matched control incidence of 7.9%, statistical significance is reached at p less than 0.01. Thyroid abnormalities were present in 68% of the irradiated patients, and 30% of these were malignant. Tumor of skin, breast, and parotid gland also occurrred more frequently than expected.
